HILBERT COLLEGE SPORTS MEDICINE POLICIES AND PROCEDURES This policies and procedure manual pertains to the Athletic Department at Hilbert College. It is intended to give direction and answer questions concerning athletic injuries, prevention of injuries, insurance, emergency procedures and medical coverage of practices and home events. PRE-PARTICIPATION PHYSICALS Each athlete must receive a pre-participation physical upon beginning participation in Hilbert College Athletics. All returning Hilbert College Athletes must receive an update physical exam each year by their primary physician. Individuals who do not receive a physical and do not appear on the team's roster are not covered under the Athletic Department insurance policy. It is the responsibility of the head coach to provide an up to date roster to the Athletic Training Department, and to inform us regarding any new athletes who wish to begin participation before or during the season. All student athlete physical forms are on the Hilbert College Sports Medicine webpage. PRE-EXISTING CONDITIONS The Hilbert Athletic Department will not be financially responsible for any injuries or illnesses sustained prior to a student-athlete s release for and participation on any intercollegiate athletic team. Additionally, the Sports Medicine Department will not be responsible for the treatment and, if necessary, rehabilitation of pre-existing injuries. It is the student-athlete s responsibility to arrange for ongoing treatment and rehabilitation of their injury when they arrive on campus. SICKLE CELL TESTING REQUIREMENTS All Hilbert College student-athletes must sign the Sickle Cell Testing Waiver form included in their preparticipation physical indicating their sickle cell trait status and desire for testing. If a student athlete indicates that they know their sickle cell trait status, proof of a negative test must be included the preparticipation physical. Student-athletes that wish to be tested for their sickle cell trait status should contact Hilbert College Sports Medicine Staff for more information. OUT OF SEASON INJURIES OR ILLNESSES The Hilbert College Athletic Department will not be financially responsible for injuries or illness when the student-athlete is not actively engaged in a formal, official practice or game during the competitive season. Official dates of in-season practice are controlled by the NCAA and are available to the head coach of each sport. Absolutely no charges will be honored for injuries sustained outside the official NCAA designated playing season.

REPORTING OF ATHLETIC INJURIES All injuries are to be reported to the Athletic Training Staff as soon as they occur, or as soon as possible. Before an athlete is seen by an outside physician, they must first be seen by the Hilbert College Sports Medicine Staff. Obvious emergency or life threatening situations and situations that occur on the road and require immediate care, (when a member of the Sports Medicine Staff is not traveling) do not require approval. These are the only two exceptions to this rule for reporting injuries. If your team is on the road and an athlete sustains an injury, it is the Head Coach s responsibility to contact one of the Head Athletic Trainer prior to departing for home, so that we can plan to have the athlete seen when they arrive back on campus, when necessary. ELIGIBILITY OF SERVICES Student-athletes are permitted to utilize all available services provided by the Hilbert College Sports Medicine Department as long as the student-athlete adheres to the Policies and Procedures contained herein, remains a member in good standing of a Hilbert College intercollegiate athletic team, and is injured as a result of participation in a Hilbert College sponsored practice, game or travel scenario. Services provided by the Sports Medicine Department cease upon the completion of intercollegiate eligibility by each student-athlete. TREATMENT OF INJURIES Rehabilitation of injuries that occur through participation in the Hilbert College Intercollegiate Athletic Program will be performed in the Athletic Training Room at Hilbert College. In the event we do not have the equipment necessary to adequately rehabilitate a specific injury, we will refer the individual to the proper facility. Injuries that occur as a result of participation in activities that are not Hilbert College Athletic Department sanctioned practices or games (i.e. intramurals, pick-up games or individual workouts) will not be the responsibility of the Hilbert College Athletic Training Department. We are not allowed to treat these individuals as per the New York State Department of Professions Guidelines for Athletic Training. PHYSICIAN REFERRAL All medical referrals for injuries sustained while participating in a Hilbert College sponsored athletic program, must be made through the Sports Medicine Staff. In most cases, these referrals will be seen through the Team Physician Dr. Jennifer Teeter, Hilbert College Wellness Center, or the student s primary medical physician. Student athletes should not be referred to any outside medical vendors by anyone other than the Hilbert College Sports Medicine Department. Any medical bills generated by student athletes who do not follow the correct referral procedure MAY NOT BE COVERED by the Hilbert College secondary insurance coverage for athletics. In cases of emergency, where an athletic trainer is not available, please refer to the Hilbert College Emergency Action Plan for Athletics. If, in the opinion of the Hilbert College Team Physician or Sports Medicine Staff, an athlete should be referred to a medical specialist for consultation due to an athletic injury, that referral will be made by one of the previously mentioned individuals.

OUTSIDE CONSULTATIONS If the student-athlete and/or parents, guardian, and/or spouse prefer to seek other medical care of their choice, the following guidelines are in effect. The Athletic Department will: Assume no financial responsibility for any expenses incurred. Not render any follow-up medical care. Not allow the student-athlete to participate until the following conditions have been met: Receive a detailed written report and other required medical records from the attending physician. This must include a written release for practice and/or competition within the student-athlete s specific sport. The student-athlete demonstrates full skills and abilities necessary to compete in their selected sport. The student-athlete seeks and passes a pre-participation physical examination by the Hilbert College Team Physician, if deemed necessary by the team physician or any other member of the Sports Medicine Staff. The guidelines above are in effect for any student-athlete who seeks medical care for an off-season, non-athletic injury as well. SECOND OPINIONS The Hilbert College Athletic Department is not responsible for expenses incurred from physician consultations for the purpose of a second opinion unless referred by the Sports Medicine Team Physician or attending physician. The Hilbert College Athletic Department is not responsible for expenses incurred from physician consultations for the purpose of a second opinion when requested by the student-athlete and/or parents, guardian, and/or spouse. SCHEDULING OF APPOINTMENTS When a student-athlete is injured they may be referred to an off campus medical facility for evaluation and treatment. All such appointments will be scheduled as deemed necessary by Hilbert College Sports Medicine Staff and team physician Dr. Jennifer Teeter, D.O TRANSPORTATION The student-athlete will normally be responsible for arranging transportation to scheduled medical appointments through their own means of transportation or through a teammate/ friend on campus. The student athlete should notify his or her coach in order to have the coach assist in facilitating transportation to the appointment via the team network. In the event that the student athlete does not have personal transportation, is unable to drive, or cannot find an alternate means of transportation through a friend or teammate; the Hilbert College Sports Medicine staff will arrange with campus safety transportation to and from the medical appointment.

CLEARANCE / RETURN TO ACTIVITY FOLLOWING INJURY Decisions regarding the clearance to participate and/or the return to participation for practice and competition of a student-athlete will be the sole responsibility of the Hilbert College Athletic Training Staff and the Team Physician. USE OF TRAINING ROOM FACILITIES The Athletic Training facilities are intended primarily for use by Hilbert College student-athletes and members of visiting teams. Use by faculty and other staff personnel will be permitted, subject to the prior needs of the athletic program. Use by persons not directly connected with Hilbert College is to be discouraged and is to be authorized only by the Athletic Training Staff. Student-athletes who cannot adhere to the Athletic Training Room Policies listed below will be asked to leave the facility, refused service and/or will be reported to the respective head coach and athletic director for disciplinary action. ATHLETIC TRAINING ROOM POLICIES Athletic Training Room Hours: M-F: 12PM until last practice ends *unless otherwise noted SAT-SUN Pending scheduled practices or events For practices or competitions outside of these hours, the facility will open 1 hour prior to the time the team is required to report, or as specified by the staff athletic trainer. Morning times (prior to 12 PM) are by appointment for prescribed rehabilitation. The Athletic Training Room Will Be Locked Unless the Athletic Trainer is Present Treatments and use of the Athletic Training Room MUST be supervised by the Athletic Trainer 1. Treatments are available to intercollegiate athletes only; in season athletes have priority. 2. Team physicians will be available periodically in the Athletic Training Room. Athletes must see a staff athletic trainer prior to seeing a physician in the Athletic Training Room. 3. During the season, an athlete may be expected to report for morning injury treatment in addition to pre-practice treatment. 4. Athletes must check in with an athletic trainer and sign the treatment log prior to beginning treatment or using any equipment. 5. All equipment is to stay in the Athletic Training Room unless issued by a staff member. Studentathletes will be billed for equipment not returned unless specified by a staff member. Failure to remit payment for equipment not returned will result in records being placed on hold by the College. 6. The Athletic Training Room is a co-ed facility. Proper dress is required for treatment. Please wear shorts and a t-shirt at all times. Come dressed and ready, and leave all personal belongings in your locker. 7. The Athletic Training Room operates on a first-come first-serve basis. Come early if you want to avoid the rush.

8. You are responsible for getting your taping or treatments done in time for all meetings and practices. Remember you are never late for practice because you were detained in the Athletic Training Room, you simply did not arrive early enough. PLEASE EXPECT THE ATHLETIC TRAINING ROOM TO BE BUSY! 9. Appropriate treatment for any and all musculoskeletal injuries will be decided on only by a staff athletic trainer. When necessary (as deemed by the athletic trainer), the team physician will be involved. 10. Continue all treatments until the staff athletic trainer releases you. Do not stop treatments just because your injury feels better. If you miss a treatment, you will be assumed to be healed, and thus okay to practice. 11. Taping will be performed only as deemed necessary by the Sports Medicine Staff. Taping must be accompanied by prescribed exercise and rehabilitation. Braces may be utilized in lieu of routine taping at the discretion of the Sports Medicine Staff. 12. No athlete is permitted to operate athletic training room equipment --- No Self Treatments. 13. Injuries not sustained during intercollegiate functions are not the responsibility of the athletic department. Also, treatment of these injuries will not be the responsibility of the Sports Medicine Staff. 14. No food or drink please. 15. Use of tobacco products is prohibited. 16. The facility is not to be used as a lounge or place to socialize. NO LOITERING. 17. No profanity or roughhousing. 18. No harassment, sexual etc., will be tolerated. This includes language, gestures or implied comments. 19. Individuals not abiding by these policies may be asked to leave the facility, refused service and/or will be reported to the respective head coach and athletic director for disciplinary action. 20. Finally, and most importantly, PLEASE COMMUNICATE WITH YOUR ATHLETIC TRAINERS!!!! We are here to help you not hinder you. WATER BOTTLE POLICY All student athletes will receive a Hilbert College issued water bottle for purposes of hydration during participation in Hilbert College Athletics. If the student athlete loses their water bottle, a $3.00 fee for each water bottle purchased thereafter will be assessed. Purchasing and maintaining a usable water bottle will not be optional. CHANGE OF PRACTICE / GAME SCHEDULES / PRACTICE SCHEDULES Changes in practice / game times and dates must be reported to the Hilbert College Athletic Training Staff at least 48 hours prior to the scheduled change. Failure to adhere to this policy will result in your practice / game being uncovered by the Athletic Training Staff. Pre-season and regular season practice schedules must be approved by both the facilities coordinator, compliance coordinator and the Hilbert

College Athletic Training Staff prior to the start of the season. With a limited sports medicine staff and practice facilities cooperation among all in season athletic teams in regards to scheduling is a must. EVENT COVERAGE Home athletic contests will have a Certified Athletic Trainer present or on call. With multiple sports, the possibility of conflicting schedules and only one Certified Athletic Trainer, it will require flexibility on behalf of all parties involved. The coverage decisions and the physical presence of an athletic trainer at any given time will be determined by the NATA document Appropriate Medical Coverage of Intercollegiate Athletics, which can be viewed at http://www.nata.org/statements/support-statements. PRIORITY OF MEDICAL COVERAGE Medical coverage for athletic teams will be provided on the following basis. "In-Season" sports will be given priority coverage. "Out-of-Season" or "Non-Traditional" sports will be covered by the Athletic Training Staff as available. The Athletic Training Room will generally be staffed by someone from the Athletic Training Staff. In the event there is no one in the Training Room, a sign will be posted detailing the whereabouts of the staff member on duty. It is the coaches' responsibility to provide, on a timely basis, practice and game schedules as well as changes to these schedules. We WILL NOT guarantee coverage for last minute changes to the schedule. MEDICAL COVERAGE FOR VISITING TEAMS The Athletic Training Staff will provide medical coverage for visiting teams. In the event of an injury, we will follow our Emergency Action Plan and the NCAA Sports Medicine Handbook. As soon as possible we will notify the visiting teams Certified Athletic Trainer as to the injury and what care was rendered to the injured athlete. INSURANCE All student-athletes must have on file a properly completed insurance information packet to participate in Hilbert College Athletics. The College's athletic insurance policy requires that the athlete's or parent's insurance be utilized for primary coverage of medical and surgical expenses. Approved expenses not covered by the primary policy will then be submitted to the University's policy for payment. The University will pay no bills until they are first submitted to the athlete's or parent's policy. The College s policy will pay claims that are denied or paid in part, but only after a copy of the "Explanation of Benefits" showing payment record and all original itemized bills are submitted. Fall Sports: print the insurance information document and submit it via mail to their head coach Winter Sports: print and submit the insurance documents to the head coach beginning meeting Spring Sports: print and submit the insurance documents to the head coach by beginning meeting EMERGENCY TRAINING All Hilbert College Head Coaches and Assistant Coaches will be certified in CPR, AED and First Aid as is required by the AMCC. All athletics department members and assistant coaches are strongly encouraged to complete the certification and will be offered certification free of charge each August prior to the start of classes.

ROLE OF THE ATHLETIC TRAINING STUDENT The role of the athletic training student is to prepare the student-athletes for practice and competition (tape, provide ice & water and other modalities, etc. at the event site), and provide basic first aid in the event of an injury. It is NOT the role of the athletic training student to determine the exact injury, severity of the injury or if and when the athlete may return to activity. In the event the Certified Athletic Trainer is not present, the athletic training student may only keep the coach informed as to the condition of the athlete. LIGHTNING SAFETY All coaches and athletes will use the guidelines set forth by the NCAA and the NATA in the event of lightning. The Hilbert College Athletic Department utilizes the available local weather radar and local forecasts and severe weather warnings as they become available, for the detection of lightning during all outdoor athletic seasons. The Hilbert College Athletic Department has decided that when lightning is detected in the range of 8-20 miles, or the flash to bang count approaches 30, all student-athletes and College personnel should immediately seek shelter. Resumption of practices and/or games will be allowed only after there has been a 30 minute span of time since the last visible lightning strike or sound of thunder. The decision to resume will be at the sole discretion of the Hilbert College Athletic Training Staff and the Athletic Director. The Following is a list of Hilbert College athletic fields and the corresponding primary structures for shelter in the event of a lightning strike or other severe weather: Practice Soccer Field - Hafner Recreation Center Game Soccer Field - Trinity Hall Softball Field - Trinity Hall Baseball Field - Paczesny Hall INJURIES TO NON STUDENT-ATHLETES Injuries that occur to individuals that are not student-athletes (i.e. intramural participants, club sports athletes, etc.) are not the responsibility of the Hilbert College Athletic Training Department. We can provide only first aid care and the proper referral for follow-up treatment. Providing more care than this violates the rules and regulations set forth for the Athletic Training Profession by the New York State Department of Professions. Those that are not Hilbert College Student Athletes will be referred to Hilbert's Wellness Center located on the first floor of St. Joseph Hall. The Wellness Center is open Monday - Friday from 9:00 am until 2:00 pm. during the academic year. WRITTEN PROTOCOL WITH TEAM PHYSICIAN We are allowed to practice athletic training services on our student-athletes, as deemed necessary by the New York State Office of the Professions, because we have a written protocol with our team physician. This written protocol is defined as follows: A written agreement developed in conjunction with one or more supervising physicians which identifies and is signed by the supervising physician and the certified athletic trainer. It describes the manner and frequency in which the certified athletic trainer regularly communicates with the supervising physician. It includes standard operating procedures, developed in agreement with the supervising physician and certified athletic trainer that the certified athletic trainer follows when not directly supervised on-site by the supervising physician.
