This document sets out what is and is not covered, together with the sums insured and any special terms that may apply. Please check that it meets your needs and you understand it. The Contract of Insurance This document and schedule form a legally binding contract of insurance between you and us. The contract does not give, or intend to give, rights to anyone else. No one else has the right to enforce any part of this contract. The insurance provided by this document covers loss, damage, death or disability that happens during the period of insurance for which you have paid, or agreed to pay, the premium. This insurance is provided under the terms and conditions contained in this document. Language - This Insurance is written in English and all communications about it will be English. Governing Law - Unless we have agreed otherwise with you, this contract is governed by English Law. Underwritten by Acromas Insurance Company Limited. Financial Services Compensation Scheme (FSCS) As Acromas Insurance Company Limited are members of the Financial Services Compensation Scheme (FSCS), you may be entitled to compensation under the scheme if we cannot pay out all valid claims under this insurance. This depends on the type of policy you have and the circumstances of the claim. The scheme will cover 90% of the cost of the claim with no upper limit. For types of insurance you must have by law (such as third party motor insurance for motor claims), the scheme will cover the whole claim. You can get more information about the scheme from the FSCS or you can visit their website at www.fscs.org.uk

Evidence of Cover under master Certificate No. : PAG 1700/12/ SCHEDULE Reference number: 3245917-007/MC Policyholder: Mr A W Test Period of insurance: 12 months from 27th August 2014 or until termination of the attaching Motorcycle Insurance Policy if earlier Premium: 24.06 Insurance premium tax: 1.44 Total premium: 25.50 Definitions The following words or phrases are defined and have the meaning shown whenever they appear in this document in bold print. Accident Bodily injury A sudden, unexpected, violent and external specific event which happens during the time of cover, at an identifiable time and place which causes bodily injury. An identifiable physical injury to you caused directly and solely by an accident and independently of illness or disease or any other cause except illness directly resulting from that physical injury which results in your death or disability within 12 months of the date of the accident. Emergency dental treatment Emergency treatment to natural teeth (including fillings and crowns sitting in or on a natural tooth but excluding dentures and implants) within seven days of the road rage assault. Hospital Insured motorcycle Loss of limb(s) Loss of sight Loss of use Period of insurance A lawful establishment (other than a convalescent nursing or rest home or convalescent nursing self-care or rest sections unit of a hospital) which has accommodation for resident patients with organised facilities for diagnosis and major surgery and which provides a 24-hours a day nursing service by registered nurses. The motorcycle defined in your Motorcycle Insurance Policy. The loss of a hand or foot by physical severance or total loss of use of an entire hand or foot. The permanent and total loss of sight which we consider as having happened: in both eyes, if your name is added to the Register of Blind Persons on the authority of a fully qualified ophthalmic specialist; or in one eye if, after correction, the degree of sight you have left in that eye is 3/60 or less on the Snellen Scale (meaning you can see at three feet what you should be able to see at 60 feet). The total and irrecoverable loss of use where the loss is continuous for 12 months and such loss is deemed permanent and beyond the possibility of improvement. The period of time covered by this insurance as shown on your schedule. This insurance runs concurrently with your Motorcycle Insurance Policy. In the event of cancellation or non-renewal of your Motorcycle Insurance Policy all cover under this policy shall cease. Permanent total disablement Disablement which entirely prevents you from working in any business or occupation of any and every kind and which after a period of 12 months from the date of the disablement, in the opinion of a medical referee, shows no sign of ever improving. Personal effects Articles worn, used or carried by you, excluding motorcycles and their accessories, money, stamps, tickets, documents or securities, goods or samples.

Road rage assault United Kingdom We, us, our You, your A sudden, unexpected, unusual and specific event caused by an unknown third party with deliberate intent to cause bodily injury at an identifiable time and place following a road accident within the United Kingdom. Great Britain (England, Scotland, Wales), Northern Ireland, the Isle of Man and the Channel Islands. Acromas Insurance Company Limited. The policyholder only who is named on the Motorcycle Insurance Policy schedule or in the event of the policyholders death, the policyholders legal personal representatives.

The Cover We will pay up to the sums insured shown below if you suffer loss, damage, death or bodily injury whilst riding on, mounting onto or dismounting from the insured motorcycle, which is being ridden by you and is being used for social, domestic and pleasure purposes (including daily travel between your normal residence and your place of work) within the United Kingdom during the period of insurance. 1. Accidental death 25,000 2. Total and irrecoverable loss of sight in one eye 25,000 3. Total and irrecoverable loss of sight in both eyes 25,000 4. Loss of use of or loss of one limb 25,000 5. Loss of use of or loss of two or more limbs 25,000 6. Permanent total disablement 25,000 You will also be entitled to the following additional benefits in the event of a road rage assault. 7. Hospital daily cash benefit in the event of a road rage assault of 100 per day of confinement, but not beyond 30 nights and not for the first night, unless two or more nights are spent in hospital. 8. Emergency dental treatment in the event of a road rage assault up to 250 excluding the first 25 of each and every loss. 9. Loss or damage to clothing and personal effects in the event of a road rage assault up to 150 excluding the first 25 of each and every loss. Where an article is under two years old and proof of purchase can be provided, we will pay the full replacement value subject to the policy limit. For items over two years old, we will pay the value of the article as at the date of the loss or damage after allowing for wear and tear. We will not pay claims under this section for loss or damage to property covered under any other insurance. 10. Five sessions of stress counselling following a claim under items 2, 3, 4, 5, 6, 7 and 8. SUBJECT TO THE FOLLOWING EXCLUSIONS AND CONDITIONS General Exclusions We will not pay claims under items 1 to 10 of this insurance for loss, damage, death or disablement directly or indirectly arising out of or consequent upon or contributed to by: 1. Use of a motorcycle for: a) Private or public hire. b) Racing competition, rallies, trials, speed testing or for any purpose in connection with the motor trade. c) Your business or in connection with your employment, other than daily travel between your normal residence and your place of work. d) Courier and/or messenger services. 2. All claims arising from physical or mental conditions or disabilities of a recurring or chronic nature from which you suffered and were known to suffer, prior to the inception of this insurance. 3. You committing or attempting to commit suicide or intentionally inflicting self-injury, while sane or insane. 4. Deliberate exposure to exceptional danger (except in an attempt to save human life), your own criminal act or you being under the influence of alcohol or drugs. 5. Provoked assault or fighting (except in bona fide self-defence). 6. Any matrimonial or family dispute. 7. Any surgery or treatment that is not medically necessary, cosmetic surgery, reversing cosmetic surgery, or any corrective treatment needed as a result of cosmetic surgery. 8. War, riot, act of foreign enemy (whether war is declared or not), civil war, revolution, power being seized unlawfully, terrorism, nuclear, chemical or biological materials being released or escaping, or any other similar event. For the purposes of this exclusion, terrorism means any act, or acts, of any person or group committed for political, religious or similar purposes, with the aim of influencing any government or putting the public, or any section of the public, in fear. Terrorism can include, but is not limited to, using or threatening to use force or violence. The people who carry out acts of terrorism can either be acting alone, or can be acting on behalf of or in connection with any organisation or government. If any part of this exclusion is not valid, or we cannot enforce any part of it, the rest will still apply. 9. We will not pay the following: a) The sum insured for item 1 if the bodily injury does not lead to death within 52 weeks of an accident. b) The sum insured for items 2 to 5 if the loss results in death within 52 weeks of an accident. c) The sum insured for item 6 if the disability results in death within 52 weeks of an accident. d) More than 25,000 plus any of the additional benefits for road rage assault. General Conditions 1. Observance by you of the following requirements is a prior condition of your cover in respect of any claim made under your policy. If you do not observe them we have the right to refuse or withdraw cover for your claim. 2. That at the start of this period of insurance you have not attained 90 years of age. 3. That you are resident within the United Kingdom. 4. Claims Procedure If you need to make a claim, please contact the Saga Select claims line on 0800 096 7597. When a claim or possible claim occurs, you must tell our claims handlers as quickly as possible. You must also report the claim to your motorcycle insurers. You must get and act on advice from a registered medical practitioner, and have any medical examination that we ask and pay for. You must give us any documents, information and evidence that we need and continue to do so until the claim is concluded. If you die, we will be entitled to ask for, at our expense, a post-mortem examination.

5. Aggravated Physical Disability If the consequence of an injury is aggravated by a physical disability or condition which existed before the accident occurred, the amount payable under this insurance in respect of the consequences of the accident shall be the amount which is considered would have been payable if such consequences had not been so aggravated. 6. Your Right to Change Your Mind You may cancel the insurance, without giving reason, by sending us written notice and returning the insurance documents within 14 days of it starting or (if later) within 14 days of you receiving the insurance documents. We will refund any premium you have paid in full so long as a claim has not been paid and a claim is not likely to be made against us, otherwise the full premium is payable. 7. Cancellation You may cancel the insurance at any time by sending us written notice and returning the insurance documents. This insurance runs concurrently with your Motorcycle Insurance Policy. In the event of cancellation or non-renewal of your Motorcycle Insurance Policy all cover under this insurance shall cease. However, unless it is cancelled within the 14 day cooling off period, there will be no refund in premium. We may cancel this insurance by sending you 14 days' notice in writing to your last known address. We will refund the part of the premium which applies to the remaining period of insurance. The policy may be cancelled if: you do not pay any policy premium requested or, if applicable, stop paying instalment premiums; your insurance details change such that they no longer meet our insurance criteria; you fail to provide further information or documentation we have requested in writing: - in support of your application, renewal or adjustment you wish to make to your policy; - to assist us to handle a claim that is being made; or - we act under General Condition 10 Fraudulent Claims. 8. Notification The police must be notified as quickly as possible following any event likely to give rise to a claim under this insurance. 9. Your Duty The insurance cover this policy provides depends on the terms and conditions in this document, which must be read as one document with your schedule. You must comply with these terms and conditions to have the full protection of your policy. If you do not pay a premium we will not provide cover from the date the premium was due. It is your duty to provide us with correct information and to answer our questions with reasonable care and honestly, truthfully and completely. If you, or anyone acting for you, has deliberately misrepresented the answer to our questions, or misrepresented any answers provided to online questions by manipulating an answer and these answers would have: influenced our decision to provide you with insurance we would otherwise not have agreed to provide; or influenced the terms and conditions on which we have provided cover; or prevented us from charging the correct premium; We will treat your policy as if it never existed and return any premium you have paid for the policy unless we have identified a false or fraudulent claim when General Condition 10 will also apply. If the misrepresentation is not deliberate but is careless we will either: treat your policy as if it never existed and return any premium you have paid for the policy if we would not have accepted the risk had you given the correct information; or amend your policy details to record the correct information, apply any terms that would have been applied had you given the correct information, and charge and collect any additional premium due. 10. Fraudulent Claims If we discover that you, anybody insured by this policy or anyone acting for you has knowingly: made a fraudulent or false claim in full or in part or exaggerated the amount of the claim; misrepresented any answers to our questions or withheld any relevant information in order to influence us to accept a claim; provided false or invalid documents in support of a claim; or following an allegation or suggestion of fraud by us or another insurer, withdrawn a claim, had a claim refused or declined or had a policy cancelled or made void we will investigate the claim and this could result in legal action by us.

We may: or treat your policy as if it never existed from the date of the fraud or misrepresentation and retain any premium you have paid for these policies; serve you with a 7-day notice of cancellation on all other policies which you hold with us; and pass details to the police and fraud prevention agencies. refuse to pay the whole of your claim if any part is in any way fraudulent, false or exaggerated and recover from you any costs that we have incurred. 11. Keeping to Legal Responsibilities Managing claims If you make a claim, we may need to release information to another person or organisation involved in that claim. This includes, but is not restricted to, others involved in the incident, their insurer, their solicitor or representative and medical teams, the police or other investigators. We may also have to investigate your claim and conviction history. Under the conditions of your policy, you must tell us about any incident (such as an accident) which may or may not result in a claim. When you tell us about an incident, we will pass information relating to it to Insurance Database Services Limited (IDSL). Use of Your Information The information that you have given to Saga Select and/or the insurer will be held and used to manage your insurance policy, which will include both underwriting and claim handling. For this purpose Saga Select and/or the insurer may disclose it to other interested parties, for example to other insurers, regulatory authorities and to agents who provide services on their behalf. Your data may be disclosed to regulatory bodies for the purpose of monitoring and/or enforcing compliance with any regulatory rules/codes. It may also be used for market research, offering renewals, research and for statistical purposes and crime prevention. Saga Select and/or the insurer will try to ensure that any personal data you provide will be held securely and in accordance with The Data Protection Act 1998. From time to time Saga Select and/or the insurer may use service providers and organisations outside of the European Economic Area (EEA) for the purpose of processing services, system testing and maintenance. It is worth noting however that some non-eea countries do not afford the same level of data security as the UK. By submitting your details, you consent to this transfer. The insurer and/or, if applicable Saga Select will always use every reasonable effort to ensure sufficient protections are in place to safeguard your personal information. Your calls may be monitored and recorded in order that the service may be improved and to help prevent and detect fraud. Saga Select and/or the insurer may check information provided or received and may also undertake additional fraud searches. Credit reference agencies: To assess your insurance application and the terms on which cover may be offered (including the quote and payment methods Saga Select and/or the insurer is able to offer you), Saga Select and/or the insurer may obtain information about you from a number of sources, including credit reference agencies, to check your credit status and identity. The credit reference agencies will keep a record of the search; this may be reflected in your credit score. By applying for this policy you consent to Saga Select and/or the insurer processing personal data, including sensitive personal data, about you and any other persons who are the subject of the application and/or who may be insured under the policy. You understand that all personal data you provide must be accurate and that if you supply another persons personal data you are, by doing so, confirming that you have the specific consent of that other person to disclose that data. If you contact us electronically we may store your Internet Protocol (IP) address or your telephone number supplied by your Service Provider. This may be used to identify repeat website visits, fraudulent behaviour or mystery shoppers using Acromas Group websites. By providing Saga Select with your personal information, including your medical data, you consent to the Saga and Acromas groups of companies (the Group) using it to contact you by post, telephone, email, SMS or other electronic means about offers, products and services which may be of interest to you, and using data we obtain about you for other products in respect of both marketing and preparing quotations. The Group will do this unless you contact it or you make use of the regular opportunities that it provides you with, to confirm which channels and products you do and do not wish to hear about, or unless you tell the Group that you prefer not to receive direct marketing. In the case of personal data, with limited exceptions, you have the right to access and, if necessary rectify information held about you by formal written application to Saga's Group Data Protection Officer at Enbrook Park, Sandgate, Folkestone, Kent CT20 3SE. Fraud Prevention and Financial Crime To help Saga Select prevent fraud, money laundering and other financial crime, the information you provide may be submitted to fraud prevention agencies and other organisations whose records may be searched.

As part of the insurers fraud prevention process it will complete a number of enquiries to check the details you provide. If false or inaccurate information is provided and fraud is identified, details will be passed to fraud prevention agencies. Law enforcement agencies may access and use this information. The insurer and other organisations may also access and use this information to prevent fraud and financial crime, for example, when: Checking details on applications for credit and other credit related facilities. Managing credit and credit related accounts or facilities and recovering debt. Checking details on proposals and claims for all types of insurance. Checking details of job applicants and employees. The insurer and other organisations may access and use, from other countries, the information recorded by fraud prevention agencies in the United Kingdom. Cheat line - protecting policyholders against fraud To protect our policyholders we are members of the Insurance Fraud Bureau (IFB). If you suspect insurance fraud is being committed, you can call them on their confidential cheat line 0800 422 0421. Customer satisfaction - our promise to you We aim to provide a first-class service. It is always our intention to provide the best possible standard of service, however if something does go wrong we would like to know about it, and we will do our best to resolve it for you as quickly as possible. We will contact you within five working days of receiving your complaint to inform you of what action we are taking. We will try to resolve the problem and give you an answer within four weeks. If it will take us longer than four weeks we will tell you when you can expect an answer. If you are not satisfied with our final response, you can ask the Financial Ombudsman Service to review your case. Financial Ombudsman Service The Financial Ombudsman Service resolves disputes in an independent and fair way. Email: complaint.info@financial-ombudsman.org.uk Important note The Financial Ombudsman Service will only consider your complaint if you have already given us the opportunity to resolve the matter. However, if we have not provided a final response within eight weeks you can refer your complaint straight to the Financial Ombudsman Service. (These procedures do not affect your rights to take legal action if necessary.) Future underwriter changes This policy is currently provided and underwritten by your insurer as part of an agreement between Saga Select and them. At some time in the future Saga Select may enter into an agreement with a new provider. You hereby authorise Saga Select to transfer any personal data to a new provider, including data defined as 'sensitive personal data' under the Data Protection Act, and consent to the new provider being able to offer cover to you.
