Specific hours and days are determined by the clinical site. Additional class hours are required in information sessions during previous semesters as communicated in the Information Session schedule. Course Grading Scale: Satisfactory/Unsatisfactory Prerequisites: Good standing in the Physical Therapy Program and successful completion of all Phase I and Phase II academic and clinical course work. A fully completed and up-to-date Physical Therapist Intern Medical Information Form must be on file. Current evidence of all certifications/training (CPR, First Aid, OSHA, HIPAA) and vaccines (MMR) must be presented to the PT Program before beginning PTH 515. Program Goals: The goals of the and its individuals are: 1. To educate students to be clinicians trained for general practice, who provide best care respectful of patient/client values, grounded in evidence-based practice and clinical reasoning, and compliant with federal, state, professional, and institutional regulations and guidelines. 2. To engage in ongoing peer reviewed scholarly activity including the scholarship of teaching, discovery, integration and application using qualitative and quantitative methodologies. 3. To engage in ongoing clinical practice that integrates learning and scholarship into the delivery of physical therapy services. 4. To engage in ongoing service to Carroll University, the physical therapy and related professions, and Waukesha County and surrounding communities Professional Track Mission Statement: The aim of the professional track in the Carroll University Entry-Level Physical Therapy Program is to prepare an entry-level generalist trained clinician to successfully meet all requirements of the professional practice and practice management expectations. Graduates will demonstrate self-directed learning, critical thinking, problem-solving ability and community needs awareness to allow autonomous practice within a changing and diverse health care environment. Course Description: Students participate in a full-time internship for seven weeks at clinical facilities affiliated with the Physical Therapy Program. During the internship, students integrate and apply prior knowledge to actively participate in patient management in a supervised environment. Learning is advanced and student performance is assessed. Course Objectives: During the internship the student will, with additional instruction and supervision as necessary, meet the following objectives within the domains of 1) professional practice expectations, 2) patient/client management expectations, and 3) practice management expectations. I. Physical Therapy Values and Behaviors A) Acts in accordance with Core Values and Core Professional Behaviors 1) Adherence to the Standards of Practice, Code of Ethics, institution regulations, and state and federal laws. 2) Core Values: accountability, altruism, integrity, professional duty, compassion/caring, excellence and social responsibility in a culturally competent manner. 3) Service to the profession and/or community through efforts that support and promote Physical Therapy. 2
4) Core Professional Behaviors: critical thinking, communication, problem solving, interpersonal skills, responsibility, professionalism, use of constructive feedback, effective use of time and resources, stress management, and commitment to learning. II. Clinical Aptitude A) Patient/Client Management 1) Competent physical therapy examination, evaluation, diagnosis, prognosis, intervention and progression. 2) Sound clinical reasoning strategies that exemplify evidence-based practice. 3) Competent communication skills to effectively receive, interpret and express verbal, non-verbal, and written messages in a respectful manner. 4) Integration of health promotion, wellness and prevention into physical therapist practice. B) Practice Management 1) Ensures coordinated care 2) Application of practice management principles III. Clinical Scholar A) Scholarship 1) Use of information technology to access sources of published information related to physical therapy and critically evaluates the literature. 2) Integrate best evidence for practice from sources of published information with clinical judgment and patient/client values to determine and administer best care. Format and Teaching Methods: The student will be assigned by the ACCE to a 7 week rotation. The student will be evaluating and treating patient/clients under the direct supervision of a licensed physical therapist who is the Clinical Instructor (CI). The CI is responsible for directing the student's activities, documenting the student's level of practice, and evaluating the student's performance. Grading Methods: Clinical Internship I is graded on a Satisfactory/Unsatisfactory (S/U) basis. The student is expected to meet course and program objectives and demonstrate appropriate progression on the CPI instrument. Satisfactory performance expectation is Advanced Beginner level. The grade is assigned by the Program's ACCE. The ACCE determines if the student has satisfactorily met the clinical objectives and requirements for the internship through evaluation of the results of the CPI that has been completed by the student's CI. Further pertinent information may be gained through discussion with the CI and/or CCCE. The student will receive a grade of U if the course and program requirements are not met successfully. Unsatisfactory performance in any one of the following criteria: 1 (safety), 2 (professional behavior), 3 (accountability), 4 (communication) or 7 (clinical reasoning) exhibited by the student is grounds for a U grade and possible dismissal from the program. If a grade of U is earned for failure to meet the objectives in the 7 week rotation the student fails the course. The failed rotation may be repeated only once. If the student receives a grade of U a second time, the student will be dismissed from the program. 3
To change a U to an S grade, the student will be required to do remediation as specified by the ACCE. Remediation is dependent upon availability of clinical facilities or faculty. The student will not be allowed to graduate until the remediation has been completed satisfactorily. A student's graduation date can be delayed when internship remediation is required. Course Policies: Statement on Academic Integrity The Carroll University Academic Integrity Policy is located in your student handbook. Students are expected to be familiar with it. If a student violates this policy in any way, the instructor reserves the right to impose a sanction of failure in the course. Accommodation for Disabilities Any requests for accommodation must be made through the Disability Services Coordinator at the Walter Young Center. Appropriate accommodations will be made once notification has been received. Modification of the Syllabus The instructor and the University reserve the right to modify, amend, or change the syllabus (course requirements, grading policy, etc.) as the curriculum and/or program require(s). Course Requirements: 1. Correspondence with the Clinic At least six weeks prior to the beginning of the internship the student is to write to the CCCE /C I to introduce herself/himself and thank them for the opportunity to intern at their facility. The student may request information regarding where, when and to whom the student is to report on the first day. Other questions regarding the facility should be limited and only asked if it is pertinent and if the information is not found in the Clinical Site Information Form (CSIF) or web sites. The letter, once approved by the ACCE, along with a completed Physical Therapy Intern Experience Form and a copy of the course syllabus which informs the CI of the expectations and objectives of the course is then mailed. 2. Documentation of Required Records Students are required to take documentation to verify current immunizations, CPR, First-Aid, and background checks with them to the internship. All records must be current through the last day of the internship. It is the students' responsibility to see that they have the needed copies prior to leaving for the internship. 3. Journal: The student is to write a journal during the internship. This journal must be submitted to the ACCE with the other paperwork within three business days after the end of the internship. 4
Ground Rules and Guidelines for Journal-Writing a. Purpose This exercise is to give the student an opportunity to reflect on the mental and emotional aspects of going into the clinic and on taking the responsibility for patient care. The student is to think about what actions and motivations he / she experiences during the internship and what level of confidence the student has for the activities done. It is important to reflect on your own actions, what additional work you need to do on your skills, how to take advantage of your past experiences in current situations, and how possible outcomes are altered by personal insights. This is also an experience in self evaluation and self discipline. b. Ground rules 1) The student is to write at least 4 journal entries although you may write more if you wish. The four required times to write are: the night before the internship, the night after the first day, the end of the first week, at the end of the last day after the final evaluation. 2) The journal is to be submitted electronically on LMS 3) There is to be a cover page with the following information: Title it Clinical Internship I, followed by the name of the facility and inclusive dates of the rotation. 4) Date each separate entry. c. Guidelines for writing Please comment on your perceptions of yourself as a neophyte physical therapist, as a person learning a new set of skills, and on the facility and staff. During the course of your journaling you may specifically comment on: 1) Your skills as a PT - strength / weakness within the limits of what you learned in school 2) Your interactions with the personnel and patients 3) Your ability to communicate - especially with the CI and the patient 4) How you are working to make yourself better. Where do you need to be better within the limits of what you have learned in school? 5) Comment on your emotions / feelings as appropriate to this experience 6) Curriculum: What are your needs now that you are immersed in the clinical education environment? What are the ways that you could improve yourself? 7) Context: What strikes you about the operations of the department, the staff, the organization, and the health care system? 8) Conflict management: What is the source? What are your strategies to resolve it? 4. Weekly Goal Planning Log Responsibilities of the Student: Complete the initial orientation and planning page with a list of areas of interest or focus at this particular internship prior to the start of the internship. This information should be reviewed and discussed between the student and CI within the first day or two of the internship. 5
Near the end of each week complete a page of the log noting your strengths and areas for growth. This includes addressing the accomplishment of goals established for the past week. Brief bullet points of the information are fine as well as the information is further discussed with the clinical instructor. Share this information with the CI and seek feedback regarding these issues. Collaboratively with the clinical instructor establish goals for the following week. Student will record these on form. After discussion with clinical instructor, sign and date the page of the form used that week. Submit completed goal planning log to ACCE with all their other final materials. Responsibilities of the Clinical Instructor: Review the student s planning log from the past week and acknowledge accomplishments and areas for growth and offer suggestions to these as well. Summarize this information on the weekly planning log form. Brief bullet points of the information are fine as well as the information is further discussed with the student. Discuss the information listed on the goal planning log and provide feedback as appropriate to student and well as seek feedback regarding how the internship is going thus far. Collaboratively with the student establish goals for the following week. Student will record these on form. 5. Midterm Call for Assessment of Performance A component of the midterm assessment is a telephone discussion between the student, the CI and the ACCE (or designated Adjunct faculty) at midterm. The ACCE will send the students a schedule of times the ACCE and Adjunct Faculty are available for the call. This appointment, which is made via email or phone call to the ACCE, needs to be established by the student by Wednesday of the third week of the rotation. It is the student s responsibility to initiate the phone call. During the phone call, both the student and the CI will talk with the ACCE or adjunct faculty, each individually and in private. The student must make sure that he/she has discussed potential times with the CI and has at least three alternative times in mind when scheduling. Both the student and the clinical instructor must be available at the time of the call. In the event that a student and CI have a conflict (staffing, illness, etc.) and are unable to make the call at the established time, the student is expected to contact the ACCE/Adjunct faculty (depending on whom they were scheduled with) to notify and reschedule. 6. Clinical Performance Instrument (CPI) This is the assessment tool used to evaluate the student's clinical performance. It is web based and requires successful completion of the training modules before it can be accessed. The student and CI individually fill it out and then meet to discuss their assessment. If there is more than one CI, they collaborate to fill out one CPI instrument. The midterm CPI is to be completed no later than Friday of the fourth week and the final CPI no later than the final day of the clinical. The student and CI must sign off on both copies. 7. Attendance The student is required to adhere to the Clinical Internship Attendance Policy as outlined in the student manual and below. 6
Clinical Internship Attendance Policy Promptness and attendance are mandatory during clinical internships. Absences for communicable illness, health emergencies, family emergencies and presentations at Program approved professional conferences are allowed during clinical internships. A student will not be excused for more than one professional conference presentation in any given seven week rotation. In the event of any absence the ACCE and the clinical facility must be notified in a timely manner and the absence must be accounted for and made up. Make-up time is dependent upon clinical faculty and facility availability. The student will not be allowed to progress or graduate until the make-up time has been satisfactorily completed. Unexcused absences are not acceptable and subject to disciplinary action as determined by the ACCE. 8. Forms Upon completion of the internship, all required paperwork must be delivered/submitted to the Physical Therapy Department within three business days after the clinical experience unless otherwise stipulated below. The paperwork is to be mailed or placed in the mailbox of the ACCE. Paperwork is not to be e-mailed. The required paperwork includes: a. Electronic submission of the student and CI versions of the CPI. If for some reason a paper version was completed it is mailed/submitted with the other paperwork. The electronic version of the CPI for the midterm assessment is submitted at the time it is completed but no later than the end of the fourth week. The final CPI must be submitted no later than the last day of the internship b. Journal c. Student Goal Planning Log d. Clinical Instructor Survey e. PT Student Evaluation of Clinical Experience and Clinical Instruction. This is completed and submitted electronically by the last day of the internship.. In addition a paper copy will also be turned in. The signature of the CI (s) is required in the designated space.
