APPLICATION GUIDELINES The Graduate School of Social Work This packet includes a number of sections which must be completed, and it is the applicant s responsibility to compile and submit a complete packet. All applicants who wish to pursue a master of social work at Dominican University must have completed a baccalaureate degree from an institution that is accredited by a nationally recognized regional accrediting association. Students who wish to pursue the Type 73 School Social Work Certification or Gerontology Certificate program must be currently enrolled in Dominican s MSW program or have a completed MSW degree from a CSWE accredited program. Please type or print clearly. Complete all applicable sections of the application form for the MSW, Type 73 Certification or Gerontology Certificate program. Personal Statement A personal statement is required. Please respond to all five questions listed on page 4. Resumé Please submit a detailed current resumé, including both work and volunteer experience. The resumé should also include descriptions of leadership activities, other relevant life experiences and all languages read and/or spoken in addition to English. Recommendations Three recommendation forms must be completed. One recommendation form should be completed by an academic reference. Two recommendations forms should be completed by work or volunteer-related references. Please request that the recommendation forms be returned to you in sealed envelopes, signed across the seal. No personal references nor professional references from those with whom you have had a client or patient relationship will be accepted. Advanced Standing Applicants Please Note: The academic reference must be different from the individual who completes your advanced standing application form. Transcripts Official copies of all transcripts from all universities or colleges attended are required. Please request that the registrar(s) return transcripts to you in a sealed envelope, signed across the seal. GPA (grade point average) The Graduate School of Social Work expects students to show prior academic performance through GPA. A minimum GPA of 2.75 on a 4.0 scale is required for general admittance. A minimum GPA of 3.0 on a 4.0 scale is required for advanced standing applicants. Advanced Standing Application Complete the enclosed advanced standing application form. The form must be completed by your Bachelor of Social Work (BSW) program director or field director. You must have graduated from a Council on Social Work Education (CSWE)- accredited BSW school within the last six years. Application Fee (non-refundable) A non-refundable check or money order for $25, payable to Dominican University, is required as part of the application process. Graduate Assistantships, Scholarships and Financial Aid Graduate Assistantships and Scholarships are available to students based on student need, GPA and fund availability from the Graduate School of Social Work. MSW Personal Statement APPLICATION FOR ADMISSION The Graduate School of Social Work This is an integral part of your application. The admissions committee will evaluate the quality of writing and the content. The essay should address all of the questions in a minimum of five double spaced pages. Please place your name in the upper right hand corner of each page of your essay. If you are applying for the MSW program and Type 73 School Social Work Certification or MSW program and Gerontology Certificate program please disregard the following MSW personal statement questions and answer the Type 73 or Gerontology personal statement questions. 1. Why do you want to earn a graduate degree in social work from Dominican University? Why have you chosen social work as your profession? Have you considered other professions? Discuss your personal values, philosophy, knowledge of social issues and the professional and community experiences that have influenced your decision to enter the profession. Briefly describe your career plans. 2. What are your personal strengths that will be assets to a career in social work? What attributes do you possess that contribute to your ability to help others? What do you consider your limitations that may impede you in your practice as a social worker? And, what have you done in the past to address these limitations? Tell us about a specific example or a critical situation in which you have played a helping role. 3. What experiences, positive and negative, have you had in working or volunteering in a social work agency or organization? 4. What life experiences personal, family, educational have contributed toward your decision to pursue social work? 5. Discuss a contemporary social problem. Include the following: your reasons for identifying it as a social problem; what elements comprise the problem; what would be done to address this problem and by whom; and what your role might be as a MSW in addressing this social problem. This form may not be completed by any individual who also has completed a letter of recommendation on the applicant s behalf. Information on this form assists us in evaluating the applicant s ability to function at a second-year graduate performance level. This report should include both semesters of the senior year but should be submitted by mid-second semester. Should the information not be available for a complete assessment, an interim report is requested.
