One should not try to deduce solely from the classifications used whether or not there was an absence of care or whether lessons can be learned which would materially have affected what happened. That information primarily arises frorn the detailed consideration of individual cases. The data is of course recorded as part of a system which is designed to identify and disseminate learning which will help to improve the safety and quality of services and I understand that the Health and Social Care Board (HSCB) will be providing more detailed information on the process whereby individual Serious Adverse Incidents are investigated. The Departmental panel will also be able to discuss with the Inquiry how this type of information feeds into the discharge of the Department's functions. Adverse Incidents Serious Adverse Incident Reporting completed within 12 weeks following the date the incident was discovered or from the date of the SEA.
