MAINEGENERAL HEALTH SCHOLARSHIP APPLICATION FORM 2015 Application must be received by the Office of Philanthropy no later than Wednesday, April 8, 2015. Our Scholarship Program: To support the education of those pursuing careers in direct-service health care professions at MaineGeneral. MaineGeneral Health (MGH) and individual sponsors offer many scholarships. Selected students receive this financial aid based on scholastic achievement (must have minimum current GPA of 2.5, or 85 for high school students), financial need, civic contributions, acceptance at an accredited school or licensed program and residence within the MaineGeneral Health service area. Your application will be considered for all MaineGeneral scholarships. Questions: 626-7238 are given toward academic degrees in direct-service health care disciplines practiced at MGH. INSTRUCTIONS Only COMPLETE applications will be considered. This includes (check off as you complete your application): An up-to-date transcript including your most recent grades. Two letters of recommendation from people who know you well, but are not related to you, such as teachers, employers, clergy, etc. Letters must be dated and no more than one year old at the time of submission. Be sure to request your letters of recommendation with plenty of advance notice. You will not be contacted if your letters are not received by the deadline and your application will be incomplete without them. We strongly encourage you to include your letters with your application if possible. A neatly typed or printed Personal Story (see instructions on page 2). If you have applied for a federal loan, a copy of the entire completed application (FAFSA or SAR) must be included. A neat, complete, original application (faxed or emailed applications will not be accepted), with all required fields filled in. Please type or print all your answers. A blank answer field means your application is incomplete. If a section is not applicable, please mark it so. Scholarship recipients are selected in the spring and payment is made after successful completion of the fall semester. Available Health Care Sandie Belden Memorial Scholarship Awarded to a Winthrop High School student pursuing a career in health care. Available MaineGeneral Health Scholarship Awarded to students or employees within MaineGeneral Health who are pursuing health care careers. Available MaineGeneral Medical Center Auxiliary Scholarship Awarded to students pursuing a career in health care. Available MaineGeneral Medical Center Scholarship Endowment Awarded to medical center employees or their family members who are pursuing educational opportunities in professions in which MaineGeneral Medical Center administration believes there is a demonstrated need. Available Pauline N. Young Memorial Scholarship This is available only as a supplement to the MaineGeneral Medical Center Scholarship Endowment. Available

General Nursing Mary-Grace Bean Memorial Scholarship Fund This is available only as a supplement to the MaineGeneral Medical Center scholarship. Available Florida C. Lewin Memorial Scholarship Awarded to nursing students from the Augusta area. Available in even-numbered years only. The Michele Joly Memorial Scholarship Awarded to nursing students. Available Robert A. Longo Memorial Scholarship For health care students either changing careers or returning to school after being away for a number of years. Ottie T. McCullum, Sr. Memorial Scholarship Awarded to students from Augusta who are pursuing a nursing career. Available in even-numbered years only. Antoinette Stevens, RN Memorial Scholarship Awarded to nursing students. Available in even-numbered years only. Joan H. Melendy Nursing Scholarship Awarded to nursing students. Available J.E. Ciembroniewicz, MD Nursing Scholarship Awarded to MaineGeneral Health employees studying toward a degree in health care with priority given to employees pursuing a nursing degree. Available Mary I. Crawford Memorial Scholarship Awarded to nursing students or to nurses employed within MaineGeneral Health who are seeking professional growth. Available in odd-numbered years only. Advanced Nursing Specialty Nursing George Landwehr, MD Scholarship Awarded to MaineGeneral nursing staff who are seeking to further their education. Preference is given to students in obstetrics and gynecology. Other employees or students in health-related fields may be considered. Available Sarah L.
