The issuer of the Creditshield Insurance is MetLife Insurance Limited (MetLife). Cover is provided by Citibank, pursuant to a Policy provided by MetLife to Citibank. "We", "Our" and "Us" refers to Citibank Card Services. "Insurer" refers to MetLife Insurance Limited. About MetLife With over 140 years of experience, the MetLife group of companies is one of the world s largest and most respected financial institutions, serving 60 million customers in the America, Asia, Australia and Europe. MetLife serves both individual and institutional customers with a broad array of financial service capabilities, including investments and risk insurances, with one goal in mind to build financial freedom for everyone. In Australia, MetLife offers group and individual life insurance through financial planners, brokers and/or direct marketing channels, including Term Life Insurance, Trauma Insurance, Salary Continuance Insurance, Loan Protection and Mortgage Repayment Insurance Products. MetLife also offers investment products through financial planners, including the Guaranteed Income Plan, Allocated Pension, Term Allocated Pension and Personal Superannuation. Each of these investments is a fixed term/fixed rate investment that provides clients with a capital guarantee. The invitation to apply for this policy is only made to people receiving this PDS in Australia. It is not made directly or indirectly to people in any other country.
CREDITSHIELD Insurance makes it possible for you to borrow with peace of mind, knowing that your credit repayments will be met in the event of death, disablement or unemployment. This optional form of credit protection can be taken to cover a new or existing Credit Facility. What are the significant benefits? Disablement Benefit The Disablement Benefit Option acts as a safety net as accidents and illnesses can occur without warning leaving you unable to work and suffering a loss of income. With CREDITSHIELD we will make your repayments for you if you are unable to work due to an accident or illness. If we believe that you will be unable to ever work again, we will pay an amount equal to your authorised Credit Facility balance. Death Benefit The Death Benefit is designed to ensure your loved ones are not burdened by additional expenses should the unfortunate occur. Unemployment Benefit This option is designed to provide temporary assistance with Credit Repayments while you seek new employment. Optional Cover You are under no obligation to effect this optional insurance. Your decision to include CREDITSHIELD Insurance as part of your Credit Facility may simply be to provide you and your family with peace of mind. Some important benefit limits on your CREDITSHIELD Insurance policy. Death & Total Disablement The maximum amount payable is $30,000. The cover will terminate after the payment of a Death or Total Disablement Benefit. Pays an amount equal to all authorised transactions made on your insured Credit Facility which remain unpaid prior to the injury or illness giving rise to your death or total disablement. 1
Temporary Disablement While your cover continues, there is no limit to the number of claims you can make but you may be entitled to a Total Disablement Benefit after 6 consecutive months of disablement. No benefit is payable for the first 30 days of temporary disablement or after age 65. Involuntary Unemployment While your cover continues, there is no maximum number of claims you can make. However, no benefit is payable while you are aged 21 years or younger. No benefit is payable for the first 30 days of unemployment or after age 65. There is a maximum of 6 monthly payments in any 12 month period. Fees and Costs All Benefits Benefit Limit: The cumulative amount of benefit(s) paid under this policy or any other CREDITSHIELD policy shall not exceed $30,000, irrespective of the number of policies held by you in force and whether they relate to different Credit Facilities. No benefit is payable where the illness or unemployment occurs on or before the commencement date of this policy or within 28 days thereafter, or payable due to a pre-existing condition. What is the cost of your CREDITSHIELD Insurance? You pay just 49 cents per $100 of your outstanding statement balance. This charge is automatically billed to your account. In any month that you have no statement balance on your Credit Facility, you pay nothing. Fees and Charges We do not charge any other policy fees and there are no costs to pay in addition to the premiums. Important information about your insurance What are the significant risks? The significant risks associated with holding CREDITSHIELD Insurance policy which you should consider include: Your policy is not a savings plan, you will not get anything back if it is cancelled. Whether your policy will provide the cover you require. Cover may not be adequate because: The amount of cover you have is more or less than you need; The type of cover you require does not match the cover provided by your policy (for example, because you do not satisfy terms and conditions of cover or an exclusion applies); If you do not comply with your duty of disclosure under the Insurance Contracts Act 1984, and we will not pay your claim, pay only part of your claim, or cancel your policy; and If you do not comply with the policy terms and conditions (for example, premiums are not paid when due, or you have delayed in notifying us of a claim and our interests have been prejudiced by the delay) and we can refuse to pay part or all of your claim; No claim is payable unless you can prove the extent of damage. You should consult your adviser to assist you in determining which of the above risks are significant in your particular circumstances. Policy Exclusions and Limitations CREDITSHIELD policy provides worldwide accidental protection, 24 hours a day. There are some circumstances in which benefits will not be paid. For CREDITSHIELD policy, benefits will not be paid where the event giving rise to the benefit occurs as a result of: a. death by suicide within 13 months of the commencement date of this policy; b. illness or unemployment occurring within 28 days of the commencement date of this policy; c. any deliberate self-inflicted injury; d. the effects of or complications arising from pregnancy; 2 3
e. any accident relating to any aircraft except where you are a fare paying passenger on a licensed commercial or chartered airline; f. riot, civil commotion, strikes and war; g. the influence of alcohol or drugs other than proper use of drugs prescribed by a legally qualified medical practitioner; h. death or any disability attributable to a specific condition or illness of which you had been notified at or prior to the commencement date as being excluded from cover; i. acquired AIDS or the presence of HIV; j. in respect of unemployment, such circumstances as any injury or illness, a strike involving you and your employer or seasonable nature of employment. Documentation You Will Receive Once your application for CREDITSHIELD has been accepted, we will send you a Welcome Letter and Policy Schedule confirming insurance cover. You should read the letter in conjunction with this Product Disclosure Statement including the Policy Terms and Conditions to ensure you fully understand the extent of the cover and its limitations. How You Can Obtain Up-to-Date Information The information contained in this document is up-to-date at the time of its preparation. However, some of the information can change from time to time. We will issue a supplementary or replacement Product Disclosure Statement if there is a materially adverse change to information in this document, or a materially adverse omission from this document. Cooling-off period If you feel your CREDITSHIELD policy does not meet your needs, you may cancel it by written request or by phoning 1300 135 538 within 30 days of the date of our confirmation letter confirming your insurance and if any premium has been deducted, it will be refunded in full. Claims and Claims Procedures Written notice of any intended claim must be forwarded to us as soon as possible after the happening of the event giving rise to the claim. A claim form will be sent to you and no claim shall be payable unless the form has been properly completed. If you make a claim under this policy the Insurer may conduct investigations to assess the value and validity of this claim. This may involve the use of investigation agents, legal advisers and personal data that MetLife may collect. You must cooperate in allowing the Insurer to obtain any certificates or evidence reasonably required by them as to any claim made under this policy and the Insurer will be entitled at its own expense to require the insured to undergo any such medical examinations conducted by a medical practitioner appointed by the Insurer as deemed necessary or to have a post mortem examination carried out. CREDITSHIELD At any time after a claim has been submitted or a potential claim advised to us, Citibank may in its absolute discretion indefinitely suspend your and any account holder s right to use your Credit Facility. You should not attempt to use your Credit Facility during any period where its use has been suspended. Interest will still be charged while the claim is being assessed. Complaints Resolution We have established a complaints resolution process. If you have any complaints concerning your policy, please call 1300 135 538. We will work with you to resolve your concerns. Taxation Under most circumstances, lump sum payments under life or accidental death insurance policies to individuals are not assessable for income tax purposes, and premiums are not tax deductible. This information is based on our interpretation of the present laws. These laws may change and individual circumstances may differ. You should seek advice from a suitably qualified professional in relation to your particular circumstances. GST does not apply to CREDITSHIELD premiums. If Parliament alters the tax law, we reserve the right to increase premiums or charges to reflect any new or increased tax. Register of Alternative Forms of Remuneration MetLife maintains a register of alternative forms of remuneration paid to Australian Finanical Services Licensee groups, advisers, platform providers, etc as required by the Investment & Financial Services Association Ltd (IFSA) of which it is a member. The purpose of this register is to provide an outline of the alternative types of remuneration paid and received and is maintained by Fund Managers, IDPS (platform) providers, Representatives and Licencees. These registers are publicly available and you can obtain a copy by calling MetLife on 1300 555 625. Eligibility and Joint Cover CREDITSHIELD Eligibility You are eligible to apply for these policies if you are between 18 and 60 years of age. Privacy Card Services will provide your personal information to MetLife for the purpose of claims assessment. For more information refer to your Citigroup Privacy Policies. You may obtain a copy by calling 1300 135 538. MetLife Privacy Policy Keeping customer information secure is a top priority for all of us at MetLife in Australia. This notice is intended to inform you about the way in which we handle the personal information about you that we collect and how you can control our disclosure of your personal information. MetLife in Australia is subject to the National Privacy Principles under the Privacy Act 1988, and this document outlines how we intend to deliver all the rights and protections customers are entitled to. Our Policies and Practices to Protect Your Personal Information We protect the personal information we collect about you by maintaining physical, electronic, and procedural safeguards that meet or exceed applicable law. We will only permit authorised employees to have access to your personal information. We train people who work for us how to properly handle personal information and we restrict access to what is necessary for specific job functions. We require third parties that process personal information on our behalf to follow stringent standards of security and confidentiality. We provide you with an opportunity to opt out of receiving information about unrelated products and will not disclose your information for marketing purposes to unrelated companies unless you agree. Purposes of Collecting Your Information We may collect your personal information for a number of purposes, which may include: providing you with a particular product or policy; processing receipts and payments; administering your product or policy; assessing, processing and investigating insurance risks or claims; statement production and other mail related services; meeting legal and regulatory requirements; providing you with information about other products and services, with your consent. Disclosures of your Information We may share your personal information with selected third parties for the purpose of administering your product or policy (some of whom may be situated outside Australia), and your information may be provided to them on a confidential basis for this purpose. We will not disclose your sensitive information (if applicable) for any purpose other than to underwrite your insurance cover or assess a claim. We will investigate all complaints and respond to you within 14 days. Access You are entitled under the Privacy Act 1988 to access most of the information subject to the restrictions identified by the Privacy Act 1988, an organisation holds about you. We want to make this as simple as possible for you. You can obtain a form from the MetLife Privacy Officer, whose contact details appear on page 10. To help us locate and provide the information you request, we would ask that you be reasonably specific about the information you require, and take a few minutes to complete the form. We will only provide your information to you or someone that you specifically authorise. Where a customer establishes that the personal information we hold on them is not accurate, complete or up-to-date we will correct our records if appropriate. We may charge you an administration fee for providing access in accordance with your request. Your request will usually receive a response within 30 days. 8 9

Policy Terms and Conditions 30 Day Cooling Off Period The Insurer provides a 30 day Cooling Off period, which allows you to check that the insurance meets with your requirements. If you are not completely satisfied, you can cancel the insurance from its inception by returning it to us within 30 days of receiving this Certificate. We are confident however that you will be completely satisfied with your CREDITSHIELD Insurance cover. Definitions For the purpose of this Policy the following Definitions apply unless the context otherwise dictates: You or Your means the Credit Facility holder who is named in the schedule to this Certificate, but does not extend to any additional account holders. We, Our or Us means Card Services Pty. Limited ABN 88 004 325 080 that provides Credit to you under the Credit Facility. Accident means where the bodily injury is caused solely and directly by external, violent and accidental means, is independent of any other cause and is not caused by the Insured's own hand. Benefit means a Death, Temporary or Total Disablement or Unemployment Benefit. Commencement Date means the date of the acceptance of your application for insurance. Card Facility means your Card Services Credit Facility which has been nominated as the facility to which this insurance is to apply. Credit means the credit or other form of financial accommodation provided to you under the Credit Account. Death means death by Injury or Illness. Event means Death, Temporary or Total Disablement or Unemployment. Illness means a disease or sickness first occurring after the Commencement Date or occurring prior to that date, provided that advice or treatment for that disease or sickness was not sought or obtained from a medical practitioner, chiropractor, naturopath, or any other practitioner of a similar kind within twelve months immediately prior to the Commencement Date. Indebtedness means, where at the time of Death or Total Disablement: (a) a Temporary Disablement Benefit or Unemployment Benefit was not being paid, an amount equal to the "Closing Balance" shown on the last statement issued on your Credit Facility prior to the Illness or Injury giving rise to your Death or Total Disablement; plus, an amount equal to any authorised transactions made on the Credit Facility prior to that Injury or Illness which were not included on that statement, to a maximum of your Credit Limit; or 10 11
(b) a Temporary Disablement Benefit or Unemployment Benefit was being paid, an amount equal to the "Closing Balance" shown on the last statement issued on your Credit Facility prior to the Unemployment or the Injury or Illness giving rise to the payment of the Temporary Disablement Benefit; plus, an amount equal to any authorised transactions made on the Credit Facility prior to that Unemployment, Injury or Illness which were not included on that statement, to a maximum of your Credit Limit, less an amount equal to the reduction in that outstanding amount as a result of the Unemployment or Temporary Disablement Benefits which have subsequently been paid. Injury means bodily injury resulting from an Accident occurring after the Commencement Date. Insurer means MetLife Insurance Limited ABN 75 004 274 882. AFSL 238096 Monthly Instalment means the total minimum amount due payable to us as set out in the last Credit Facility statement issued prior to the occurrence of the Event giving rise to a claim. Temporary Disablement means disablement caused by Injury or Illness and as a result of which you are prevented from attending to your own occupation and provided you are not otherwise gainfully employed or in receipt of any payments from your employer (including sick pay) or workers compensation insurance. Total Disablement means having been Temporarily Disabled for six consecutive months and as a result of the Injury or Illness, you are rendered unable to earn income in any occupation, trade or profession for which you could reasonably be expected to be suited through education, training or experience and provided that the Insurer is satisfied that you will be so rendered indefinitely. The six month qualifying period may be waived at the Insurer's absolute discretion. Unemployment means that you are in receipt of, or are entitled to be in receipt of, unemployment benefits under the social security law (as defined in the Social Security (Administration) Act) as issued and amended by the Commonwealth Government, or would have been entitled to receive such unemployment benefits if your spouse was not in receipt of an income. Benefits Subject to the terms and conditions of this Certificate the following benefits are available: Death Benefit Upon your Death the Insurer will pay to us the amount of your Indebtedness plus interest charges on the Indebtedness for a maximum of two months after the date of Death. Temporary Disablement Benefit The Insurer will pay to us your Monthly Instalment for each month of your Temporary Disablement. Total Disablement Benefit Upon your Total Disablement the Insurer will pay to us the amount of your Indebtedness plus interest charges on the Indebtedness for a maximum of two months after the date of your Total Disablement. Unemployment Benefit the Insurer will pay to us your Monthly Instalment for each month (or part there of) of your period of Unemployment to a maximum of six (6) Monthly Instalments in any twelve (12) month period. If you have previously received an Unemployment Benefit, a subsequent Unemployment Benefit will only be available after you have been incontinuous full-time employment for a minimum period of fourteen consecutive working days since the previous period of Unemployment. Where you are receiving an Unemployment Benefit you must immediately inform the Insurer should you become re-employed or no longer entitled to the Unemployment Benefit. General Conditions Age Limitation No Death Benefit will be paid where you have attained the age of 70 (at which time your insurance shall cease). No Temporary Disablement Benefit, Total Disablement Benefit or Unemployment Benefit will be paid where you have attained the age of 65. No insurance will be effected where you have attained the age of 60. No Unemployment Benefit will be payable whilst you are aged 21 years or under at the time of the Unemployment. Payment of Benefits All Benefits will be payable to us and applied to the Credit Facility. Receipt of such Benefits by us will be a discharge to the Insurer in respect of all claims under this Certificate. Commencement of Cover Insurance will be provided under this Certificate from the Commencement Date. 12 13
Limitation on Payment of Benefits (a) No Temporary Disablement Benefits shall be payable for the first thirty (30) days of any period of Temporary Unemployment. (b) No Unemployment Benefit shall be paid for the first thirty (30) days of any period of Unemployment. (c) Where you are unemployed as a result of the termination of a contract before the agreed time for termination, no Unemployment Benefits shall be payable after the date the contract would have terminated but for the early termination. (d) The maximum cumulative amount of Benefits payable under CREDITSHIELD Insurances for any one Event shall not exceed $20,000, irrespective of the number of CREDITSHIELD Insurances in force or the number of Credit Facilities in respect of which you have obtained CREDITSHIELD Insurance. (e) If an event enables you to qualify for more than one Benefit, only one Benefit will be paid, being the higher Benefit applicable. Premiums No insurance will be provided by this Certificate and no Benefits payable unless the premium has been paid by us to the Insurer on its due date. All Premiums payable under this Policy shall be paid by us to the Insurer monthly in arrears with the amount of the premium being charged to your Credit Facility. We shall discontinue payment of premiums to the Insurer where your previous statement's "Total Minimum Payment Due" and any other amounts due and payable there under have not been paid to us on the due date. Premiums are payable at the rate of $0.45 for every $100 of the Closing Balance shown on your Credit Facility Statement each month. The Insurer may at any time vary this rate upon giving 30 days written notice. Such a variation will only be made by the Insurer if the variation applies to all insurances provided under the Master Policy. Termination of Cover The insurance cover provided under this Certificate shall terminate on the happening of the first of any of the following events: (a) Your Credit Facility having been cancelled; (b) You having attained the age of 70; (c) Your Death or Total Disablement; (d) Non-payment of premiums by us to the Insurer for a period of thirty days after they have become due, this being caused by non payment of amounts on your Credit Facility (see "Premiums"clause); or (e) Written notice from you directing that the insurance be cancelled. Claims Procedures No Benefits shall be payable under this Certificate unless: (a) Written notice is given to us within 30 days after any of the Events giving rise to a Benefit; (b) You or your legal personal representative completes the standard claims form issued by the Insurer and produces, at no cost to us or the Insurer, such evidence to substantiate the claim to the Insurer's satisfaction as the Insurer may reasonably require; (c) You undergo medical examinations at any time by a legally qualified medical practitioner appointed by the Insurer, the cost of which is borne by the Insurer; d) You undergo any medical treatment recommended by a legally qualified medical practitioner and which it is reasonable for you to undergo; (e) You or your legal personal representative provides proof of your age where required by the Insurer; (f) a post mortem examination is carried out if required by the Insurer at the Insurer's expense. At any time after a claim has been submitted under this Certificate, we may in our absolute discretion indefinitely suspend your and any additional account holder s right to use your Credit Facility. You should not attempt to use your Credit Facility during any period where its use has been suspended. Proof of Indebtedness In determining the amount of any Benefit the Insurer is entitled to rely on any statement in writing as to the amount of your Indebtedness or Monthly Instalment which has been signed on our behalf by a responsible officer. Exclusions No Benefits will be payable under this Certificate where the Event giving rise to the Benefit occurs as a result of: (a) Death by suicide within thirteen (13) months of the Commencement Date; 14 15
(b) Illness or Unemployment occurring within twenty-eight (28) days of the Commencement Date; (c) any deliberate self-inflicted injury; (d) the effects of or complications arising from pregnancy; (e) any Accident occurring on or in or about any aircraft other than an aircraft in which you were travelling as a bonafide paying passenger and which is operated by a licensed commercial or chartered airline; (f) Riot, civil commotion, strikes and war (whether war be declared or not); (g) the influence of alcohol or drugs other than proper use of drugs prescribed by a legally qualified medical practitioner; (h) Death or any disability attributable to a specific condition or illness (or aggravated by a specific condition or illness) of which you had been notified at or prior to the Commencement Date as being excluded from cover; (i) (i) Acquired Immune Deficiency Syndrome (AIDS), AIDS RelatedComplex (ARC) as defined by the World Health Organisation from time to time; or (ii) the presence of the Human Immunodeficiency Virus (HIV) as revealed by the positive HIV anti-body or HIV test; (j) in respect of Unemployment: (i) any Injury or Illness; or (ii) a strike or labour disturbance in which you or your employer is involved; (iii) the seasonal nature of your employment, including the fact that you are employed for a specific period and that period has come to an end; (iv) the cessation of a contract of employment on the termination date provided for in that contract where the termination date is determined by a date specified or upon completion of a task, project or service provided under the contract; (v) your voluntary resignation or retirement; (vi) Unemployment existing at the Commencement Date; (vii) Unemployment occurring outside Australia; (viii) your misconduct, dishonesty or criminal activities. No Total Disablement Benefits will be payable under this Certificate where you have previously received Total Disablement benefits under any CREDITSHIELD Insurance Policy. Cancellation You may cancel your insurance at any time giving us notice in writing. 16
