We offer a comprehensive, competitive benefits package. We encourage you to take advantage of it to get and stay healthy, protect your income, and prepare for retirement. Before you enroll, make sure you learn about your choices by using the enrollment tools on EnableMidstreamBenefits.com. When you do, you can make an informed choice for your 2015 coverage. You have two types of decisions to make your Health & Income Protection Benefits and your Retirement Benefits. In the Health & Income Protection Benefits category, you ll elect Medical, Dental, Vision, Life Insurance and more. In the Retirement Benefits category, you ll set up your new Enable Midstream 401(k) Retirement Savings Plan account. 2
Don t Miss Your Chance You will receive an email telling you it is time to enroll, as soon as the online enrollment system receives your data (within a few weeks after your start date). Your benefits will be effective on the first of the month following your hire date. If you don t enroll, you won t have Enable Midstream medical, dental or other voluntary coverage in 2015 unless you experience a qualified life event. Even if you have benefits outside of Enable Midstream, you should still enroll. Why? Enrollment is the time to designate your beneficiaries for Enable Midstream s fully-paid benefits like Basic Life Insurance and Accidental Death and Dismemberment (AD&D). Medical When it comes to your medical coverage, Enable Midstream has something to fit everyone. We offer two medical plan options: a Health Savings Account (or HSA) paired with a High Deductible Health Plan (HDHP) or a PPO. The same medical services are covered by each plan the differences are in how and when you pay for care. You can choose from four coverage levels Employee Only, Employee + Spouse, Employee + Child(ren), or Employee + Family. Blue Cross Blue Shield (BCBS) and Express Scripts administer our medical and prescription drug plans. Self-funded Medical Plans Enable Midstream s medical plans are self-funded. That means claims are administered by BCBS and Express Scripts. Enable Midstream and you pay for coverage and care through premiums, copays, coinsurance and HSA contributions. Medical Plan Option 1: HSA + HDHP The Health Savings Account (HSA) paired with a High Deductible Health Plan (HDHP) option provides medical coverage for you today and lets you save tax-free dollars for future medical expenses even into retirement. Helping You Save Enable Midstream jump-starts your savings by contributing money to your HSA. In 2015, Enable Midstream will contribute the first $500 if you select Employee-Only coverage or $1,000 if you cover other family members. These contributions will be prorated based on your hire date. 4

Fidelity handles the administration of your HSA. Here are a few highlights of the plan. Make an Informed Choice Portability: If you should leave Enable Midstream, you may take your HSA with you. Lower taxes: You can save for your future medical expenses as well as cutting your tax bill. Your contributions are tax-free. Build your savings: Any money left in your HSA account at the end of the year rolls over to the next year. How the Plan Works When you need medical care, you can use your HSA dollars or pay out-of-pocket so your HSA savings can continue to grow. Once you ve met your deductible, your BCBS HDHP benefits begin. You ll pay your share of coinsurance for your in-network covered services and copays for prescription drugs. If you reach your out-of-pocket maximum, you won t pay anything for covered services for the rest of the year. Medical Plan Option 2: PPO With the PPO Plan, you pay copays for prescription drugs and visits to in-network doctor s offices and urgent care clinics. For other care, you meet a deductible and then pay coinsurance until you meet your out-of-pocket maximum. * If you have contributed to an HSA previously in 2015, it is your responsibility to make sure total contributions (employer and employee) for the 2015 plan year do not exceed the IRS limits. 5

Dental Enable Midstream will offer a Dental PPO through Delta Dental. You can use the PPO and Premier dental networks. Here are some highlights. Flexible Spending Accounts Enable Midstream will offer two types of flexible spending accounts (FSAs): a Health Care FSA and a Dependent Care FSA. You can contribute tax-free dollars to your FSA (up to $2,500 for the Health Care FSA and up to $5,000 for the Dependent Care FSA). Discovery Benefits is our FSA administrator. They offer a mobile app to help you identify eligible expenses and make it easy to manage other aspects of your FSAs. Use your Health Care FSA to pay for medical, dental and vision care for yourself, spouse and dependent children. Use the Dependent Care FSA to pay for day care for your dependents. Plan Carefully Whenever you participate in an FSA, plan carefully. If you don t use all the money in your FSA each year, you lose it. If you are concerned about losing funds you don t use, consider the HSA + HDHP Plan. The HSA balance rolls forward every year and can be used to pay for more than medical expenses. You can use your HSA to pay for dental and vision expenses as well. You can enroll in either the Health Care FSA or the HSA + HDHP, but not both. Benefits Tip Anything you can do to reduce your taxable income could help lower the taxes you pay at the end of the year. Income Protection (Life and Disability) Enable Midstream offers you a variety of benefits that protect your income in the event of your death or disability. The Company pays the full cost of Basic Life Insurance and Basic Accidental Death & Dismemberment (AD&D). During your new hire enrollment period, Enable Midstream s Voluntary Life Insurance Plan has no evidence of insurability (EOI) requirement for employee coverage. If you elect coverage for your spouse over $50,000, EOI is required. BENEFITS BASIC LIFE INSURANCE Two times your salary up to $400,000 VOLUNTARY LIFE INSURANCE BASIC ACCIDENTAL DEATH & DISMEMBERMENT (AD&D) VOLUNTARY AD&D For yourself: Up to five times your annual earnings (or $450,000 whichever is less) For your spouse: The lesser of 50% of your voluntary coverage or $250,000 For your child(ren): The lesser of 100% of your voluntary coverage or $10,000 Two times your salary up to $400,000 For yourself: Up to five times your annual earnings (or $450,000 whichever is less) For your spouse: The lesser of 50% of your voluntary coverage or $250,000 For your child(ren): The lesser of 100% of your voluntary coverage or $10,000 Automatically Yours At Enable Midstream, full-time employees automatically get these company-paid benefits: Basic Life and Basic AD&D Insurance Short-term and Long-term Disability that provide 60% of base pay (up to plan maximums) Employee Assistance Program No election is required for these benefits, but remember to designate your beneficiaries for Basic Life and AD&D using the enrollment system you can access via www.enablemidstreambenefits.com. 9
Preparing for Retirement Enable Midstream helps you prepare for retirement through our 401(k) Plan, which features a dollar-for-dollar Matching Contribution of up to 6%, an annual Retirement Contribution of 5% of eligible compensation, and a variety of investment options. You are automatically enrolled at a pretax contribution rate of 6% of your eligible earnings after 30 days of employment, unless you elect not to participate, or you elect to contribute at a different rate. The default investment is the Vanguard Target Retirement Fund that aligns with your age and estimated retirement date. By logging on to www.netbenefits.com, you can do any of the following: Increase or decrease your savings rate at any time, or elect not to participate Begin saving and receiving Matching Contributions before 30 days of employment Learn about the investment funds available through the 401(k) Plan and choose your investments Designate your beneficiaries Learn how to roll over IRAs or eligible 401(k) accounts For more details about the 401(k) Plan, visit EnableMidstreamBenefits.com. Enrolling is Easy You will receive an email telling you it is time to enroll, as soon as the enrollment system receives your data (within a few weeks after your start date). Be sure to enroll by the deadline listed in your email. Just follow these three simple steps: 1. Go to EnableMidstreamBenefits.com. 2. To enroll in your Health and Income Protection Benefits: Click the Enroll for Health and Income Protection Benefits button. Click Register to set up your user name, password and security questions. Our Company Key is ENBL. Once you have access to the site, click Start Here and follow the instructions to enroll or waive coverage. 3. To enroll in your Retirement Benefits: Click the Enroll for Retirement Benefits button. (If you already have an account with Fidelity, you will use your existing user name and password to log on to their site.) Enrollment Tip: If you want to cover any eligible dependents, have their date of birth and Social Security number handy before you begin the enrollment process. Click the Register Now link.
