CONSENT FORM With my signature I confirm the following: I have received the information about the Public Cord Blood donation in Switzerland and have had all my questions answered to my complete satisfaction. I confirm by signing the consent form that I have read and understood all information contained in the information sheet and that I was given all necessary explanations. I am aware that until the birth of my baby I have the right to withdraw my consent to donate my baby s cord blood. I confirm that my personal data are correct and that I have answered the questionnaire to the best of my knowledge. I know that my blood and my baby s cord blood will be tested with molecularbiological tests to avoid any potential transmission of a disease to the recipient. I know that a sample of my blood and my baby s cord blood will be stored for possible controls. I agree to be informed about the results. I accept that by voluntarily donating my baby s cord blood I am transferring the ownership of the donated cord blood unit to the Directors of the Cord Blood Bank. This cord blood can be used for treatment of diseases in unrelated patients requiring stem cell transplantation. I agree to provide the maternity hospital information about any pertinent changes to my health that can impact me as a qualified donor s parent. I will allow the maternity hospital and my baby s doctor to provide relevant health information now and in the future regarding myself or my baby that might impact the quality and safety of the stored cord blood in order to avoid giving it to a potential recipient. I hereby give my informed consent to the maternity hospital for the collection of umbilical cord blood at the delivery of my baby.
