Thanks for your interest in working with our students! We work hard to provide an environment for spiritual growth in a context of healthy, positive relationships with adults. We take seriously our calling to introduce our students to Jesus Christ, to help them develop their personal relationship with Him, and to provide an environment where they feel loved, accepted and nurtured. In addition, we take seriously our responsibility to shield our students from sexual abuse, to protect our adult leaders from accusations of sexual abuse, and to limit the exposure of the church to legal risk and liability. To accomplish this, we ask that all paid staff, as well as those volunteers who'll be working with students ages 21 and under, complete our application process. Since you're looking into working with students in middle school and/or high school, we need you to carefully and thoughtfully fill out the attached application. So, you might be asking What's involved in the volunteer application process? When you fill out the application, you authorize us to check personal references and to request a background check for criminal records. Please thoroughly and honestly complete all forms. On what basis does the church approve someone to work with students? We invite into ministry only those applicants a) who have no previous conviction for sexual or physical abuse of children; b) for whom we receive positive responses from their personal and professional references; and c) who meet our qualifications and ministry standards of the position for which they are applying. If background checks raise any questions, the individual will be asked to meet with the MDPC Executive Pastor to clarify the questionable issues prior to being placed in a position relating to students. Who will see this application? The completed application and any subsequent information on you will be available only to the MDPC Youth Staff, the Executive Pastor and Human Resources Personnel. Once the approval process has been completed, your application and references will be maintained in a secure file. Thanks for understanding that this paperwork is about protecting you and our students. We appreciate your willingness to help us achieve our mission to students!

I authorize any references or churches listed in this application to give you any information (including opinions) that they may have regarding my character and fitness for youth and/or college ministry. In consideration of the receipt and evaluation of this application by MDPC, I hereby release any individual, church, youth organization, charity, employer, reference, or any other person or organization, including record custodians, both collectively and individually, from any and all liability for damages of whatever kind or nature that may at any time result to me, my heirs, or family, because of compliance or any attempts to comply, with this authorization. I further state that I HAVE CAREFULLY READ THE FOREGOING RELEASE AND KNOW THE CONTENTS THEREOF, AND I SIGN THIS RELEASE AS MY OWN ACT. This is a legally binding agreement, which I have read and understand. Print Name Signature Witness Date Thank you so much for taking the time to complete this lengthy application process. It is great that you aspire to teach, but please understand, as we do, that the teacher s responsibility is great because their words and example affect others spiritual and emotional lives. James 3:1 states, "Not many of you should presume to be teachers, my brothers, because you know that we who teach will be judged more strictly." Our students are young, impressionable, and trusting. In many ways, their opinion of and relationship with this church directly effects their opinion and relationship with God. It is hard for them to separate the two. That is why we, as the church, must take every effort to ensure that the adult relationships we are encouraging our students to embrace are healthy ones. We feel called and compelled to create an environment for our students where they feel loved, accepted and comfortable. The Youth Ministry at MDPC exists for the students, but we need the help of godly adults to make it work. We have been blessed with an outstanding youth staff and great resources. But the program is too large for the youth staff to do alone. We need, and already have, a team of leaders that are devoted to making the ministry successful. We are honored and thankful that you will possibly be joining this team. Over the following days we will be reading, discussing, thinking and praying over your application and involvement in this ministry. We invite you to pray along with us and we look forward to meeting with you. Blessings, MDPC Youth Staff
