Indiana University is committed to offering a benefits program that provides value while containing costs. This commitment includes taking advantage of IU s size to offer optional benefit plans at attractive group rates. IU employees, Graduate Assistants, and Retirees now have access to a greater variety of plans in this offering of the IU Voluntary Benefits Program. This program is designed around products that can help save money and protect assets. This brochure provides an overview of each plan and its features. For details, visit www.iuvoluntarybenefits.com. The Web site includes premiums based on factors such as type of coverage, age, and effective dates. Plans in this program are fully paid by the enrollee. Most plans can be paid for by a single, after-tax payroll deduction. SPECIAL ENROLLMENT PERIOD The first opportunity to select Voluntary Benefits begins September 1. September 1 to September 30, 2009, is a special enrollment period allowing enrollment in certain plans with more favorable conditions regarding proof of good health. During the month of September and anytime thereafter, enrollment can be done online at www.iuvoluntarybenefits.com. Auto, Home, and Renters Insurance requires a brief phone call to confirm premium, coverage, and effective date. Enrollees can also call 1-866-795-6272 to speak with a customer service representative, Monday through Friday, from 9 a.m. to 6 p.m. Eastern time. The groups listed below are eligible for the checked benefits. Eligible family members will be listed in the part of the plan description. The IU Voluntary Benefits Program is provided through Marsh Voluntary Benefits (not associated with Marsh Supermarkets) and offered to employees, Graduate Assistants, and Retirees of Indiana University on a voluntary basis. Indiana University does not endorse any of the benefits offered through this program. Marsh Voluntary Benefits, and not Indiana University, is the administrator of the program. Indiana University will provide the program administrator with the necessary records and information to verify eligibility under the program. Indiana University reserves the right to amend or terminate all or any part of this plan.

LONG-TERM CARE INSURANCE Long-term care insurance offers important coverage that is generally not provided under medical and long-term disability (LTD) benefits or fully covered through Medicare or Medicaid. A Group Long-Term Care Insurance plan from John Hancock will help an individual manage the expenses associated with a nursing home stay and home health care. Long-term care can become necessary if an employee needs assistance with activities of daily living, like eating and dressing. The need for long-term care services can occur at any point during an individual s life due to an accident or illness. Coverage is purchased in Daily Maximum Benefits (DMB). The DMB is the most the insurance may pay for covered services received on any day. Individuals choose the amount they would like to have available per day. There is a choice of five DMBs: $100, $150, $200, $250, $300. Each DMB has a 5-year benefit duration. During the Special Enrollment Period, employees in pay status do not need to provide proof of good health. All other eligible individuals (including employees enrolling after the Special Enrollment Period) must provide proof of good health. Those employees enrolling during the Special Enrollment Period, September 1 to September 30, 2009, will have a November 1, 2009 effective date if they meet all applicable eligibility requirements. Full-time and part-time employees and Retirees and their spouses/domestic partners, parents, grandparents, adult children, and certain other family members. SHORT-TERM DISABILITY INSURANCE Short-term disability coverage helps employees protect their income if they experience a disability that results from a non-work related accident, illness, or health condition. Once the employee s paid time-off has been used, short-term disability benefits pay weekly benefits equal to 60 percent of the employee s salary up to a maximum of 90 days from the date of disability. This benefit underwritten by The Standard is paid directly to the employee to help cover expenses. This plan requires a minimum number of employees to participate in order for it to be viable. Should enrollment be lower than the minimum, enrollees will be notified. This plan is not available for individuals to keep upon separation of employment with Indiana University. During the Special Enrollment Period, employees can enroll with an effective date of coverage on November 1, 2009, and the minimum waiting period for benefits to begin is 15 days from the date of disability. When enrolling after the Special Enrollment Period, there is a 60-day waiting period for illness disability only during the first 12 months of coverage. Full-time Staff employees only.

CRITICAL ILLNESS INSURANCE Medical insurance covers many expenses associated with critical illness. However, there are many other expenses to consider such as deductible and co-insurance costs and everyday living expenses. This plan offered by Unum provides financial relief to an already stressful situation by providing a lump-sum payment. The payment can be used at the insured s discretion upon diagnosis of a covered serious illness, regardless of any other insurance coverage the employee may have. Covered conditions include: Cancer Heart attack Stroke Carcinoma in situ Coronary artery bypass surgery End stage renal (kidney) failure Blindness Major organ failure Benign brain tumor Coma as the result of Permanent paralysis as the result of severe traumatic brain injury a covered accident See policy definitions for complete details about these covered conditions. Coverage may be purchased in lump sum benefit amounts between $5,000 and $50,000 for employees, and $5,000 and $30,000 for spouses/domestic partners. Dependent children are covered on the employee s plan with benefits of 25 percent of the employee s coverage. During the Special Enrollment Period, employees can enroll in up to $10,000 of guarantee issue coverage (without answering health questions), and a spouse/domestic partner can enroll by answering a few health questions. The effective date is November 1, 2009. For higher coverage amounts and enrollments beginning after the Special Enrollment Period, a full medical questionnaire is required. Full-time and part-time employees and their spouses/domestic partners and children. This insurance provides limited benefits. The policy or its provisions may vary or be unavailable in some states. The policy has exclusions and limitations which may affect any benefits payable. Underwritten by: Unum Life Insurance Company of America. CU-9364 VISION WEAR PROGRAM This vision wear program offered by EyeMed Vision Care provides benefits and discounts for glasses and contacts with services received from private ophthalmologists and optometrists and retail optical providers. Benefits and discounts are given at the time of service or when a purchase is made. The best value is when an EyeMed Vision Care network doctor is used. This plan is not available for individuals to keep upon separation of employment with Indiana University. Participants must enroll during the Special Enrollment Period for vision coverage beginning November 1, 2009, through the end of 2010. The next opportunity to begin or end coverage will be the annual enrollment period in October/November, 2010, with an effective date of January 1, 2011. Full-time and part-time employees, Graduate Assistants, and Retirees are the primary eligible individuals. Spouses/domestic partners and children may be covered only when the primary individual is enrolled in a coverage level that includes dependents. Underwritten by Fidelity Security Life Insurance Company of Kansas City, Missouri, except in New York. This is a snapshot of your benefits. The Certificate of Insurance is on file with the employer.

AUTO, HOME, and RENTERS INSURANCE The Auto, Home, and Renters program is an innovative solution that delivers multiple choices and savings on insurance along with personalized customer service. Individuals can receive multiple, side-by-side quotes and special group discounts from three of the nation s most respected insurance companies. Individuals receive reduced premiums on a wide variety of insurance policies including automobile, motorcycle, homeowners, condominium, mobile homes, renters, boats, recreational vehicles, personal excess liability, and flood insurance. Additional discounts based on individual circumstances include: - Payroll deduction discount - Multi-policy discount - Multi-vehicle discount - Anti-theft device discount Ability to compare quotes from four insurance companies with one phone call or one visit to the Web site Coverage can begin as early as the next day More information about this option will be mailed in late September. Starting October 1, 2009, web enrollment options will be available followed by a brief phone call required to confirm premium, coverage, and effective date. Full-time and part-time employees, Graduate Assistants, and Retirees and their spouses/domestic partners and dependent children under age 23. THE HOME MORTGAGE PROGRAM The Home Mortgage Program, a benefit provided by Wells Fargo Home Mortgage to employees, Graduate Assistants, and Retirees of Indiana University and their immediate family members, provides access to home financing that can help advance immediate and long-term financial goals. ID THEFTSMART TM PROGRAM The process of restoring one s identity can be overwhelming and time-consuming, and victims can spend hundreds of hours working to restore their name and credit file information after an identity fraud occurs. The ID TheftSmart program provides continuous monitoring of an individual s credit report. If an individual finds out that her/his identity has been tampered with, the Licensed Investigators at ID TheftSmart will provide assistance. An ID TheftSmart membership includes: A tri-merged credit report from all three national credit repositories Confidential Credit Smart Score, including up to four factor descriptions that help explain why a credit score is as it appears Access to a continuous credit monitoring service that will alert the member to five types of credit activity commonly associated with identity theft Assistance of professional Licensed Investigators to consult with the member who has questions or concerns about identity theft and fraud, and to perform searches of non-credit databases. If the member becomes a victim of identity fraud, the Investigator will restore his or her identity to pre-theft status Enrollment can take place at any time. ID TheftSmart is offered by Kroll Inc. and Marsh Affinity Group Services, a service of Seabury & Smith, Inc. (Marsh). PET INSURANCE Treatment for a pet can cost hundreds or thousands of dollars. A pet insurance policy from Veterinary Pet Insurance (VPI), a licensed property and casualty insurance company that has been protecting pets since 1982, offers pet medical care insurance at affordable costs.
