Dallas County R-1 School District is seeking bids to upgrade their network infrastructure. Specifications for this are contained in this document. The information in this Request for Proposals (RFP) is provided in conjunction with the Universal Service Administrative Company (USAC) Schools and Libraries Division (SLD) Forms 470 and 471, in partial fulfillment of the requirements for the Federal Communications Commission (FCC) Universal Service Fund discounts. Vendors must be an E-Rate eligible service provider and must supply a valid Service Provider Identification Number (SPIN). Vendors must also provide contact information for the individual who is responsible for the filing of vendor E- Rate forms. Bids Should be Labeled: Network Infrastructure Upgrade All bids shall be delivered in duplicate in a sealed envelope clearly marked. Bidding Period: The bidding contract period is February 5, 2015 through March 6th, 2015. Dallas County R-1 School District reserves the right to extend the contract term to coincide with an E-Rate program year or an extended service end date for an E-Rate program year pursuant to a service delivery deadline extension, as defined by the FCC and/or the USAC. In submitting a bid, the bidder agrees to the following terms: 1) Any and all bids must be valid thru completion of work. 2) The Bidder has examined the specification document and has inspected in detail the site of the proposed work, and has familiarized himself/herself with all of the detailed requirements of installation, and understands that in making this proposal he/she waives all right to plead any misunderstanding regarding same. 3) The Bidder understands that the district reserves the right to reject any or all bids, to accept or reject portions of the bid, and to waive any informality in the bidding. 4) The Bidder further agrees that not less than the prevailing hourly rate of wages as determined by the State of Missouri Division of Labor or determined by the Court of Appeals shall be paid to all workers performing work under this Proposal. 5) The Bidder agrees that this bid shall be good and may not be withdrawn for a period of thirty (30) calendar days after the scheduled closing time for receiving bids. 6) Mandatory vendor meeting and walk-through February 19 th beginning at 9:00 A.M. at 500 W. Main, Buffalo, MO 65622.
E-Rate Contingency: All awards are contingent upon receiving E-Rate funding to supplement local funding for support of the project. Evaluation of Proposals: An evaluation committee will review submitted, complete proposals and make a recommendation for contract awards. School District will evaluate proposals from qualified vendors using the following criteria: Price for Eligible Equipment and Services (one-time as well as recurring): 30% Compatibility with Existing Network and Infrastructure: 25% Technical Capabilities and Background/Personnel Qualifications: 10% Prior Experience and Vendor Track Record and References: 20% Cost of Ineligible Equipment and Services (one-time as well as recurring): 15%
Bid Scope Network Infrastructure Upgrade 2015 1) Include the quote for labor and installation charges that may or may not be accepted for the engineering and design, installation, start-up, operational performance testing, and operation requested in the RFP. 2) The cost of labor should be line itemed separately from the cost of equipment and materials. 3) In addition, the vendor shall provide materials and a schedule for all training for the tech staff needed for the optimal utilization of these technologies across schools. 4) A final walk thru will be required to finalize installation performed. 5) Pricing should be clearly delineated as recurring or non-recurring, and inclusive of any and all fees necessary to turn-up service, including but not limited to sub-contractor s fees, installation and special construction fees. 6) Bids should be itemized with E-Rate eligible portion ($150 per student per school) identified and everything over $150 per student should be identified as additional bids for each school. 7) Bids must be submitted in a form compatible with the new USAC Form 471. 8) All products mentioned below are products compatible with those currently used at this district. Other products or models can be used but need to be equivalent and compatible with the mentioned items.
