Yes No c) Did it extend over the road and/or on to your lot? Yes No d) Did you see the runoff occurring or evidence of it afterwards? Yes. I saw the surface runoff water around my lot. No. I saw evidence afterwards of runoff waters on my lot. e) Describe the extent of flooding on your lot. 4. a) Did your house experience flooding: Yes No If yes please answer next questions. Do you have a pump in your basement for sanitary sewage? 9. a) Do you have a sanitary check valve preventing backflow on your sanitary sewer connection? Don t Know If yes answer the remainder of Question 9, otherwise proceed to Question 10. b) Is the sanitary check valve easily accessible or is it beneath a sub-floor, carpet, etc., easily accessible, beneath a sub-floor or carpeting c) Do you monitor the check valve to ensure it is operating satisfactory? d) Are you aware that the sanitary check valve should be regularly inspected to ensure they are operating as intended?

4 e) Was the sanitary check valve installed since the 2002 flooding event? 10. Do you have a check valve on your storm drainage system? Did you see any evidence that the water in your basement may have entered through a crack in the basement wall? If yes, what specifically leads you to believe this?
