As an accumulation member, you can apply to increase your cover by one level when a specific life event has occurred by completing this form. You can only apply to increase your cover up to four times under this option (only one application will considered in any calendar year). If you are already on Level 9 you will be ineligible. Step 3 Criteria If you answer Yes to the question below, any additional cover you receive as a result of this request will be provided as New Events Cover only until such time as you satisfy the criteria in the question for at least 30 consecutive days. Full time basis is considered to be at least 35 hours per week. We collect, use and disclose personal information about you in order to manage your superannuation benefits and give you information about your super. We may also use it to supply you with information about the other products and services offered by us and our related companies. If you do not provide the personal information requested, we may not be able to manage your superannuation. We may sometimes collect information about you from third parties such as your employer, a previous super fund, your financial adviser, our related entities and publicly available sources. We may disclose your information to various organisations in order to manage your super, including your employer, our professional advisors, insurers, our related companies which provide services or products relevant to the provision of your super, any relevant government authority that requires your personal information to be disclosed, and our other service providers used to assist with managing your super. In managing your super your personal information will be disclosed to service providers in another country, most likely to Mercer s processing centre in India. Duty of Disclosure: Before you enter into a contract of life insurance with an insurer, you have a duty, under the Insurance Contracts Act 1984,to disclose to the insurer every matter that you know, or could reasonably be expected to know, is relevant to the insurer s decision whether to accept the risk of the insurance and, if so, on what terms. You have the same duty to disclose those matters to the insurer before you vary or reinstate a contract of life insurance. Your duty however does not require disclosure of a matter: that diminishes the risk undertaken by the insurer; that is of common knowledge; that your insurer knows or, in the ordinary course of business, ought to know; as to which compliance with your duty is waived by the insurer. If you fail to comply with your duty of disclosure and the insurer would not have entered into the contract on any terms if the failure had not occurred, the insurer may avoid the contract within three years of entering into it. If your non-disclosure is fraudulent, the insurer may avoid the contract at any time. Alternatively, the insurer may vary the premium rates quoted. The insurer may, subject to the provision of written notice within three years of entering into the contract, elect not to cancel but to reduce the existing insured cover for members in accordance with a formula that takes into account the premium that would have been payable if all relevant matters had been disclosed.
