Health Care Enhancements Incorporating National WEDI Industry Card Standards and the UnitedHealthcare Branding Transition UnitedHealthcare is the first health company to use the national Workgroup for Electronic Data Interchange (WEDI) industry card standards to provide uniformity of information, appearance, and technology for Commercial, Medicare and Medicaid health care ID cards for all business. New data standards mean physicians and care providers can read any carrier s ID card using a single card reader. We believe this is an important step in simplifying the administration of health care. We conducted research with physician offices/practice managers, pharmacists, hospitals and consumers; and we responded to comments and improvements suggested by employers to ensure that we provide the optimal health care ID card. Why Are We Becoming One, Unified Master Brand? We are transitioning to a single UnitedHealthcare brand for our benefits businesses. Our goal is to build a brand that consumers and businesses can turn to for health care benefits no matter where they live, no matter what their age, and no matter what type of product they need. We feel a single UnitedHealthcare brand will make it easier to do business with our company. Key Features of the Health Care Some of the important WEDI health care ID card standards include: Consistency layout based on WEDI standards for data fields, placement and size; personalization and variable information with labels printed on front of card and instructions on back of card Readability limited information on the card, machine readable formatting, and use of large font size Copying/Scanning ability to clearly copy or scan for patient health records with white background Inclusive meets national health care ID card guidelines set by the National Council for Prescription Drug Programs (NCPDP) What does Community Plan Mean? As part of our rebranding, AmeriChoice and Unison will no longer be used and will now be referred to as UnitedHealthcare Community Plan. The term Community Plan is representative of UnitedHealthcare s effort to provide dedicated service to America s most vulnerable and under-served populations one community at a time, one member at a time. What does Medicare Solutions Mean? Medicare Solutions is the family of Medicare plans that include Part D Prescription Drug Plans, Medicare Supplement Insurance Plans and Medicare Advantage Plans featuring the UnitedHealthcare, AARP, SecureHorizons or Evercare brand names. As part of our rebranding for 2012, all of the Medicare Solutions products will either be UnitedHealthcare branded, or in the case of the AARP Medicare Plans, insured through UnitedHealthcare.

What s Changing on the Health Care s? Our health care ID cards will display the new UnitedHealthcare logo in the top left corner of the ID card and there will be an additional name modification to the right of the logo for our Medicare and Medicaid plans: AmeriChoice Medicaid plan is now: UnitedHealthcare Community Plan SecureHorizons Medicare plan in 2012 will be: UnitedHealthcare Medicare Solutions Cobranded AARP Medicare plans will continue to have the AARP logo but the sub-brand will change from SecureHorizons to UnitedHealthcare Additionally, the cards include an industry-standard bar code for machine readability of the information printed on the card. What s NOT Changing on the Health Care s? Our claims process, claim addresses and customer service numbers are not changing. What Do the New Cards Look Like Compared To the Old Cards? See below to see how the cards will look for each line of business. Highlights are included for changes to the cards. Q. How do I identify a member s product coverage (commercial vs Medicaid vs Medicare)? A. You will be able to identify the correct product coverage by looking at the logo on the top of your patient s health care ID card where it references UnitedHealthcare, UnitedHealthcare Community Plan or UnitedHealthcare Medicare Solutions. If a member calls in to set an appointment, you can verify the product by asking the member to tell you the name on the front of the card as well as the address for claims on the back of the ID card. This will allow you to identify the product line. * Please note, that the branding transition will take place over more than a year. Therefore, depending upon that timing, it is likely that you will encounter members with a non-branded ID card. Q. Now that everything is referred to as UnitedHealthcare can I find information on UnitedHealthcareOnline.com for all my patients? A. Although most of our member information can be accessed on UnitedHealthcareOnline.com, you may need to utilize other URL s due to the fact that UnitedHealthcare supports multiple claim platforms for our various lines of business. Please reference the URL on the back of your patient s health care ID to select the correct web portal. Q. How does this rebranding affect my contracts with UnitedHealthcare? A. The branding initiative does NOT affect your contractual agreement with UnitedHealthcare and its lines of business. Q. Where should I call if I have questions? A. You should call the phone number as noted on the back of the health care ID card. Q. Which website URL should I use in order to check eligibility or claim status? A. You should use the website URL noted on the back of the health care ID card. For the majority of our lines of business use the following: Commercial Continue to use UnitedHealthcareOnline.com Medicare Continue to use UnitedHealthcareOnline.com Medicaid Use UHCCommunityplan.com Key Tips u When scheduling a patient, reference the P.O. Box on the back of the health care ID card to confirm the member s product u When calling the customer service number enter the member s current ID# into the Interactive Voice Response system to ensure proper routing u When submitting claims enter the Payer ID which is noted on the cards u Always use the member s ID card as the source of truth. Check it each time that you see or talk to the member. M47500C&S 5/11 2011 United HealthCare Services, Inc.
