The IRS determines what items and services are eligible for reimbursement through your HSA, and may change the list at any time. The following is a sample of common covered expenses. Your HRA will reimburse you for the eligible medical expenses you incur for: Yourself Your spouse Your dependents Your adult children How do I use my funds? Your HSA comes with the Ceridian Benefits Card, a fast and convenient way to access your funds at the point of purchase. Simply swipe your card at the register for eligible expenses wherever MasterCard is accepted. If you cannot or do not wish to use your card for an expense, you can easily request that funds be distributed to you via direct deposit or check using your online self-service portal. You will not need to file a claim and upload a receipt, however, you should always retain receipts in case of an IRS audit. Refer to the FAQs within this guide and the Ceridian Benefits Card addendum for more information. How do I manage my investments? Your online self-service portal provides an easy way for you to manage your cash and investment accounts, including adjusting the amount at which your funds transfer into your investment account, how your money is invested and more. Refer to the FAQs within this guide for more information. 2

How do I login to the website? First time login instructions*: 1. Go to www.ceridian-benefits.com. 2. Select the link First Time Login Click Here! 3. Start the Registration Process by entering ONE of the following: a Ceridian Benefits Services Account ID (found on most correspondence sent to you from Ceridian Benefits Services). OR your Social Security Number. 4. Click Continue at the bottom of the page. 5. On the next screen enter: Your first name and last name as they appear on Ceridian correspondence. Your street address. Your city, state abbreviation and the first five numbers of your ZIP code. If you have a non-u.s. address, check the appropriate box and fill in the resulting form. 6. Click the Continue button. You will be prompted to do the following: Enter and confirm your email address. Create a new user ID (minimum of 6 characters; at least one letter). Create and confirm a new password (case sensitive; minimum of 7 characters; at least one number and one letter). Select your security questions and provide your answers. 7. Click the Register button to complete the process and continue to your Ceridian Benefits Services account(s). For returning users. Ceridian now requires two security questions and answers to better secure your information. You may see your profile page when you login. If so, re-confirm your email address, select a second security question and provide your answer to proceed to your account. *If your company uses Ceridian s Health & Welfare benefits enrollment solution, you may access your HSA using a different self-service website. Reference your benefits enrollment materials for the correct login information. 3

From the HOME tab, it s easy to navigate the self-service website. Often, you will notice there is more than one way to accomplish the same task. For example, you may access your profile information by clicking the PROFILE tab at the top of any page on the site, or by using the links under Profile at the bottom of any page. This quick guide is designed to help you locate commonly used information and actions. 1. Tabbed navigation to main pages for HOME, ACCOUNTS, PROFILE, NOTIFICATIONS, FORMS, LINKS (these are available on each page of the site, as are the links at the bottom) 2. Your current available balance 3. A link to your account summary page 4. Actions: Links to request a distribution, contribute to your HSA, view your account activity or change your election amount 5. Click the My Account tab in your self-service portal, then click Investment Detail to access your investment account. Click Fund Performance next to investment options to review returns and access fund fact sheets and prospectuses. What investments should I choose? Please carefully review the fund prospectuses under the Investment Account link called Fund Performance. You can also consult your financial advisor to assist you with your selection. What price will I receive when I sell my mutual funds? The price you will receive when you sell your shares will be the next price set for the fund after your redemption order is received. What is my return on my account? You can find your personal rate of return using the link called Personal Rate of Return. Select a specific time period or fund using Select a Fund or Select a Period. The default is for the return to be calculated based on all transactions in your account during the previous 1825 days. To find the return on an investment in your account, click Fund Performance. How do I change my HSA investments? By default, your investment account funds are invested in a money market account. You can select how future contributions are invested by clicking Investment Elections, and can change how your current balance is invested by clicking Realign Investments or Transfer Investments. Realign Investments affects your entire account balance. A realignment initiates the sale of your existing investments and reinvestment of the proceeds according to your new investment instructions. Transfer Investments affects a single mutual fund and authorizes the liquidation of that particular fund and the reinvestment of the proceeds into an alternative investment. Trades initiated before 1:30 p.m. Central Time will settle the same day. Trades initiated after 1:30 p.m. Central Time are settled at the close of the next business day. Note: Funds may have a redemption fee or excessive trading policy that may affect the sale and purchase of funds in your account. Information will be provided as it applies to the fund and to your account. What is a ticker (symbol)? A ticker (also called a symbol) identifies each security or fund on a stock exchange. It is mostly used for looking up information on a stock or fund and placing trade orders. Find the ticker for available HSA investment funds by clicking Fund Performance on the fund fact sheet or prospectus, or by clicking Balance by Investment. 5

Click Pending/Activity History to access the recent transactions by type and status. There are three statuses: Active transactions will be processed unless you cancel before trade cutoff (1:30 p.m. CT). Processed transactions have been processed. Canceled transactions were submitted and canceled before trade cutoff (1:30 p.m. CT). To cancel a submitted transaction, click on Cancel under Transaction Status. Once a transaction is canceled, you must redo the transaction in order for it to be submitted again. I initiated a transfer/realignment. How do I see what was sold and purchased and at what price? You can view the trades involved in your realignment or transfer using the link called Transaction Detail. Select the period when your realignment or transfer occurred by entering a date in the date fields or clicking on the calendars and choosing a date. Sort your transactions by activity type by clicking on the column heading Activity Type. The realignment or transfer will include sales due to fund transfer and purchases due to fund transfer. The transaction types will be sale of shares and purchase of shares. The number of units and price per share are shown. You can also view a summary by fund using the link called Fund Activity Summary. Select the time period when your realignment or transfer occurred using the drop-down boxes. A summary of fund transfers occurring during your selected period will be listed by fund and shown in the column titled Fund Transfers. Can I cancel a transaction that has already been submitted and processed? A transaction that has been submitted and processed becomes a permanent item in your account history. To restore your account to pre-processing status, a new transaction must be submitted. How long is my transaction detail available on the HSA Investment Account website? You can view transaction detail for 180 days. What is the length of time a Fund Activity Summary is available? You can view a Fund Activity Summary for a specified period of time up to 1825 days or from your first transaction, whichever is less. 6

You can determine your total contributions for a specified period by using the link called Transaction Detail. Select a period by entering a date in the date fields or clicking on the calendars and choosing a date. Sort your transactions by activity type by clicking on the column heading Activity Type. Contributions will be indicated as HSA Contribution. A contribution will consist of a cash receipt (the total amount of your contribution) and a purchase of shares (for each fund in which your contribution was invested). Add your cash receipts to determine the total contributions. You can also view a summary by fund using the link called Fund Activity Summary. Select the period using the dropdown boxes. A summary of contributions occurring during your selected period will be listed by fund and show in the column titled Contributions. How can I see when my most recent contribution posted? You can see your most recent contribution posted by using the link called Transaction Detail. Select a period by entering a date in the date fields or clicking on the calendars and choosing a date. Sort your transactions by date by clicking on the column heading Transaction Date and look for the most recent date. Contributions will be listed under Activity Type as HAS Contribution. A contribution will consist of a cash receipt (the total amount of your contribution) and a purchase of shares (for each fund in which your contribution was invested). How can I get a printable version of my transaction detail? You can print your transaction detail using the link called Transaction Detail. Select the time period by entering a date in the date fields or clicking on the calendars and choosing a date and sort your transactions as you choose. When you have transactions pulled up per your specifications, click on Export as an Excel Spreadsheet or Export as PDF Document on the upper right side of the screen under Export Transaction Detail. Once the data is exported, you can print the information. How can I export and save my transaction detail? You can export and save your transaction detail using the link called Transaction Detail. Select your period by entering a date in the date fields or clicking on the calendars and choosing a date and sort your transactions as you choose. You can see interest posted to your account by using the link called Transaction Detail. Select your period by entering a date in the date fields or clicking on the calendars and choosing a date. Interest will show under Activity Type as Interest Received (Cash Receipt) and Reinvested Interest (Purchase of Shares). What is the difference between Transaction Detail and Fund Activity Summary? Transaction Detail shows transactions in your account by transaction type (purchase of shares, sale of shares, etc). Transaction detail can be sorted by source, fund, transaction type, or activity type by clicking on the appropriate column heading. Transaction detail history is available for 180 days. Fund Activity Summary shows the activity in your account by fund. Fund Activity Summary also shows beginning and ending balance for the time period you selected, total contributions, withdrawals and fund transfers within your account during the selected time. Your investment elections are also shown. Fund activity history is available for 1825 days. What funds were my contributions invested in? You can view a summary by fund of your contributions using the link called Fund Activity Summary. Select a time period using the drop-down boxes and you will see a summary of the contributions occurring during your selected period listed by fund in the column titled Contributions. You can view each specific contribution using the link called Transaction Detail. From the dropdown box titled Only Show Transactions Having, select Transaction as Purchase of Shares and select your period by entering a date in the date fields or clicking on the calendar and choosing a date. Purchases from HSA Contributions will be listed as HSA Contribution under Activity Type. How can I add or remove custom resources? You can add or remove custom resources using the Customize button on the home page of the HSA Investment Account website. To add a resource, enter a name and URL (e.g., www.morningstar.com) for your online resource and click Add Resource. The link will open a new window when clicked. To remove a resource, choose the link to remove from the drop-down menu under Remove Resource and click Remove. How can I project the future value of my account? You can project the future value of your account using the link called Account Projection. This will give you a hypothetical estimate of how your balance may accumulate over time. Enter an annual percentage rate, monthly contribution amount and years until dollars are needed and click Calculate for a projection. Of course, this is only an estimate, and your own HSA s performance could be different. 8

For noncard purchases, you can also choose to receive distributions via either direct deposit or a mailed check. Option 1: Use your Ceridian Benefits Card. For the fastest reimbursement, simply swipe your Ceridian Benefits Card at the point of purchase wherever MasterCard is accepted. Option 2: Submit a request online. Log into self-service to request a funds distribution electronically, and elect to receive your funds via direct deposit or a mailed check. You can view a summary by fund of your contributions using the link called Fund Activity Summary. Select a time period using the drop-down boxes and you will see a summary of the contributions occurring during your selected period listed by fund in the column titled Contributions. You can view each specific contribution using the link called Transaction Detail. From the dropdown box titled Only Show Transactions Having, select Transaction as Purchase of Shares and select your period by entering a date in the date fields or clicking on the calendar and choosing a date. Purchases from HSA Contributions will be listed as HSA Contribution under Activity Type. How do I use my Ceridian Benefits Card? The Ceridian Benefits Card is basically a debit card that is connected to your HSA funds, and requires no PIN to use. Simply swipe your card at the point of purchase for eligible expenses wherever MasterCard is accepted, and sign for your purchase as if using a credit card. Your expense will be processed directly using your account funds, without the need to submit a distribution request, but remember to keep your receipt and/or Explanation of Benefits in case of an IRS audit. If you have further questions, please review the Ceridian Benefits Card addendum or contact the service number on the back of your card. What do I do if my benefits card is declined? In rare instances, your card may be declined if you do not have sufficient funds in your cash account, or for various other reasons. If this occurs, you can pay for your eligible health care expense out of pocket and request an HSA funds distribution using your online self-service portal. Call the customer service number on the back of your card for assistance. Do I need to keep my receipts and/or Explanations of Benefits? Yes. Although you do not need to submit receipts or Explanations of Benefits to Ceridian to receive funds distributions from your HSA, you should keep them in case of an IRS audit of your taxes. How long should it take for me to receive funds distributions? Using your online self-service portal, requests for HSA distributions will be processed and issued to you within 5 business days. Using a paper form and electing to have funds sent to you via check rather than direct deposit may increase processing time. If you do not receive your funds in a timely manner, please contact your Ceridian representative. 10

Since your HSA can be used for eligible health care expenses now and retirement expenses in the future, it consists of two accounts: Cash account: Your HSA contributions are automatically placed into your cash account until your balance reaches the investment transfer amount. Readily available funds Like a checking account, your cash account does not accrue interest but keeps funds readily available for immediate use. Minimum balance Your cash account does not have a minimum balance to remain open. If you have funds in your investment account, however, your minimum required cash account balance is $750. If your cash account balance dips more than $100 below this minimum to $650 or less, and you have funds in your investment account, investments will be automatically sold to bring your cash account up to $750. Investment account: HSA funds are automatically moved into this account and invested to earn interest once your cash account reaches the investment transfer amount. Investment options By default, investment account funds are placed in a money market account; however, there are a variety of investment options available to you at any one time. You can change how your funds are invested using your online self-service portal. Delayed transfer of funds In order to use investment account funds, your investments must be sold.
