The information contained in the Fee Data Collection Spreadsheet (the information) that has been provided voluntarily to the Australian Prudential Regulation Authority (APRA) will be released to the Australian Securities and Investments Commission (ASIC). 2. The information will be made available on ASIC's FIDO website. 3. The information will be made available on ASIC's FIDO website for a period of 3 months. General instructions for completing the data collection spreadsheet: 1. Please enter all products on one spreadsheet, do not create a new spreadsheet for each product. 2. If you are using ASIC Class Order [CO 04/1030] In-use notices for employer-sponsored superannuation, you have two options: Provide details for each subplan as described below; OR Provide identical fee information for each subplan. To do this, enter as a minimum Contribution Fee or Management Cost the figure corresponding to lowest cost product based upon the cost of fund minimum. Enter the maximum costs corresponding to the highest cost product based upon the cost of fund maximum. This data must be based on the examples of annual fees and costs and associated notes in your PDS as set out in Division 5 of Schedule 10 of the Corporations Regulations (as amended by Corporations Amendment Regulations 2005 (No.1)).
