Zurich Insurance Middle East FGB Credit Shield Credit Card Insurance Benefits and Exclusions Credit Shield Benefits Death The amount of Cardholder s indebtedness at the date of death up to AED 100,000 or credit limit whichever is lower. Permanent Total Disability The amount of Cardholder s indebtedness from the date of permanent total disablement up to AED 100,000 or credit limit whichever is lower. The benefit will be paid after 6 consecutive months of total disability recognized by a physician authorized by the company. Involuntary Loss of Employment The amount equaling to 10% of Cardholder s indebtedness, subject to a maximum amount of AED 4,000 payable for every month of unemployment up to a maximum period of 12 months Definitions Certain words have specific meanings when they appear in this policy. These meanings are shown below. They are printed in bold type. Any other words not appearing in the definition section will follow the English language definition: Accident means a sudden and unforeseen specific event that happens unexpectedly caused solely and directly by violent external and visible means which occurs at an identifiable time and place resulting in Injury. Benefit means the risks covered under this policy as specified in the Policy Schedule. Beneficiary shall mean the person(s) or entity receiving the Benefit as detailed in the Policy Schedule. Cardholder means a primary account holder of the Credit Card Facility with the Policyholder who meets the Eligibility Criteria to receive the Benefits under this Policy. This definition excludes corporate credit cards. Claim means the occurrence of an event that triggers one or more of the Benefits. Commencement Date means the date the Cardholder-having applied for Benefits under this policy, is accepted and granted coverage by Us. Credit means the credit or other financial arrangement authorized by the Policyholder and provided to the Cardholder under a Credit Card Facility. Credit Card Facility means the Policyholder s Credit Card Facility which has been nominated as the facility to which the Benefits under this Policy are to apply. Date of Event shall mean the date to be considered for the purpose of establishing if a Claim is payable under the policy. The Date of Event in respect of each of the risks covered is specified in the Special Conditions. Eligibility Criteria means the conditions that the Cardholder must meet to be insured under this policy. Illness means a sickness or disease of the Cardholder. Indebtedness means the total amount outstanding on the Cardholder s Credit Card Facility on the Date of Event excluding any Credit facility provided after the Date of Event subject to the Cardholder s credit limit. Injury means bodily injury sustained in an Accident directly and independently of all other causes. Insurance Period means the dates during which this policy is operative as detailed in the Policy Schedule. Medical Practitioner means a person who: (a) (b) (c) (d) Is acceptable to us in case of death and critical illness Is legally qualified by degree in western medicine and legally authorized in the geographical area of his or her practice to render medical and surgical services. Is a specialist in an area of medicine appropriate to the cause of the Claim Is not a member of the Cardholder s family Policyholder means the bank named in the Policy Schedule. Policy Schedule means the issued policy document outlining information and policy details such as, Benefits, Sums Insured and limits. Covers only apply if shown in the Policy Schedule and up to the maximum amount shown thereon. Pre-existing Condition means any Injury or Illness: (a) Which existed before the Commencement Date, which presented signs or symptoms of which the Cardholder was aware, or (b) For which treatment, or medication, or advice, or diagnosis has been sought or received during the two years prior to the Commencement Date by the Cardholder from a Medical Practitioner, chiropractor, osteopath or any other practitioner of a similar kind, or (c) Which was known by the Cardholder to exist prior to the Commencement Date whether or not treatment, or medication, or advice, or diagnosis was sought or received. Scope of Coverage means the context of occurrence or time in which a Benefit applies as specified in the Policy Schedule. Where this is not specified, it is assumed that the Scope of Coverage is 24 hours a day. Sum Insured means the amount payable under each cover of the Policy Schedule and in accordance with the policy terms and conditions. Terrorism shall mean an act of terrorism and includes any act, preparation or threat of action of any person or group(s) of persons whether acting alone or on behalf of or in connection with any organization(s) or government(s) de jure or de facto committed for political, religious, ideological, or similar purposes, including the intention to influence any government de jure or de facto of any nation or any political division thereof and/or to intimidate the public or any section of the public of any nation and which: (a) involves violence against one or more persons; or (b) involves damage to property; or (c) endangers life other than that of the person committing the action; or (d) creates a risk to the health or safety of the public or a section of the public; or (e) is designed to interfere with or disrupt an electronic system. Waiting Period means a period of time consisting of consecutive days counted from the Commencement Date. Claims with a Date of Event during the Waiting Period are not valid. We/Us/Our/Ours/The Company means Zurich Insurance Middle East S.A.L. and/or Zurich International Life Limited Article 1: General Conditions The following provisions apply to this insurance: 1. Entire Contract. No agent or other person has the authority to change or waive any provision of this policy. No changes in this policy shall be valid unless approved by Us and evidenced by endorsement. 2. Marketing Page 1 of 6

Should any discrepancies arise between the policy and any literature received by the Policyholder, the terms, conditions, endorsements and exclusions in the policy will govern in all cases. 3. Age Limit The insurance shall apply to a Cardholder within the age limits as stated in the Policy Schedule and the Special Conditions for each risk covered as of the Commencement Date. 4. Maximum Liability Our maximum liability in respect of any one Claim and any one Benefit per Cardholder shall not exceed the Maximum Amount Payable as stated in the Policy Schedule under this policy. 5. Misrepresentation If the Cardholder, or anyone acting for the Cardholder, or the Policyholder makes a statement in the application form or in connection with any claim knowing the statement is false or uses any fraudulent means or devices to obtain any benefit under this policy We will not be liable for any Claim. All covers under this policy shall cease and no premium will be refunded. This policy shall be considered void at Our discretion in the event of misrepresentation, mis-description or non-disclosure by or on behalf of the Cardholder or Policyholder of any information material to this policy. 6. Compliance The Cardholder and Policyholder must follow Our advice or instruction otherwise We may decline to pay the whole or any part of the Claim. Failure to comply with any of the provisions contained in this policy shall invalidate all claims hereunder. 8. Cardholder Coverage Coverage under this policy starts automatically in respect to each Cardholder as of the Commencement Date. The Cardholder has the option to opt out of the scheme at any time. In case the Cardholder decides to enrol again, he/she will be submitted again to the Underwriting process and a new Commencement Date and a new Waiting Period will apply. Such enrolment has to be approved by Us. Coverage in respect to a Cardholder shall automatically terminate at the earliest of the dates specified below: The premium due date when any or any part of the premium pertaining to this policy is not paid; (ii)the expiry date of the policy, (iii )if the right to Cancellation is exercised by either party. (iv) In respect to any specific risk covered, the date the Cardholder no longer meets the Eligibility Criteria for that coverage. (v) In respect to any specific Cardholder, the last day of the monthly loan amortization cycle during which the Cardholder opts out of the scheme. (vi) The Loan object of this policy is repaid, recovered or otherwise cancelled, regardless of the reason. (vii) Subject to the above Provision 7 - Misrepresentation; () Cancellation of the Cardholder s Credit Card Facility. However, the cancellation of cover shall be effective only from the 90th day of cancellation of the respective Credit Card Facility. (iii) Upon payment of a claim under any benefits listed in the Policy Schedule (iv) The Cardholder becomes a defaulter for a period of 90 consecutive days. However, this policy will be automatically reinstated once the Cardholder has paid his dues; (v) Cardholder becoming unemployed voluntarily. However, the termination of cover in such event shall be only in respect of Section 3 Involuntary Loss of Employment; (vi) Six months prior to the Cardholder s normal retirement age depending upon the age of the Cardholder and the law of the land where he is employed. However, the termination of cover in such event shall be only in respect of Involuntary Loss of Employment; (vii) Cancellation of the benefits under this policy by the Insured or the Cardholder at any time in accordance with the terms and conditions of this policy. 9. Governing Law and Jurisdiction The interpretation of this Policy is governed by and shall be construed in accordance with the laws of the United Arab Emirates. In the event of a dispute or difference between Us and the Policyholder as to the interpretation, effect, or validity of this Policy the parties agree to submit to the exclusive jurisdiction of the courts of the United Arab Emirates, unless a valid arbitration agreement has been entered into between the Policyholder and Us, in which case We and the Policyholder agree that any dispute or issue arising under or in respect of this Policy, including in relation to the breach, termination or validity of the Policy, shall be referred to arbitration in accordance with the provisions of the arbitration agreement. 10.Language This policy has been written in both the English and Arabic languages. In the event of any discrepancy in interpreting this policy, the Arabic language will prevail. 11. Statement of Purpose for Collection of Personal Data All the personal information collected or held by Us, howsoever obtained, may be used by Us or disclosed to any individual or organization within or outside United Arab Emirates for the following purposes: (1) to assess and service this policy, (2) to provide marketing material for Us or Our associated companies (3) to conduct insurance claims or analysis. 12. Data Furnished by the Policyholder The Policyholder shall maintain, and shall furnish to Us upon request, the names of all persons initially insured, of all new persons who become insured and of all persons whose insurance is cancelled, together with the data necessary for the calculation of premium. Article 2- Eligibility Criteria A Cardholder must meet the following conditions to be insured under this policy: 1. Meet the eligibility criteria of the Policyholder in order to become a Cardholder. 2. Be a Cardholder of the Policyholder 3. Be proposed to Us by the Policyholder, apply for Benefits under this policy and be accepted by Our underwriting assessment 4. Not have been removed from the Policy by the Policyholder 5. Meet the Eligibility Criteria of the different covers under this policy as outlined in the Special Conditions 6. The benefits under this policy shall be extended only to Primary Cardholders and not to an additional or supplementary Cardholder Article 3: Exclusions The following exclusions apply to all risks covered under this policy. We will not cover losses arising directly or indirectly from: 1. Any illegal or unlawful act by the Cardholder. 2. Any Pre-existing Condition, congenital or hereditary condition but this exclusion shall not apply in respect of death occurring after a continuous period as defined in the Special Conditions from the Commencement Date in respect of the particular Cardholder 3. Claims with Date of Event prior to the Commencement Date, during the Waiting Period or outside of the Insurance Period. 4. Suicide (as defined in the UAE Civil Code), attempted suicide or any deliberate or self-inflicted injury within 12 months of the Commencement Date. 5. Claims incurred by Cardholders that do not meet the Eligibility Criteria. 6. Alcohol abuse, alcoholism, substance abuse, solvent abuse, drug abuse or addictive conditions of any kind, other than the proper use of medications regularly prescribed by a Medical Practitioner. 7. Air travel where the Cardholder is other than a fare-paying passenger on a regular scheduled airline or licensed chartered aircraft. 8. Engaging in any kind of occupational activities underground or aerial photography; handling or requiring the use of explosives; naval, military or air force service or operations or armed force services (including being in service or on duty with or undergoing training with any military or police force, or militia or paramilitary organization). 9. Engaging in a sport in a professional capacity or where the Cardholder would or could earn income or remuneration from engaging in such sport. 10. Dangerous sports even as an amateur, such as: mountaineering or rock climbing, bungee jumping, scuba diving, pot holing or other underground activities, BMX stunt riding, boxing, gymnastics, high diving, martial arts, micro-lighting, any kind of motor racing, outdoor endurance or outward bound courses, show jumping, stunt events, water ski jumping, white water rafting or wrestling any loss, attributable to HIV (Human Immunodeficiency Virus) and/or HIVrelated illness including AIDS and/or any mutant derivative or variations thereof however caused or however named. 11. Any event arising from war, invasion, act of foreign enemy, hostilities (whether War is declared or not), civil war, rebellion, revolution, insurrection, labour disturbances, riot, strike or lockout, military force or coup. 12. Any loss of or damage directly or indirectly arising from nuclear radiation, nuclear fission, nuclear fusion and/or any kind of radioactive contamination, as well as the dispersal or application of pathogenic, poisonous, biological or chemical material. 13. Any terrorist act or bomb incident or threat thereof as well as the intentional use of military force to intercept, prevent, or mitigate any known or suspected terrorist act. 14. Illness occurring within 30 days of the Commencement Date 15. Death or Injury resulting from childbirth or pregnancy related issues 16. All cover and payments under the policy will be subject to any prohibition or embargo that may apply under Relevant Sanctions Laws. Relevant Sanction Laws under this contract are defined as follows - any law, regulation or similar rule relating to trade sanctions and/or embargoes imposed by any authority in/of: (i) countries where any party to this contract is resident (United Arab Emirates), domiciled or otherwise business active, (ii) the United States of America including any federally controlled territory, (iii) the United Kingdom, (iv) the United Nations, and (v) the European Community. 17. Notwithstanding any other terms under this insurance contract, no insurer shall be deemed to provide coverage or will make any payments or provide any service or benefit to any Policyholder&/or Cardholder or other party to the extent that such cover, payment, service, benefit and/or any business or activity of the Policyholder&/or Cardholder would violate any applicable trade or economic sanctions law or regulation.. Article 4: Claims The following applies to Claims that may arise under this policy: 1. Notice of Claims If during the Insurance Period an event occurs which may result in a Claim under this policy the Cardholder or their legal representatives must follow the following procedure: 1. Give timely written notice to Us within the time frames specified in the Special Conditions for each covered risk. 2. Complete the claim form provided by Us. 3. Provide evidence of Claim in the form of documents indicated in the Special Conditions for each covered risk as well as additional documents to substantiate the Claim as We may reasonably require. 3. Proof of Loss Written proof of loss must be furnished to Us together with the claim form provided by Us. Failure to furnish such proof within the time required shall not invalidate any claims if it was not reasonably practicable to give proof at such time, provided that such proof is furnished as soon as reasonably practicable, and in no event later than ninety days from the time when such proof is otherwise required. All certificates, information and evidence in such form and of such nature and within such time as We may reasonably require shall be furnished without expense to Us. The onus will always be on the Policy Holder to provide sufficient supporting documents deemed necessary by Us to process the claim. Failure to do so may result in appraising the claim to Our own discretion and decision. We may refuse to provide Benefits to the Cardholder if the Cardholder cannot provide sufficient supporting documents deemed necessary by Us to process the claim. 4. Medical Records and Examination We reserve the right to access any of the Cardholder s current or prior medical records in order to finalize and/or proceed with the assessment of a Claim. We shall be entitled in the case of non-fatal injury to call for examination by a medical referee appointed by Us whenever required and in the event of death to have a post-mortem examination at Our expense. Article 8: Context Throughout the Policy where the context allows, words in the singular shall include the plural, the masculine gender shall include the feminine, and vice versa. Page 3 of 6

Special Conditions - Death Article 1: Object of the Cover The cover hereon has the following purpose: If after the Commencement Date and during the Insurance Period a Cardholder dies the Beneficiary will be paid an amount equal to the Indebtedness of the Cardholder up to AED 100,000 or credit limit; whichever is lower. Article 2: Provisions Date of Event: 1.The date of death of the Cardholder. 2. Non Cumulative Benefits: Any Benefit payable under this cover should be reduced by any amount paid under any other cover that reduced the Indebtedness of the Cardholder. 3. Waiting Period: This benefit is subject to a nil Waiting Period Article 3: Eligibility Criteria The Cardholder s age is between 18 years and 65 years (maximum coverage age). Article 4: Exclusions Illness due to pre-existing condition but this exclusion shall not apply in respect of Death occurring after a continuous period of 6 months from the Commencement Date in respect of the particular Cardholder. However, this exclusion is applicable only if we are able to substantiate pre-existing condition within 1 month from the date of submission of all required claims documentation to the Company. Article 5: Claims Claim Definition: Claim means a request for payment under this Policy 1. Timeliness of Notice: Claims under this cover must be reported to Us no later than ninety days from the Date of Event. 2. Documents to be provided: In order to process claims under this cover We need a copy of the following documents: 1. The Death Certificate. 2. The national identity card or a copy of the passport and visa page if applicable. 3. The Post Mortem Report (wherever legally required). 4. In case of an Accident triggering the Death, the Police Report on the Accident that originated the Claim. 5. The Medical Report including a detailed diagnosis with cause of death if required where the cause is not clearly mentioned in the Death Certificate. 6. Credit Card Facility outstanding statement at the Date of the Event. 7. Any other documents which may be required by us. Special Conditions Permanent Total Disablement Definitions Permanent Total Disablement means that the Cardholder is rendered unable to earn income in any occupation, trade or profession. The disability should be for a period of six consecutive months and at the end of that period, it should be in the opinion of a Medical Practitioner/ in our Opinion beyond reasonable hope of improvement, total, continuous and permanent. Article 1: Object of the Cover The cover hereon has the following purpose: Permanent Total Disablement means that the Cardholder is rendered unable to earn income in any occupation, trade or profession. The disability should be for a period of six consecutive months and at the end of that period, it should be in the opinion of a Medical Practitioner/ in our Opinion beyond reasonable hope of improvement, total, continuous and permanent. In that case, the Beneficiary will be paid an amount equal to the Indebtedness of the Cardholder up to AED 100,000 or credit limit; whichever is lower. Article 2: Provisions 1. Date of Event: The date of recognition of Permanent Total Disablement by a Medical Practitioner. 2. Non-Cumulative Benefits: Any Benefit payable under this cover should be reduced by any amount paid under any other cover that reduced the Indebtedness of the Borrower. 3. Waiting Period: This benefit is subject to a nil Waiting Period 4. Waiver of the Consolidation Period: In case of total and irrevocable loss of sight of both eyes, or the loss or severance of two or more limbs (at or above wrist or ankle), the consolidation period of twenty six weeks can be waived by Us and the state of Permanent Total Disablement recognized immediately. Article 3: Eligibility Criteria The Cardholder is aged between 18 years and 65 years (maximum coverage age). Article 4: Exclusions 1. Insanity, mental or nervous disorders including, but not limited to, anxiety disorders, eating disorders, psychotic disorders, affective disorders, personality disorders, substance use disorders, somatoform disorders, dissociate disorders, psychosexual disorders, adjustment disorders, organic mental disorders, mental retardation and autism. 2. Any condition resulting from pregnancy, childbirth or miscarriage, abortion, pre-natal care as well as post-natal care and other complications arising thereof, venereal disease, services and supplies related to the diagnosis or treatment of infertility or other problems related to inability to conceive a child; birth control, including surgical procedures and devices. 3. Suicide, attempted suicide or any deliberate or self-inflicted injury Article 5: Claims 1. Timeliness of Notice: Claims under this cover must be reported to Us no later than ninety days from the Date of Event. 3. In case of an Accident triggering the Disability, the Police Report on the Accident that originated the Claim. 4. Disability Certificate from U.A.E Medical board. 5. A copy of all medical documents including hospitalisation and surgery reports, histology reports, specialists reports, laboratory reports and any other related to the claim. 6. Credit Card Facility outstanding statement at the Date of the Event. 7. Any other documents which may be required by us. 3. Evaluation of Disablement: We reserve the right to evaluate the Total Permanent Disablement in the light of the documents provided and appoint a Medical Practitioner to confirm the validity of the Claim and the relevant Date of Event. Special Conditions Involuntarily Loss of Employment Definitions Excess Period means a period of time consisting in consecutive days counted from the Date of Event for which Benefits are not payable. Involuntary Loss of Employment means unemployment of the Cardholder arising out of the unilateral decision of the Cardholder s employer to terminate the Cardholder s employment contract for any reason other than those excluded under Article 4 hereon. Minimum Payment means the minimum amount that must be paid into a Credit Card Facility on or before an established date to avoid a financial penalty. Full Time Permanent Employment means employment under a contract that has no stated expiry period and that requires the Cardholder to work at least 35 hours per week. Article 1: Object of the Cover The cover hereon has the following purpose: If after the Commencement Date a Cardholder is subject to Involuntary Loss of Employment during the Insurance Period, We will pay the Beneficiary amounts equal to 10% of the Indebtedness subject to the maximum of AED 4,000payable for every month of unemployment and up to a maximum period of 12 months. Article 2: Provisions 1. Date of Event: The date when the Cardholder should have received a salary payment under the employment contract that was terminated. 2. Non Cumulative Benefits: Any Benefit payable under this cover should be reduced by any amount paid under any other cover that reduced the Indebtedness of the Cardholder. 3. Waiting Period: This benefit is subject to a 90 days (ninety) Waiting Period. 1. The Cardholder is aged between 18 years and 65 years (maximum coverage age). 2. The Cardholder has been employed with the same employer for at least six continuous months. 3. The Cardholder has a Full Time Permanent Employment contract with his or her employer. Article 4: Exclusions 1. Cardholders who are on probation with their employer. 2. Cardholders who are on a fixed term contract for less than 2 years or part time or temporary employment. 3. Cardholders who are employed as part time or temporary employees. 4. Resignation or leaving by mutual agreement or voluntary unemployment or redundancy. 5. Disability, Accident or Illness or any other medical reasons either mental or physical. 6. Where the unemployment is a normal seasonal part of the employment or due to non-renewal of employment contract by the authorities. 7. Where the Cardholder was aware of pending unemployment on or before the Commencement Date. 8. Where the Cardholder has neither been terminated nor become redundant but his or her salary or allowances are being withheld in part of in full for any reason of the employment contract. 9. Unemployment due to any of the following reasons; a. Misconduct b. Refusal to accept orders from superiors c. Conviction of a crime d. Dishonesty or fraudulent act e. Non-performance or under performance 10. The period for which payment from the employer is received instead of working notice. 11. Termination of employment due to voluntary retirement. 12. Emiratisation. Dismissal during any trial or employment probationary period 13. Self-employment. 14. If the Cardholder does not accept employment from the employer for a different but suitable job. 15. Dismissal from employment arising outside of the United Arab Emirates. 16. Cardholders who are not resident in the United Arab Emirates. 17. Dismissal or redundancy when the employer is a next of kin or family member of the Cardholder or where the Cardholder is a majority shareholder of the company or a member of its board. 18. Strikes, lockouts or other organized labour disputes or any unlawful acts. 19. Company failure where a contributing cause was a natural catastrophic event, war or warlike event. Article 5: Claims Procedure 4. Excess Period: This cover is subject to a nil Excess Period. 5. Payment Criteria: The first Benefit payment object of this cover is the first Payment due after the Waiting Period runs out, provided that the Cardholder is still unemployed at that time. We will continue to pay subsequent Payments as they become due while the Cardholder remains unemployed up to the maximum number of payments stated in this Policy. 6. No Partial Payments No partial payments will be made for portions of a month during which the Cardholder was unemployed but was subsequently re-employed before the Payment was due. 7. Mutually Exclusive Covers The Involuntary Loss of Employment cover is available to Cardholders that have an employee-employer relationship and therefore are not eligible to be covered by the Temporary Total Disablement cover. Article 3: Eligibility Criteria Page 5 of 6 1. Timeliness of Notice: Claims under this cover must be reported to Us no later than sixty days from the Date of Event. The onus will always be on the Cardholder to provide sufficient supporting documents deemed necessary by Us to process the claim. Failure to do so may result in appraising the claim to Our own discretion and decision. 2. Documents to be provided: In order to process claims under this cover We need a copy of the following documents: 1. The national identity card or a copy of the passport and visa page. 2. Notice of termination from the Cardholder s employer. 3. Proof of full time employment on the employer s letterhead including a copy of the employment agreement between the employer and Cardholder clearly stating that the Cardholder was employed by the employer on a full time basis. We may also request a copy of the Labour contract from the Cardholder s employer if it is required to verify the period of the employment contract. 4. Three months Credit Card Facility outstanding statement at the Date of the Event. 5. Three months preceding bank statements showing salary credit

Persistency of Unemployment: Benefits under this cover are payable while the state of unemployment persists - up to the maximum number of payments stated in the Policy Schedule. We reserve the right to require additional documentation after the Claim is first approved and the first payment is made to the Policyholder to verify that the Cardholder is still unemployed.
