Funding: NYS OASAS, Public Assistance/No client fees charged. Services: Room and board in a structured, secure setting. Medically monitored withdrawal services. 24 hour hotline. Assessment for individual counseling/groups. Referrals to appropriate treatment. Length of stay: 1-14 days. Referral Sources: Self/family/friends: other drug/alcohol treatment providers. Parole and Re-entry can NOT be part of screening process and was suggested that if they come from half-way house or established housing for 6 months with good standing, that it would be a good time to apply. Staff provides life skills training and support in a person-centered environment with goals of assisting residence to achieve chosen community living goals. Permanent Housing (Jessica Isaacs ext. 251) Outreach and Retention Program (Adult Housing) Intensive Independent Living Program (Shari Weiss ext. 232) ages 20-29 Sheltered Plus Care (Sabrina Garcia ext. 223) HUD definition for homelessness, application must be submitted along with a letter of disability from physician, mental health professional or substance abuse provider, survivors of domestic violence need only a letter indicating DV has been documented. DSS, private pay. *This program works from Housing First approach/harm reduction model, residents do not have to be clean and sober to secure housing. 12

Transitional Housing Young Woman's Achievement Program: Shari Weiss ext. 232 HUD definition of homelessness, female age 18-23. DSS, private pay *This program works from Housing First approach/harm reduction model, residents do not have to be clean and sober to secure housing. **The majority of women entering into permanent housing do so through the Emergency Housing Program. This is not a requirement in all cases. It would be determined on a case by case basis. Women can reside in the Emergency Housing Program (EH) for up to 21 days. From the day of admission to the EH, guests are assisted by a case manager to obtain information and referrals to other social service agencies as well as provided with information about local food pantries, clothing stores, etc. EH guests have access to a computer lab where they may look for housing via the internet. Lists of local landlords are provided to all guests of EH. ***Residents of the long term housing programs and transitional housing have access to an inhouse Vocational/Educational Counselor to address daily living skills, budgeting and funding issues, as well as their individual social worker or case manager. Residents of transitional and long term housing programs are also afforded use of the computer lab housed within the building. 13
