GENERAL PLAN APPROVAL AND/OR GENERAL OPERATING PERMIT APPLICATION INSTRUCTIONS General Permit BAQ-GPA/GP-5 Natural Gas Compression and/or Processing Facilities GENERAL INFORMATION 1. The owner or operator of any natural gas compression and/or processing facility proposing to operate under the general plan approval and/or operating permit BAQ-GPA/GP-5 must comply with the terms and conditions specified therein. Failure to conform to the applicable laws, rules and regulations and terms and conditions of this permit, for any reason, are grounds for the revocation or suspension of the permittee's authorization to operate under this permit. 2. In accordance with Condition 4 of Section A of the General Permit, an authorization to construct, modify or operate cannot be issued to a facility if the total emissions from all air contamination sources located at the facility exceed the Title V major facility threshold during any consecutive a 12-month rolling period. The emissions from all sources and associated air pollution control equipment located at a natural gas compression and/or processing facility shall not equal or exceed on a 12-month rolling sum basis: (i) Nitrogen oxides (NOx), Carbon monoxide (CO), Sulfur oxides (SOx), Particulate matter with an aerodynamic diameter less than 10 microns (PM10) and 2.5 microns (PM2.5) 100 tons. (ii) Volatile organic compounds (VOCs) 50 tons. (iii) Any individual hazardous air pollutant (HAP) 10 tons. (iv) Total hazardous air pollutants (HAPs) 25 tons. (v) Greenhouse gases, expressed as carbon dioxide equivalent (CO2e) 100,000 tons. (vi) In addition, the emissions from all sources and associated air pollution control equipment located at a natural gas compression and/or processing facility in Bucks, Chester, Delaware, Montgomery, or Philadelphia counties shall not equal or exceed 25 tons of NOx and VOCs emissions. 3. BAQ-GPA/GP-5 is applicable to any of the following air contamination sources: spark ignition internal combustion engines (SI ICE), simple cycle gas turbines, centrifugal compressor, glycol dehydration unit and associated equipment including gas-condensateglycol ( GCG ), separator (flash tank separator), natural gas fractionation (such as depropanizer, de-ethanizer, de-butanizer), storage vessel(s), equipment leaks, pneumatic controllers and sweetening units used in natural gas compression and/or processing facilities. 4. Authorization to use BAQ GPA/GP-5 is valid for a fixed term of five years. The applicable fees are detailed in Condition 13 of Section A of the General Permit. An application for renewal and the renewal fee must be submitted 30 days prior to expiration of the General Permit. - 1 -

5. The application and fees specified in Condition 13 of Section A of BAQ-GPA/GP-5 shall be submitted to the appropriate Regional Office. 6. The permittee may not transfer authorization to operate under this General Permit. The new owner shall submit a new application and fees as described in Condition 13 of Section A of the General Permit. 7. If the natural gas compression and/or processing facilities cannot be regulated by the requirements of this general permit, an applicant must apply for a plan approval under Chapter 127, Subchapter B in lieu of this general plan approval BAQ-GPA/GP-5. APPLICATION INSTRUCTIONS 8. Every section of the application form for authorization to use General Permit BAQ- GPA/GP-5 must be filled completely, otherwise the application would be deemed incomplete. Any section that is not applicable must be marked as N/A. Estimated emissions must include emissions from all of the sources at the facility. This includes stack and fugitive emissions. Contents in the application form are self-explanatory. Still, for convenience of the applicant further explanations have been provided as following: SECTION A. (APPLICATION TYPES) 9. An applicant shall check the appropriate box that matches the type of authorization the applicant is seeking. Based on the application type checked by the applicant and the fees listed in Condition 13 of Section A of the General Permit, the payable amounts for the GP- 5 authorization fees may vary. The GP-5 authorization fee by means of one check, payable to the Commonwealth of Pennsylvania, Clean Air Fund, must be included with the application. 10. An owner or operator of a new or existing facility, applying for a BAQ-GPA/GP-5 authorization, shall specify the first GP-5 authorization date as asked in Section A of the application form to help ascertain the applicable Best Achievable Technology (BAT) for the source(s). 11. Department approves all ownership transfers in writing. Within thirty (30) days after a change of ownership of the facility, the new owner or operator shall submit to the Department a GP-5 Application, compliance review form, and all applicable fees. SECTION B. (OWNER INFORMATION) 12. Enter the owner information in this section. If the facility has an existing permit, use the same information. SECTION C. (OPERATOR INFORMATION) 13. Enter the operator information (if different from the owner information) in this section. SECTION D. (CONTACT INFORMATION) 14. Enter the contact information in this section. - 2 -

SECTION E. (PERMIT INFORMATION) 15. An applicant of an existing facility shall provide current and past authorization numbers as indicated in the application. Previously exempted sources have to be listed with date of exemptions. SECTION F. (APPLICANT S CHECKLIST) 16. Verify that the appropriate enclosures are included with the application by checking the boxes. 17. The GP-5 application must be submitted with the General Information Form (GIF). 18. An applicant for BAQ-GPA/GP-5 must complete compliance review forms in accordance with 25 Pa. Code 127.412. Use forms 2700-PM-AQ0004 and 2700-PM-AQ0005 as appropriate from DEP s online elibrary. 19. An application fee is required each time the facility owner or operator installs or modifies an existing source at the natural gas production, compression and/or processing facilities in accordance with this General Permit. 20. An applicant for BAQ-GPA/GP-5 shall complete Attachment-A (the Questionnaire and Checklist for Single Source Determination) of the Application for Authorization to Use GP-5. 21. A facility owner or operator proposing to use the General Permit shall notify the local municipality and county governments where the air pollution source is to be located that the applicant has applied for the authorization to use GP-5. The notification shall clearly describe the proposed sources and/or modifications. The owner or operator shall also submit to the Department proof of submittal of the municipal notification along with a copy of the Application for Authorization to Use GP-5. 22. An applicant for BAQ-GPA/GP-5 shall review the Department issued Guidance for Performing Single Stationary Source Determinations for Oil and Gas Industries, Document number: 270-0810-006 and Fact Sheet, Document number 2700-FS-DEP4349 prior to generating the Map/Layout of the facilities under common control required in Section F of the application form. 23. An applicant for BAQ-GPA/GP-5 shall provide map/layout of adjacent facilities under common control with SIC code, permit number of each source, and distances between boundaries of compressor station(s), the well(s), and associated natural gas processing plant(s) printed clearly on the map/layout required in Section F of the application form. 24. An applicant for BAQ-GPA/GP-5 authorization must obtain Erosion and Sediment Control General Permit (ESCGP-1) before conducting any earth disturbance activities. An applicant also shall obtain Pennsylvania Natural Diversity Inventory (PNDI) review receipt and if required a clearance letter. The clearance letter should be provided if the search indicates a potential impact. The clearance letter is not required for administrative completeness, but must be provided prior to issuance of the authorization. See the - 3 -

Provide a signature, date, and print name of the responsible official as defined in 25 Pa. Code Section 121.1 (relating to definitions) certifying this GP-5 application. Applications without a signed certification will be returned as incomplete. SECTION H. (FACILITY INFORMATION) 26. The physical location of each stack must be provided by latitude and longitude measures in appropriate degrees, minutes, and seconds. 27. An applicant seeking BAQ-GPA/GP-5 authorization shall list all sources of the facility that the application would cover. Enter the information requested in Sections H1 through H8 for the equipment and control devices being installed or modified as part of this General Permit. SECTION H1 and H2 (INTERNAL COMBUSTION (IC) ENGINE(S) & SIMPLE CYCLE NATURAL GAS TURBINE(S) INFORMATION) 28. Calculations for Estimated Atmospheric Emissions (under Emissions Data table) for engines and turbines shall be based on Post Control Emissions Rates, maximum hours of operation of 8760 hrs. and maximum rated capacity (BHP) of the engine or turbine. Attach calculations with the application. 29. Provide all engine and turbine control information in sections H1 and H2. Use section H8 only for providing information for control equipment that are associated with other process equipment like Dehydrator, Regenerator, Tank (s) etc. SECTION H3. (DEHYDRATOR AND ASSOCIATED EQUIPMENT INFORMATION) 30. Provide detailed control equipment information using appropriate form(s) under section H8. Estimated Atmospheric Emissions, in TPY units for Dehydrator and associated equipment (under Emissions Data table), shall be calculated using controlled emission rates and maximum hours of operation of 8760 hrs. Describe how the estimated emission values were determined. Attach calculations. SECTION H4. (FLASH TANK and/or REGENERATOR INFORMATION) 31. See SECTION H3 above. SECTION H5. (FRACTIONATOR INFORMATION) 32. See SECTION H3 above. SECTION H6. (CONDENSATE OR MISCELLANEOUS STORAGE TANK INFORMATION) - 4 -

33. See SECTION H3 above. SECTION H7. (STORAGE TANK (Pressure vessel)) 34. If the pressure relief valve discharges to an overflow tank, provide information of the overflow tank and control equipment (if any) by filling out Sections H6 and H8 separately. SECTION H8. (AIR POLLUTION CONTROL EQUIPMENT INFORMATION) 35. Provide information about various air pollution control equipment in Sections H8.1 through H8.4. Do not use this section for engine and turbine controls information. Use sections H1 and H2 for engine and turbine controls. Sections H8.5 for Other Air Pollution Control Equipment may include any other devices not specified anywhere in the application form. SECTION H9. (FUGITIVE EMISSIONS FROM COMPONENT LEAKS) 36. An applicant for BAQ-GPA/GP-5 shall use Leak Emission Factors from EPA Protocol or other Department approved methods for calculating fugitive emissions due to leaks from component types covered under this GP-5 application. Estimated Atmospheric Emissions from component leaks will be estimated based on maximum hours of operation of 8760 hrs. SECTION H10. (ESTIMATED ATMOSPHERIC EMISSIONS FROM EACH SOURCE COVERED UNDER THIS GENERAL PERMIT) 37. Provide Estimated Atmospheric Emissions from each individual source including control equipment and component leaks. Use controlled/outlet emission rates and maximum hours of operation of 8760 hrs to estimate atmospheric emissions (TPY). SECTION H11. (TOTAL ESTIMATED EMISSIONS FROM ALL SOURCES COVERED UNDER THIS GENERAL PERMIT) 38. Provide total Estimated Atmospheric Emissions for each pollutant from all sources in the facility including component leaks. - 5 -
