The Phyllis E. Edwards Nursing Scholarship Award Lighting up every room that she entered, Phyllis Edwards was a warm, humorous, compassionate, and dedicated pediatric staff nurse. She was later promoted to charge nurse and eventually pediatric nurse manager at the Henry Ford Wyandotte Hospital, where she worked throughout her more than 30-year career. In loving memory of Phyllis Edwards, her family has graciously chosen to establish an endowed nursing scholarship fund at Beaumont Health System. With this gift, the family of Phyllis Edwards would like to encourage those who wish to pursue a degree in nursing, with a preference for supporting those interested in pediatric nursing. The scholarship will be awarded annually to the person who most represents the qualities that Phil possessed: kindness, patience and devotion to children. THE AWARD PROGRAM Beaumont Health System is pleased to announce The Phyllis E. Edwards Nursing Scholarship Award. Established through a generous donation from the Edwards family, this scholarship is awarded annually to students interested in pursuing a career in nursing. This scholarship program is administered by Scholarship Management Services, a division of Scholarship America. Scholarship Management Services is the nation s largest designer and manager of scholarship and tuition reimbursement programs for corporations, foundations, associations and individuals. Awards are granted without regard to race, color, creed, religion, sexual orientation, gender, disability or national origin. ELIGIBILITY Applicants to The Phyllis E. Edwards Nursing Scholarship Award, sponsored by Beaumont Health System, must be: Majoring in nursing and have been accepted into an accredited nursing program seeking an ADN or BSN degree at a two- or four- year college or university located in Michigan. Enrolled in a full-time undergraduate course of study. Preference will be shown o to applicants with an expressed interest in pursuing a career in pediatric nursing o to applicants who reside in Southeast Michigan s Downriver or Grosse Ile communities AWARDS One award of no less than $1,000 will be presented annually to a qualified applicant. The award is not renewable, but students may reapply to the program each year they meet eligibility requirements. Awards are for undergraduate study only.

APPLICATION Interested students must complete the application and mail it along with a current, complete transcript of grades to Scholarship Management Services postmarked no later than April 20. Students currently or previously enrolled in college or vocational-technical school must include all college or vo-tech transcripts of grades from each previously attended postsecondary school. Grade reports are not acceptable. Online transcripts must display student name, school name, grade and credit hours earned for each course, and indicate the term in which each course was taken. Applicants are responsible for gathering and submitting all necessary information. Applications are evaluated on the information supplied; therefore, answer all questions as completely as possible. Incomplete applications will not be evaluated. All information received is considered confidential and is reviewed only by Scholarship Management Services and the selection committee. Applicants will receive acknowledgment of receipt of their application. If an acknowledgment card is not received within three weeks, applicants may call Scholarship Management Services to verify that the application has been received. SELECTION OF FINALISTS AND RECIPIENTS Scholarship recipients are selected on the basis of academic record, demonstrated leadership and participation in school and community activities, honors, work experience, goals and aspirations stated in the application, unusual personal and family circumstances, and an outside appraisal. Financial need is not considered. Selection of finalists is made by Scholarship Management Services and a Beaumont Health System selection committee will determine the recipient. All applicants agree to accept the decision as final. Applicants will be notified in May. Not all applicants to the program will be selected as recipients. Students may reapply to the program each year they meet eligibility requirements. PAYMENT OF SCHOLARSHIPS Scholarship Management Services processes scholarship payments on behalf of Beaumont Health System and The Phyllis E. Edwards Nursing Scholarship Award. Payment is made on August 15 via a check mailed to the recipient s home address which is made payable to the student s school. OBLIGATIONS Recipients have no obligation to Beaumont Health System. They are, however, required to notify Scholarship Management Services of any changes in address, school enrollment, or other relevant information and to send a complete transcript when requested. REVISIONS Beaumont Health System reserves the right to review the conditions and procedures of this scholarship program and to make changes at any time, including termination of the program. ABOUT BEAUMONT HEALTH SYSTEM Beaumont opened on January 24, 1955, as a 238-bed community hospital in Royal Oak, Michigan. Today, that hospital is a 1,070-bed major academic and referral center with Level 1 trauma status. It was Michigan s first Magnet-designated hospital for nursing excellence and it is an associate member of the National Association of Children s Hospitals and Related Institutions. A second Beaumont hospital opened in Troy in 1977 as a 200-bed acute care community teaching hospital. It is now a 406-bed facility. In 2009, Troy also received Magnet status for nursing excellence. In October 2007, Beaumont became a regional health care provider when it acquired a third community hospital with 250 beds in Grosse Pointe. Ninety-one medical and surgical specialties are represented on the Beaumont medical staffs of more than 3,700 physicians. The Joint Commission has accredited Beaumont. Beaumont is a major teaching facility that has 37 accredited residency and fellowship training programs with about 400 residents and fellows and partnered to establish the Oakland University William Beaumont School of Medicine in 2011. For undergraduate training, it is affiliated with the University of Michigan and Wayne State University schools of medicine. Beaumont also has a nursing affiliation with Oakland University (OU), including a top-ranked certified registered nurse anesthetist school.
