SOUND NURSERY PARENT QUESTIONNAIRE Dear Parent(s) We would like to give you the opportunity to complete a questionnaire to help us improve our service. Please tick the appropriate box and feel free to add any comments in the spaces provided. We have also added a section to find out children s ideas. Please feel free to fill in this section with your child. We have based the questionnaire on the Care Commission quality themes. Please complete and return to staff by the end of February Thank you. How long has your child attended Sound Nursery? Overall are you happy with the service YES NO Please tick the most appropriate box and add comments in the space provided. SECTION 1 QUALITY OF CARE AND SUPPORT My child is happy at nursery My child enjoys the activities at nursery I am happy with the quality of care my child receives.
