PerformPlus gives you: n All the flexibility you need to design a plan that s tailored to your needs and those of your employees n An insurance plan that s easy to manage, thanks to the tools we give to your plan administrators n An insurance plan that s practical for your employees, with a wide range of tools and services To custom build your own PerformPlus group insurance plan: n You must have between 3 and 49 plan members n Your plan must include basic life insurance for all plan members n Your plan must include at least one other benefit offered under PerformPlus After that, you can add as many additional benefits as you like to meet your needs and those of your employees. 2 PerformPlus

5 PerformPlus services Our services add value to your plan. Some are automatically included with PerformPlus and others are add-ons to other benefits. You can also select individual services à la carte. OUR ONLINE SERVICES Automatically included in all PerformPlus plans Our online services make group plan administration easier and include all the tools plan members and administrators need. For plan administrators Our secure site is a user-friendly way to quickly and efficiently perform virtually every transaction involved in managing your PerformPlus plan: View plan member profile summaries Add new plan members Manage absences Manage monthly billing Get plan management reports and forms View your transaction history For plan members PAYMENT CARD Included with the extended healthcare (drug coverage) and dental benefits With the direct payment card, pharmacists and dentists can immediately tell plan members or insured dependants how much is covered by their plan. Eligible reimbursable expenses are deducted on the spot, and the plan member then pays the difference (i.e., the uninsured portion of the expenses). It is also useful for insureds to carry the payment card with them when travelling, both at home and abroad. In case of an emergency, it contains the contact information for the Travel Assistance service, which they can contact anytime, anywhere in the world. TRAVEL ASSISTANCE Included with the extended healthcare benefit The Travel Assistance service offers personal on-the-spot medical assistance to plan members and their dependants worldwide, 24 hours a day, 7 days a week. The service makes the necessary arrangements to help them deal with a medical emergency: it can direct them to a doctor or healthcare facility, cover advances required by a hospital, locate interpretation services, repatriate them to their city of residence, connect them with a lawyer in case of legal issues, and more. Our secure site offers plan members a variety of features, including: Online claims for a range of extended healthcare services, including vision care An online drug cost simulator Pre-filled forms Management of dependant information and coordination of benefits Enrolment in our Direct Deposit and Electronic Notice service Printable payment card Printable reports for income tax purposes Access to the Health is Cool 360 platform (if included in your plan) Plan members registered for the secure site can also download our Claim 360 app and use their tablet or smartphone to submit claims for care or services administered by a healthcare professional (including vision care). With the pay-direct claims service, certain extended healthcare providers can submit claims directly to us on behalf of their patients. For your plan members, that means quick and secure claims processing and no more paper forms. And if your plan includes a travel insurance option, your plan members can download the Desjardins Travel Solution app and take it with them wherever they go for immediate emergency assistance. 6 HEALTH IS COOL 360 PLATFORM Automatically included with the long-term disability benefit Accessible online or by phone, this platform contains a variety of reliable tools and up-to-date resources to help employees make informed decisions about their health and wellness. Some of its features: A health evaluation tool that plan members can use to identify their health risks and generate a personalized action plan to help them reduce or avoid these risks Health PACT, a counselling service, available online and by phone, designed to reduce absenteeism and keep plan costs down. It helps employees understand medical conditions that require special attention and encourages them to deal with health problems proactively A list of local services and wait times in the Canadian healthcare system Information on drugs and treatment options for specific illnesses Information on how to cope with a chronic illness or disability Comprehensive directories and a variety of health-related articles HEALTH SPENDING ACCOUNT Optional service A Health Spending Account (HSA) is the perfect add-on to a group insurance plan. You deposit a certain number of credits for each employee at the beginning of the year, and they use the credits to pay for health or dental expenses (for themselves or their dependants) that aren t covered by their provincial health insurance or any other group insurance plan. There are a number of advantages to an HSA. It helps you manage plan costs more effectively and makes your employees more aware of health costs.
