ONE SIZE DOES NOT FIT ALL You step into the unknown every day. Do you have a safety net? Life Insurance Even with the best plans in place, it s impossible to see what s coming tomorrow. If something were to happen to you, how would your mortgage, car payment, credit card bills and other expenses get paid? Your MGM MIRAGE benefits package includes life insurance equal to one times your annual base income, at no cost to you. If that s not enough to give you and your family peace of mind, you can purchase additional term life insurance through Sun Life. Term Life and AD&D Insurance provided by Sun Life Select the coverage amount, up to $1,000,000 or five times your annual base income (whichever is less), and the benefit is payable to the beneficiaries you elect. In addition, you can purchase voluntary Accidental Death & Dismemberment (AD&D) insurance to complement your term life policy. You may also purchase term life insurance for your spouse and dependent children. 3
ADDITIONAL VOLUNTARY BENEFITS Disability Insurance Picture yourself with nine of your coworkers. Chances are, three of you will become disabled at some point before retiring. How would you pay for everyday expenses like your home, car and groceries if you were unable to work for awhile? Disability insurance is income replacement. You can have peace of mind knowing your income is protected when you cannot work due to an accident, serious illness or pregnancy. Short-Term Disability provided by Sun Life Short-term disability insurance can replace your income for up to 26 weeks. You can choose the elimination period that best meets your needs: Elimination Period: The waiting period before you receive benefits. Choose 14 days or 30 days. Benefit Amount: Coverage replaces 60% of your income, up to a maximum benefit of $1,000 per week. Long-Term Disability provided by Sun Life Long-term disability insurance can replace your income when you are unable to work for an extended period of time. Choose your level of benefit and the benefit duration that best meets your needs: Benefit Amount: Choose to replace 50% or 60% of your income, up to a maximum benefit of $10,000 per month. Benefit Duration: Choose the length of time the benefit is payable to you: five (5) years or up to age 65. Long-term disability benefits begin after the 180-day elimination period. Disability benefits keep Tony in the game Work hard, play hard that s my motto! My indoor soccer league keeps me busy. But last February, I hurt my knee during a game. A surgery sidelined me for the rest of the season. Worse, I couldn t work for nearly six weeks. My short-term disability insurance was a lifesaver. This policy pays a first occurrence benefit as well as hospital confinement and continuing care for heart attacks, strokes and much more. Personal Accident Indemnity Plan provided by Afl ac Accidents are the leading cause of injury-related emergency room visits. The Personal Accident Indemnity Plan pays for initial treatment, hospital confinement and much more, whether an accident occurs on or off the job. Supplemental benefits protect Jen and her son My son Michael is my pride and joy. Just ask my family and friends they ll tell you about all the photos of him that I carry. Making sure he gets to enjoy his childhood no matter what is a big priority for me. I enrolled in Aflac s Personal Accident Indemnity Plan, just in case. If I end up in the emergency room thanks to a clumsy fall or other accident, the plan will help make sure Michael and I still have cash for groceries, school field trips and his Little League dues. It feels good knowing MGM MIRAGE offers benefits that help me take great care of my son. Personal Cancer Indemnity Plan provided by Afl ac According to the American Cancer Society, men in the United States have slightly less than a 1-in-2 risk of developing cancer during their lifetime; for women, the risk is a little greater than 1-in-3. The Personal Cancer Indemnity Plan includes coverage for just about every part of the treatment regimen, from hospital confinement to radiation and chemotherapy. The policy also pays a first-occurrence benefit when a covered individual is first diagnosed as having internal cancer. 5
