This Credit Defense Platinum Service Guide ( Service Guide ) will help you understand the features and benefits of the Credit Defense Platinum service. This Service Guide amends your Customer Agreement and Disclosure Statement with Wells Fargo Bank, N.A. What is Debt Cancellation The Credit Defense Platinum service is a debt cancellation service. It provides you with benefits outlined in this document. This service will cancel debt owed on your account if any of the covered events listed in this document happens to you or your joint cardholder (if joint account). Except for the benefit due to loss of life, the benefit is a fixed amount based on the statement immediately following the date of the event, whichever is the larger of: 4% of the New Balance plus the Credit Defense Platinum fee or the Minimum Payment. For involuntary unemployment, disability, and hospitalization, the benefit amount will be credited to your account up to 12 months as long as you remain qualified. In the event of auto or pet expenses of $250 or greater, or should you move to a new address, the benefit amount will be credited for 1 month. Involuntary Unemployment If you ever lose your job involuntarily and are out of work, you may request a benefit under this service. To qualify for this benefit, you must have been employed for a minimum of 3 months as a permanent employee by someone other than yourself or another member of your household at the time of your unemployment. You will not qualify for involuntary unemployment benefits if: you voluntarily quit your job; you were self-employed or employed ed by a family member or household old member; you are retired. Please note: You are eligible for one pet services benefit in a calendar year. Moving If you have moved, you may request a benefit under this service. To qualify for a benefit for moving, you must: provide copies of utility bills showing your name with your previous and new addresses; provide documentation from campus housing verifying your move, if you are a college student living in campus housing; complete and return the forms required by us; meet all requirements, as determined by us, applicable to your circumstances as described in this Service Guide; continue to make at least your Minimum Payment(s) on your account until you are notified by us that your benefit has been approved; and the move must occur after your date of enrollment in the service. Loss of Life If you lose your life, your survivors and/or estate may request a benefit under this service. By calling 1-800-564-8280 your survivors and/or estate will be provided with a benefit request form to be completed by them. To qualify for a benefit due to loss of life: your survivors and/or estate must notify us within 24 months after the date of death by calling the Customer Care Team; we may request a copy of the death certificate; if we do so, your survivors and/or estate must complete and return the forms required by us for the proof of death within 90 days of our request; all requirements must be met as determined by us, applicable to your circumstances as described in this Service Guide; your estate should continue to make at least your Minimum Payment(s) on your account until the estate is notified by us that the benefit request has been approved; and the loss must occur after your date of enrollment in the service.
Credit Defense Platinum Service Agreement Additional Terms and Conditions This agreement contains all of the terms applicable to the benefits. The Credit Defense Platinum service will apply to your account as long as you pay the monthly fee, which will be billed on your monthly statement. Under this agreement, benefits specifically apply to the primary cardholder and/ or the joint cardholder (if joint account) on this account. Authorized Credit Card users are not eligible for benefits. Service Provided We will provide Credit Defense Platinum service benefits for the events outlined in this Service Guide when they occur to the primary and/or joint cardholder as long as the service requirements are met. As long as you maintain this service on your account and pay the applicable fee, you can use the service as many times as needed, subject to conditions outlined in this Service Guide. Receiving Benefits To receive benefits, you will need to take action. You will first need to reach us and request benefits. Then, complete the benefit request form we will send to you and possibly provide some supporting documents to obtain approval from us. If approved, the benefit payments will be shown on your monthly credit card statement. The benefit payment will be reflected as a credit amount. We will also send you a letter describing the approved benefit amount. In some cases, a benefit may be denied. In these cases, you will receive a letter which will explain why you were not eligible to receive benefits at that time. In some cases, we may simply need additional information upon the receipt of which the review process will continue. While awaiting word from us on your benefit request, you will need to continue to make your monthly Minimum Payment on this account. Your satisfaction with the Credit Defense Platinum service is very important to us. When A Benefit Ends For a disability, hospitalization or unemployment, your benefit will end upon the occurrence of any of the following: when your covered event ends; failure to provide your monthly benefit request forms within 180 days; after we have credited your account with 12 benefit payments; if you request to have your benefit stopped; if we discover you intentionally misrepresented to us any information pertaining to your covered event; if we determine, in our sole discretion, that the answers to questions in the initial report were incomplete, false or misleading in any way; if we determine, in our sole discretion, that the qualification icat ation and eligibility requirements described in this Service Guide are not met; or if we determine you violated any of the terms and conditions of this Service Guide or the Customer Agreement and Disclosure Statement. Additional Terms and Conditions (continued) When your benefit ends, regardless rdl of the reason, you will again be responsible for making payments on your account in accordance with your Customer Agreement and Disclosure Statement. For life, moving, pet and auto services, the benefit is a single payment per calendar year. Ending The Service You can end the Credit Defense Platinum service at any time by calling us at 1-800-564-8280 to cancel the service. You will be able to reach us Monday through Friday from 7:00 a.m. to 7:00 p.m. Pacific Time. If you prefer, you can also end your enrollment by writing us at: Credit Defense Customer Care Team, PO Box 740237, Atlanta, GA 30374-0237. We may end this service with no advance notice to you if: your account is closed for any reason; your account becomes 90 days (3 billing cycles) past due; you violate any of the terms and conditions of your Customer Agreement and Disclosure Statement or this Service Guide. We may end this service with 30 days advance notice to you if: we decide to discontinue offering this program to all program participants for any reason. Additional Service Details If we ever change or add benefits to the Credit Defense Platinum service, we will notify you in writing or as required by law. Any changes to this Service Guide that occur during your benefit period will not take effect until your benefit period ends, including a cancellation of this service. If you receive a notice from us of a change in the Service Guide and/or service and you do not wish to be bound by those changes, you must notify us within 30 days of receipt of that notice and we will cancel your service. If you do not notify us within the 30 day period, you will become bound to all changes for which you received notice. If you meet the benefit requirements on the date of event, you are eligible to receive benefits regardless of whether you have active protection at the time the event is reported. We reserve the right to waive any of the requirements listed in this document at our sole discretion. However, if we do this, we will not be obligated to waive the same requirement or another requirement in any other situation. Because this Service Guide is made part of your Customer Agreement and Disclosure Statement with Wells Fargo Bank, N.A. any arbitration provisions listed will apply to this service. All other provisions of your Customer Agreement and Disclosure Statement remain in full force and effect. In the event of any conflict between the terms of this Service Guide and the terms contained in the Customer Agreement and Disclosure Statement, the terms of this Service Guide shall govern. We will not file a Form 1099 with the Internal Revenue Service if your debt is cancelled in any covered event. Any and all taxes related to benefits received by you are your responsibility.
