2 Dear Prospective Student: Thank you for your interest in the Paramedic to ADN Mobility Program at Faulkner State Community College. Enclosed you will find information to assist you with the application process. Read the entire packet carefully as an application that does not meet all of the requirements is rendered incomplete and will not be considered for admittance. Admittance is based on a point system and it is competitive; therefore, it is imperative for you to pay particular attention to the point system. There is no waiting list. Use this information packet as your guide for successful entry into the program. The Paramedic to ADN Mobility Program students attend classes in the Tyson Building on the Bay Minette Campus for one to two days per week. Additional time is required for clinicals. The required number of hours per week varies with each nursing course. This program is a daytime course with set meeting times. Course times are not scheduled around rotating shifts. It is advisable to complete all general education course requirements prior to the start of nursing courses. Reliable transportation, a flexible schedule, and willingness to commute to several area hospitals are required. You may call the Nursing Department at (251) if you have further questions. 4 MINIMUM ADMISSION STANDARDS INCLUDE: 1. Unconditional admission to the college (Application must be updated if student did not attend Faulkner State last semester). Original transcripts from ALL colleges and high school attended must be in the Admissions Office by the deadline (transcript must be sent by the institution s registrar directly to Faulkner State s Registrar). 2. Submission of completed nursing application to the Nursing Department between April 1-June Minimum of 2.5 GPA on a 4.0 scale based on the last 24 credit hours of college coursework completed Good standing with Faulkner State (minimum cumulative 2.0 GPA) Meeting the Alabama Community College System Essential Functions for nursing 6. 5 TEAS NURSING ENTRANCE EXAM The Test of Essential Academic Skills (TEAS) measures basic essential skills in the academic content areas of reading, mathematics, science, and English and language usage. Reading is 58 minutes with 48 questions Mathematics is 51 minutes with 34 questions Science is 66 minutes with 54 questions English and Language usage is 34 minutes with 34 questions Total time limit for testing is 209 minutes. The TEAS is composed of a total of 150 multiplechoice questions, each with four options. Questions not answered count against the student. Registering for TEAS All individuals preparing to take the TEAS must create an ATI account to register for the test. Create an account by going to ATI s website at and click on Create a New Account then, follow the screen prompts. Registration is only required once. Click the tab for online store to order the TEAS exam. All tests are given by the Student Development Office located on the Bay Minette Campus in the Memorial Hall Building (same location as Admissions and Financial Aid). Bring ATI ID and a pencil to test. No calculators are allowed. The cost for the TEAS is $56. Payment is made online at the time of registration for the test. Preparing for the TEAS The following items are available for purchase at for an additional cost: Learning Strategies: Your Guide to Classroom and Test-Taking Success TEAS Pre-Test Study Manual TEAS Online Practice Test (2 versions) Rescheduling A notice of 48 hours is required to reschedule a testing date. Failure to notify ATI within 48 hours constitutes forfeit of the registration testing fee. Retesting The TEAS test may be repeated once during any semester. The student s highest score may be submitted with the nursing application, provided it is within the three-year time frame. The TEAS score is good for three years. 5
6 POINT SYSTEM TO DETERMINE ADMITTANCE Paramedic to ADN applicants are rank-ordered using the point system below. Those with the most points are offered admittance into the program if they have met all other criteria. 1. Score from the Nursing Entrance Exam, TEAS V, (150 points possible). TEAS V must have been taken within the past 3 years for consideration. The test is scored by a percentage. Take the percent times 150 points to determine your score, i.e. a score of 68% is.68 x 150 = 102 points 2. *Points for Grades in Select College Courses (90 points possible) Grade A B C BIO 201, Anatomy &Physiology I 30pts 20pts 10pts BIO 202, Anatomy &Physiology II 30pts 20pts 10pts BIO 220, Microbiology 30pts 20pts 10pts For example, a student who has an A in Anatomy I, a B in Anatomy II, and a B in Microbiology has 70 points. Students with A s and B s are generally more competitive and have a much higher chance of gaining admittance into the program because of the higher number of points. 3. Additional Points 1 point each for completion of the following courses with a C or higher: English Comp I, Intermediate College Algebra, General Psychology, Human Growth & Development (or Developmental Psych), Fundamentals of Speech, and a Humanities Elective such as Art or Music Appreciation, Religion, Foreign Language, Philosophy, or Ethics. (6 points possible) 5 points for completion of Chemistry, or a higher level Math such as Pre- Cal, Finite Math, or Statistics, with a C or higher. (5 points possible) Total Score A total of 251 points are possible with these selection criteria. 6

Students must seek permission from the Biology Chairperson to waive the BIO 103 prerequisite. ORI 101 Orientation to College is required for those with no previous college coursework. 8 ANTICIPATED EXPENSES FOR COMPLETION OF DEGREE The following is an approximate list of expenses that can be expected throughout the Nursing program. Keep in mind that these expenses are estimates only and may vary (immunizations, physical, etc.). Included in this testing are examinations to evaluate areas that are essential for academic success: 1. Diagnostic and Achievement Tests help to identify your strengths and areas which need remediation in comprehending the content of the courses. 2. Pre-RN mock licensure examinations provide reliable predictors of success for the NCLEX (National Council Licensure Exam). 4. Specific examinations may be required as a part of progression in identified nursing courses. NOTES REGARDING OTHER ENROLLMENT REQUIREMENTS After acceptance to the Nursing Program, students will be required to provide the following: Current History & Physical Evaluation (Form provided by nursing department) Current two-man professional CPR Certification (Classes offered through nursing department) Documentation of immunizations (Form provided by nursing department) Proof of having medical insurance (Student plan available for purchase) Drug screen and background check Ability to meet all Essential Functions with or without reasonable accommodations Computer with internet access HIV or HEPATITIS B REPORTING POLICY Public law # , Section 633, and The Alabama Infected Health Care Worker Management Act require HIV or Hepatitis B infected healthcare workers to notify the State Health Officer within 30 days of the time the diagnosis is confirmed. Physicians caring for HIV or Hepatitis B infected healthcare workers are mandated to notify state health officials within 7 days of the diagnosis. All students are required to comply with this law. 8
9 ALABAMA COMMUNITY COLLEGE SYSTEM NURSING ESSENTIAL FUNCTIONS The Alabama College System endorses the Americans' with Disabilities Act. In accordance with College policy, when requested, reasonable accommodations may be provided for individuals with disabilities. Physical, cognitive, psychomotor, affective and social abilities are required in unique combinations to provide safe and effective nursing care. The applicant/student must be able to meet the essential functions with or without reasonable accommodations throughout the program of learning. Admission, progression and graduation are contingent upon one's ability to demonstrate the essential functions delineated for the nursing programs with or without reasonable accommodations. The nursing programs and/or its affiliated clinical agencies may identify additional essential functions. The nursing programs reserve the right to amend the essential functions as deemed necessary. In order to be admitted and to progress in the nursing program one must possess a functional level of ability to perform the duties required of a nurse. Admission or progression may be denied if a student is unable to demonstrate the essential functions with or without reasonable accommodations. The essential functions delineated are those deemed necessary the Alabama College System nursing programs. No representation regarding industrial standards is implied. Similarly, any reasonable accommodations made will be determined and applied to the respective nursing program and may vary from reasonable accommodations made by healthcare employers. The essential functions delineated below are necessary for nursing program admission, progression and graduation and for the provision of safe and effective nursing care. Individuals will be asked to provide documentation of the disability in order to assist with the provision of appropriate reasonable accommodations. FSCC will provide reasonable accommodations, but is not required to substantially alter the requirements or nature of the program or provide accommodations that inflict an undue burden on the College. Requests for reasonable accommodations should be directed to: The Dean of Student Development, (251) REQUIREMENTS FOR LICENSURE The Alabama Board of Nursing will require applicants to answer the following questions when applying to take the licensure examination at the conclusion of a nursing program of study. Eligibility to take the examination is the decision of the ABN; graduation from an accredited school such as FSCC does not ensure eligibility. 1. Have you ever been arrested for, been charged with, been convicted of, entered a plea of guilty to, entered a plea of nolo contendre or no contest for, received deferred prosecution or adjudication for, had judgment withheld for, received pretrial diversion for, or pleaded not guilty by reason of insanity or mental defect to any crime other than a minor traffic violation in any state, territory, or country? A crime related to driving while impaired or while under the influence of any substance is not a minor traffic violation. 2. In the past five years, have you abused alcohol, drugs (whether legal or illegal, prescribed or unauthorized), and/or other chemical substances or received treatment or been recommended for treatment for dependency to alcohol, drugs (whether legal or illegal, prescribed or unauthorized), and/or other chemical substances? 3. Have you ever been arrested or convicted for driving under the influence of drugs/alcohol? 4. In the past five years, have you had, or do you now have, a physical or mental health problem that may impair your ability to provide safe nursing care? 5. Has the licensing authority of any state, territory, or country denied, revoked, suspended, reprimanded, fined, accepted your surrender of, restricted, limited, placed on probation, or in any other way disciplined your nursing and/or any other occupational license, registration, certification, or approval? 6. Is the Board of Nursing or other licensing authority of any state, territory, or country, including but not limited to the Alabama Board of Nursing, currently investigating you? 7. Is disciplinary action pending against you with the Board of Nursing or other licensing authority of any state, territory, or country, including but not limited to the Alabama Board of Nursing? 8. Have you ever been placed on a state and/or federal abuse registry? 9. Has any branch of the armed services ever administratively discharged you with any characterization of service besides Honorable and/or court-martialed you? A yes answer will not necessarily prevent you from eligibility but will require submission of an explanation accompanied by certified documents. 11
