NO-COST PREVENTIVE CARE DRUGS INFORMATION FOR NON-GRANDFATHERED ASO GROUPS The Affordable Care Act (ACA) requires that certain preventive care drugs and drug categories be covered at no cost to health care consumers in select populations. This means your plan participants don t have to pay a copay* or coinsurance, even if they haven t met their deductible. As of January 1, 2015, MVP Health Care will utilize an automated solution offered by CVS/caremark to ensure that ASO plan participants are correctly charged at the point of sale for these medications, in accordance with the ACA. The following lists explain: Which medications or vaccines (administered by a pharmacist) are covered in full beginning January 1, 2015, which may represent a change for some ASO plans in the smoking cessation product and contraceptive drug categories Who they are covered for (such as children up to age 6 or adults age 65 or older) What health condition or illness they help prevent Important: A doctor must write a prescription for these medications to be covered, even if they are listed as over-the-counter. TIPS FOR USING THE LIST The dosage form is how the product is supplied. For example, tablet, capsule, liquid, syrup or chewable tablet. Generic or brand name is listed if only that product type is covered. FE = ferrous sulfate (iron) EE = ethinyl estradiol hr = hour IM = intramuscular IU = international unit mcg = microgram mg = milligram ml = milliliter LEGEND oral = taken by mouth OTC = over-the-counter product Rx = prescription product soln = solution SR = sustained release susp = suspension TD = transdermal OVER -THE- COUNTER MEDICATIONS

A doctor must write a prescription for these OTC medications to be covered. Aspirin to help prevent serious heart and blood vessel problems (cardiovascular disease) in adults at least 45 years old who are at risk. Generic dosage forms up to 325 mg Aspirin examples (OTC): Aspirin tablet 81 mg and 325 mg Aspirin chewable tablet 75 mg and 81 mg Aspirin tablet, delayed-release 81 mg, 162 mg, 325 mg Iron Supplements to help prevent low red blood cell levels from not having enough iron (iron deficiency anemia). Iron supplements are for children 6 to 12 months old who are at risk. All children s oral liquid dosage forms Iron examples (OTC and Rx): Carbonyl iron susp 15 mg/1.25 ml Iron susp 15 mg/1.5 ml Ferrous sulfate elixir 220 mg/5 ml Ferrous sulfate syrup 300 mg/5 ml Ferrous sulfate soln 75 mg/ml Fluoride Supplements to help prevent cavities in children age 5 or younger whose water is low in fluoride. All oral dosage forms up to 0.5 mg Fluoride examples (Rx): Sodium fluoride tablet 0.5 mg Sodium fluoride chewable tablet 0.25 mg to 0.5 mg Sodium fluoride soln 0.125 mg/drop Sodium fluoride soln 0.25 mg/drop Sodium fluoride soln 0.25 mg/0.6 ml Sodium fluoride soln 0.5 mg/ml Folic Acid Supplements to help prevent birth defects in women age 55 or younger who are planning to become pregnant or are able to become pregnant. Generic, oral tablets Folic acid examples (OTC) Folic acid tablet 0.4 mg (400 mcg) Folic acid tablet 0.8 mg (800 mcg) Vitamin D Supplements to help prevent falls in adults age 65 years or older who are at risk. All oral dosage forms to meet dosing range of 600 IU to 800 IU Vitamin D examples (OTC) Ergocalciferol tablet 400 IU Cholecalciferol capsule/tablet/chewable 400 IU Cholecalciferol oral liquid 1200 IU/15 ml Cholecalciferol oral liquid 1000 IU/10 ml Cholecalciferol oral liquid 400 IU/ml Cholecalciferol drops 400 IU/0.03 ml (per drop) TOBACCO CESSATION PRODUCTS 2015 MVP Select Care, Inc. 2

Any copay associated with the covered products will be reduced to $0 beginning January 1, 2015 for all non-grandfathered ASO group plans. Recommended doses, ages and populations may vary (Rx) Vaccines listed below may be administered by a pharmacist (must be licensed to do so) and billed through the pharmacy benefits manager, CVS/caremark. Influenza Meningococcal Pneumococcal Varicella BOWEL PREPARATION MEDICINE Bowel Preparation Medicine for cleaning out the bowel before colonoscopy procedures for adults age 50-74. Colonoscopies screen for colon and rectal cancers. As of January 1, 2015, ASO group plans will move from covering contraceptives at a $0 copay or $0 copay with a Brand/Generic Differential on multi-source brand-name contraceptives to a $0 copay with a slightly revised Brand/Generic Differential on multi-source brand names. Multi-source brand-name contraceptives account for a very small number of scripts. Included in the CVS/caremark automated solution beginning January 1, 2015, these drugs will be covered with a copay at the point of sale. Plan participants must then submit a form to MVP to attest that they meet criteria for their prescription to be covered in full. Primary Prevention of Breast Cancer in women 35 years of age and older who are at increased risk. Generic, oral tablets (Rx) Raloxifene HCl tablet 60 mg Tamoxifen citrate tablet 10 mg and 20 mg To determine that the above drugs are being used for the primary prevention of breast cancer, your prescribing provider must submit a prior authorization request to MVP. Once approved, the medication will be available at no charge.
