To the extent not covered by Medicare or under any other provision of this policy, Combined Insurance will pay: 1. Additional Skilled Nursing Benefit: For 30 days of medically necessary Skilled Nursing Facility Care. The daily rate payable under this section shall be the maximum daily rate established for licensed nursing care facilities by the Wisconsin Department of Health and Social Services. 2. Preventative Care Benefit: Reimbursement for the actual charges up to 100% of the Medicare approved amount for each service for preventive health care services determined to be medically appropriate by an attending physician, to a maximum of $120 annually. 3. Kidney Treatment Benefit: Expenses incurred up to $30,000 per calendar year for hospital inpatient or outpatient kidney disease treatment for dialysis, transportation or donor related services.

7 Basic Policy Additional Benefits... In addition, to the extent not covered by Medicare or this policy, we will pay the usual and customary unpaid expenses for: Chiropractic Care Benefit Medically necessary chiropractic charges which you incur whether Medicare recognizes the service or not. TMJ Coverage Benefit Diagnostic procedures and medically necessary surgical or non-surgical treatment for the correction of temporomandibular (TMJ) disorders. Equipment/Supplies for Diabetes Treatment Benefit An insulin infusion pump or other non-prescription equipment and supplies, including non-prescription insulin, but not including any other outpatient prescription medications, used in the treatment of diabetes or self-management education for the treatment of diabetes, with the exception that Combined Insurance will pay for only one insulin infusion pump in a Calendar Year. Breast Reconstruction Benefit Breast reconstruction of the affected tissue as the result of a mastectomy. Dental Care Benefit Hospital or ambulatory surgery center charges and anesthetics provided in connection with dental care if the Insured is: a child under the age of 5; or an individual with a chronic disability; or an individual with a medical condition that requires Hospitalization or general anesthesia for dental care.

Exclusions Combined Insurance s Medicare Supplement policies do not cover or pay for: a) Nursing care costs not covered by Medicare and not otherwise covered under the Wisconsin 30 day skilled nursing mandate; g) The expenses Medicare Part B would have covered if the Insured were enrolled in Medicare Part B, but was not so enrolled; and h) Expenses covered and payable under Medicare. b) Home health care above the number of visits covered by Medicare and the 40 visits mandated by S.632.895 (2), Stats, or home health care visits in excess of those covered under the optional Additional Home Health Care Rider, if purchased; c) Physician charges above Medicare s approved charge; d) Care received outside of the U.S.A., when the expenses are not covered by Medicare; e) Dental care (except as provided in this policy) or dentures, checkups, routine immunizations, cosmetic surgery, routine foot care, examinations for or costs of eyeglasses or hearing aids; f) Services rendered by or covered by any agency of a State government (except Medicaid) when you have no obligation to pay for such services;

Medicare Deductible and Copayment Changes If Medicare changes its deductible and copayment amounts, the policy benefits will also change. When this happens, your renewal premium may increase. Premiums are based on attained age rating which means your renewal premiums will increase as your age increases. Renewability Combined Insurance guarantees it will renew this policy for your lifetime (so long as the required premium is paid). Combined Insurance reserves the right to change the premium. Any change in premium will be made on all policies of the same class. Definitions Hospital means an institution which is defined as such by Medicare. It does not include any facility not covered by Medicare. Usual and customary means the most common charge for services or supplies in the area where they are received. Excess Charges (ONLY available when Rider 14917 is purchased) is the difference between the Medicare Part B approved expense and the amount charged by the attending physician, which can be no more than the limiting charge allowed by Medicare. 30 Day Trial If you are not satisfied with the policy for any reason, you may cancel your policy within 30 days of the effective date and still receive a full refund.
