Understanding Medicare: Your Options When choosing a Medicare plan, it pays to know your options. The Basics The Original Medicare Plan includes Part A Hospital Insurance and Part B Medical Insurance. For additional premiums, you can add Part D Prescription Drug Coverage and/or a Medicare Supplement Plan ( Medigap ). Medicare Supplement Plans: Designed to cover some costs that Medicare does not Many plans pay the Medicare deductibles and coinsurance on expenses such as hospital and physician bills Plans may not provide any more benefits than Original Medicare Some plans can cost thousands of dollars each year Seem complex? There s a simpler option A Medicare Advantage Plan. Medicare Advantage Plans ( Medicare Part C ) are health plans approved by Medicare and run by private companies like ConnectiCare. Simplify Your Medicare Coverage with a ConnectiCare VIP Medicare Advantage Plan The chart below displays your Medicare Plan options at work. Column I shows how you can add to your Original Medicare Plan with Part D Prescription Drug Coverage and a Medicare Supplement Plan. Column II shows how a Medicare Advantage Plan, like ConnectiCare, simplifies your Medicare coverage and may save you money and offer more benefits. Original Medicare Plan Part A Hospital Insurance & Part B Medical Insurance Medicare provides this coverage. After you meet your Part A deductible and your Part B deductible, generally Medicare pays 80% of covered costs, and you pay 20%. You must pay your Part B monthly premium. Premium may vary based on your tax filing status and annual income. q For additional premiums, you can add: Part D Prescription Drug Coverage (Optional) If you want this coverage, you must join a Medicare Prescription Drug Plan run by a private company, approved by Medicare. Are You Eligible to Enroll in a ConnectiCare VIP Medicare Plan? You re eligible to enroll in a ConnectiCare VIP Medicare Advantage Plan if: You qualify for Medicare Part A; You are enrolled in and continue to pay for Medicare Part B; and You reside in Connecticut. There are no pre-existing conditions, and physical exams are not required. Except under certain limited circumstances, The Centers for Medicare & Medicaid Services does not allow Medicare Advantage plans to enroll Medicare beneficiaries that have End-Stage Renal Disease. 4

That s why ConnectiCare provides informational mailings, phone calls and postcards to encourage the use of needed preventive health services, screenings and immunizations. A pharmacist or health care provider will review your prescription medicines, over-the-counter medicine and any herbal supplements you may take. The goal is to make sure they are safe and working to improve your health. Diabetic Eye Exam Reminder An annual eye exam is very important for members who have diabetes. That s why we may send a friendly reminder when an eye exam is due or overdue. 7

Medicare Advantage Plan: Added Benefits Health and Wellness Support Services Treatment Decision Support Program If you have health problems that require treatment, you may have questions or need someone to talk to about medical decisions. Our Treatment Decision Support Program will help you make confident decisions about your personal situation. In this program: Members can speak directly with a specially trained Registered Nurse The Nurse will answer your questions and give objective, relevant health care information The Nurse will also help you work more effectively with your doctors Medicare Care Management Program Complex health care needs require special attention. This program focuses on supporting our members with chronic or complex medical needs. We identify members for this program through a Health Assessment, a post-discharge follow-up call, or based on a review of claims information. Medicare Advantage Plan: Added Benefits Peace of Mind, Wherever You Go. Worldwide Emergency and Urgent Care You want to know you re covered if you have a medical emergency or need urgent care, especially when you travel. That s why every Plan we offer includes Worldwide Emergency and Urgent Care. Knowing the difference between Emergency Care and Urgent Care can save you time and money. A visit to a hospital emergency room will cost you more out-of-pocket (higher copayment) than a visit to an urgent care center. Also, for less serious medical conditions, your wait time at an urgent care center will generally be less. What is a Medical Emergency? A medical emergency is when you reasonably believe that your health is in serious danger. What is Urgent Care? Urgent care is when you need medical help for an unforeseen illness, injury, or condition, but your health is not in serious danger. Maximum Out-of-Pocket Limit You get additional financial protection with all of our Medicare Advantage Plans with the annual Maximum Out-of-Pocket Limit, included in each Plan. This limit is the most you would have to pay in copayments/coinsurance out of your own pocket for covered medical expenses within each calendar year. If you were to reach your Plan s Maximum Out-of-Pocket Limit, ConnectiCare would pay 100% of your covered medical expenses for the remainder of the calendar year. Choosing a Plan: Things to Consider Budget How often do you need to use medical services? Determine the better value for you by weighing higher premium costs and lower copayments against lower premium costs and higher copayments. You pay more per month to have lower copayments for most services. All Plans include Prescription Drug Coverage, except the VIP Prime 4 (HMO) Plan. If you have creditable Prescription Drug Coverage from a previous employer, or from the Veterans Administration (VA), our Prime 4 Plan may be a good option for you. You will have the flexibility to purchase your covered prescriptions from our extensive network of pharmacies, using the options listed below. Where you purchase your prescription drugs may affect the amount you pay out of your own pocket. Part D Prescription Drug Coverage Getting Extra Help People with limited incomes may qualify for Extra Help to pay for their prescription drug costs. If you qualify, Medicare could pay up to 100% of your drug costs including monthly prescription drug premiums, copayments and coinsurance. When to Enroll You may only enroll in a ConnectiCare VIP Medicare Plan during specific times of the year. Important Enrollment Dates Initial Coverage Election Period You can enroll when you first become eligible for Medicare (three months before the month you turn age 65 until three months after the month you turn age 65). If you did not elect Medicare Part B when you were first eligible, you can still enroll in a Medicare Advantage Plan. You will have a three-month period to enroll, which begins three months before your Medicare Part B effective date. You will not be able to enroll after your Medicare Part B effective date. If you receive Medicare benefits due to a disability, you can join from three months before to three months after your 25th month of disability. See page 4 for more information on eligibility. Annual Election Period for 2015 Coverage: October 15, 2014 December 7, 2014 If you are eligible for Medicare, you can enroll in or switch plans during the Annual Election Period.
