EMPLOYMENT CONTRACT Assistant Superintendent of Curriculum, Instruction and Technology AGREEMENT made this 1st day of July, 2013, between the BOARD OF EDUCATION OF PRAIRIE-HILLS ELEMENTARY SCHOOL DISTRICT NO. 144, COOK COUNTY, ILLINOIS, hereinafter referred to as the "Board", and Julia Veazey hereinafter referred to as the "Assistant Superintendent of Curriculum, Instruction and Technology. W I T N E S S E T H: A. Employment and Compensation 1. The Board hereby employs Julia Veazey as the Assistant Superintendent of Curriculum, Instruction and Technology and for one (1) school year, commencing July 1, 2013, and terminating on June 30, 2014, with such responsibilities and duties as may be fixed by the Superintendent and by the Board in its policies, rules, regulations and directives. 2. The Board shall pay to the Assistant Superintendent of Curriculum, Instruction and Technology a total annual salary of one hundred seven thousand, seven hundred fifty-nine dollars - ($107,759.00), during the term of this Agreement; which includes the base salary of $98,500.00 plus the TRS employee contribution of (9.40%) in the amount of $9,259.00. The amount will be paid in equal installments in accordance with the rules of the Board governing payments of administrative staff members in the District. 3. The total annual salary stated in paragraph A.2 of this contract, includes the Board s contribution on behalf of the Assistant Superintendent of Curriculum, Instruction and Technology to the State of Illinois Teachers' Retirement System (TRS) in satisfaction 1

of the Assistant Superintendent of Curriculum, Instruction and Technology is required contribution to TRS. In addition, the Board shall pay the Assistant Superintendent of Curriculum, Instruction and Technology s required contribution to TRS for Teacher Retirees Health Insurance (THIS). It is the intention of the parties to qualify all such payments paid by the Board on the Assistant Superintendent of Curriculum, Instruction and Technology s behalf as employer payments pursuant to Section 414(h) of the Internal Revenue Code of 1986, as amended. The Assistant Superintendent of Curriculum, Instruction and Technology does not have any right or claim to said amounts except as they may become available at the time of retirement or resignation from the State of Illinois Teachers Retirement System. Both parties acknowledge that the Assistant Superintendent of Curriculum, Instruction and Technology did not have the option of choosing to receive the contributed amounts directly, instead of having such contributions paid by the Board to the State of Illinois Teachers' Retirement System, and that such contributions are made as a condition of employment to secure the Assistant Superintendent of Curriculum, Instruction and Technology's future services, knowledge and experience. 4. Any salary or other adjustment or modification made during the life of this Agreement shall be in the form of a written amendment and shall become a part of this Agreement, but such adjustment or modification shall not be construed as a new Agreement with the Assistant Superintendent of Curriculum, Instruction and Technology, nor as an extension of the termination date of this Agreement. 5. During the term of this Agreement, the Assistant Superintendent of Curriculum, Instruction and Technology shall hold a valid and properly registered certificate issued by the State of Illinois Teachers' Certification Board qualifying the individual to act as an Assistant Superintendent of Curriculum, Instruction and Technology of the School District. 2

B. Powers and Duties 1. The Assistant Superintendent of Curriculum, Instruction and Technology shall supervise the operation of an attendance center and assume other responsibilities as the Board or Superintendent shall determine necessary. The Assistant Superintendent of Curriculum, Instruction and Technology shall also assume administrative responsibilities and instructional leadership, under the supervision and direction of the Superintendent and in accordance with the laws of the State of Illinois and the policies, rules and regulations of the Board, for the planning, operation and evaluation of the educational program of the Assistant Superintendent of Curriculum, Instruction and Technology s assigned attendance area. 2. The Assistant Superintendent of Curriculum, Instruction and Technology shall submit recommendations, as requested, to the Superintendent concerning the appointment, retention, promotion and assignment of all personnel assigned to the Assistant Superintendent of Curriculum, Instruction and Technology s attendance center and shall keep such other registers and records and make such other reports as may be directed by the Superintendent and the Board or as required by law. 3. The Assistant Superintendent of Curriculum, Instruction and Technology agrees to devote her efforts to the educational program of the School District and shall not undertake nor accept other employment or responsibilities which will conflict with his or her assigned duties. 4. Nothing herein limits the right of the Board to reassign the Assistant Superintendent of Curriculum, Instruction and Technology to other positions or other responsibilities in the District so long as the reassignment does not result in a loss of compensation or benefits. 3

C. Benefits 1. During the term of this Agreement, the Board will provide the Assistant Superintendent of Curriculum, Instruction and Technology with the following benefits: a. Individual insurance coverage per the terms of the District s group hospitalization and major medical benefit plan for the professional staff; b. Term life insurance in a coverage amount of one hundred thousand dollars ($100,000.00). c. Reimbursement of Five Hundred Dollars ($500.00) each year for medical related expenses paid through payroll on the first payroll in December. Documentation of related expenses is not necessary to attain the reimbursement. d. Dependent health insurance coverage provided the Assistant Superintendent of Curriculum, Instruction and Technology contributes 40%, of the premium cost of the dependent coverage. e. Individual and/or dependent dental and vision insurance. f. From the annual salary stated in paragraph A.2 of this contract, the Assistant Superintendent of Curriculum, Instruction and Technology may authorize, in writing, a salary reduction in order that the Board may contribute to an annuity policy for the Assistant Superintendent 4

of Curriculum, Instruction and Technology as described in Section 403(b) and/or Section 457 of the Internal Revenue Code 2. Sick Leave The Assistant Superintendent of Curriculum, Instruction and Technology will be granted sick leave, as defined in Section 5/24-6 of The School Code, of fifteen (15) working days per year. The Assistant Superintendent of Curriculum, Instruction and Technology may use up to three (3) sick days annually as personal days. 3. Holidays The Assistant Superintendent of Curriculum, Instruction and Technology generally shall not be required to work on those days that the school is closed to observe national, state or legal holidays (15 days including but not limited to Christmas, New Year s, Thanksgiving and the day after Thanksgiving) or during winter or spring breaks. The Assistant Superintendent of Curriculum, Instruction and Technology will work over summer, except for 20 days in July at which time vacation days must be taken. 4. Vacation The Assistant Superintendent of Curriculum, Instruction and Technology shall receive twenty (20) work days of vacation per year (July 1, 2013 through June 30, 2014), exclusive of week days and legal holidays. Vacations may not be taken in periods of time in excess of ten (10) consecutive calendar days and are scheduled in consultation with the Superintendent and Board. Vacation must be approved in advance by the Superintendent. D. Evaluation 1. By March 1 of each year of the contract, the Assistant Superintendent of Curriculum, Instruction and Technology s performance shall be evaluated by the Superintendent and a written evaluation of that performance shall be given to the Assistant Superintendent of Curriculum, Instruction and Technology. The evaluation will consider the Assistant Superintendent of Curriculum, Instruction and Technology s duties and responsibilities, management ability and competence. The evaluation will specify, 5

with supporting reasons, the Assistant Superintendent of Curriculum, Instruction and Technology s strengths and weaknesses and will align with the Illinois Professional Standards for School Leaders or research based district standards. The evaluation will also consider student growth measures. After such evaluation, the parties shall schedule a meeting to review the evaluation and determine, if necessary, the terms and conditions of the continued future employment of the Assistant Superintendent of Curriculum, Instruction and Technology. It is the intent of the parties that the evaluation fully comply with Section 10-23.8a of the School Code. 2. Prior to October 1 of each year, the Board and Superintendent shall consult with the Assistant Superintendent of Curriculum, Instruction and Technology to determine the Assistant Superintendent of Curriculum, Instruction and Technology s short and long term objectives. Subsequent evaluations shall be based in part on the Assistant Superintendent of Curriculum, Instruction and Technology s performance based upon the agreed upon objectives. E. Termination 1. This employment contract may be terminated by: a. Mutual agreement of the Parties; b. Permanent disability of the Assistant Superintendent of Curriculum, Instruction and Technology; c. Discharge of the Assistant Superintendent of Curriculum, Instruction and Technology for cause; or 6

d. Death of the Assistant Superintendent of Curriculum, Instruction and Technology 2. The Board may terminate this Agreement for reasons of permanent disability or incapacity at any time after the Assistant Superintendent of Curriculum, Instruction and Technology has exhausted her accumulated sick leave, personal leave, FMLA entitlements and either has been absent from her employment for a continuous period of three (3) months or presents the Board a physician's statement certifying that he or she is permanently disabled or incapacitated. All obligations of the Board shall cease upon written notice of termination for permanent disability or incapacity, provided that the Assistant Superintendent of Curriculum, Instruction and Technology shall have the right of notice and hearing before the Board if the Assistant Superintendent of Curriculum, Instruction and Technology so requests. The Board reserves the right to require the Assistant Superintendent of Curriculum, Instruction and Technology to submit to a medical examination, either physical or mental, whenever the Board deems the Assistant Superintendent of Curriculum, Instruction and Technology disabled. Such examination shall be performed by a physician licensed to practice medicine in all its branches, who is selected and paid by the Board. The Assistant Superintendent of Curriculum, Instruction and Technology expressly agrees that the physician shall prepare a detailed report of the state of his/her physical or mental health and submit it to the Board of Education. 3. Discharge for cause shall be for any conduct, act, or failure to act by the Assistant Superintendent of Curriculum, Instruction and Technology which is detrimental to the best interests of the School District. Reasons for discharge for cause shall be given in writing to the Assistant Superintendent of Curriculum, Instruction and Technology, who shall be entitled to notice and a meeting with the Board to discuss such causes and to present information to the Board as to why his/her employment should not be terminated. If the Assistant Superintendent of Curriculum, Instruction and Technology chooses to be 7

accompanied by legal counsel, he or she shall bear any costs therein involved. Board meeting shall be conducted in executive session. The F. Notice All notices under this Agreement shall be deemed sufficient if given in writing and served upon the Assistant Superintendent of Curriculum, Instruction and Technology and the President of the Board personally or by certified mail, return receipt requested, addressed to the party as set forth in this Agreement, or at such other address as may be hereinafter furnished by the Assistant Superintendent of Curriculum, Instruction and Technology in writing. G. Miscellaneous 1. This Agreement has been executed in the State of Illinois, and shall be governed in accordance with the laws of the State of Illinois in every respect. 2. Paragraph headings and numbers have been inserted for convenience of reference only. In the event of any conflict between any such headings or numbers and the text of this Agreement, the text shall control. 3. This Agreement may be executed in one or more counterparts each of which shall be considered an original, and all of which taken together shall be considered one and the same instrument. 4. This Agreement contains all the terms agreed upon by the parties with respect to the subject matter of this Agreement and supersedes all prior agreements, arrangements, and communications between the parties, whether oral or written, concerning such subject matter. No other benefits referred to in any other documents or 8

Board policies are due or owing the Assistant Superintendent of Curriculum, Instruction and Technology. 5. If a court having jurisdiction issues a final decision declaring any provision of this Agreement to be unlawful or unenforceable, all other provisions of the Agreement shall remain in force and effect.
