2 Your Home Care Service Custom Companion Assistance Solutions An Evolution In Senior Home Care According to Health and Human Services, 70% of Americans will need some type of home care in their lifetime. That is nearly three out of four of us. Many American seniors are under the impression that traditional health care programs will cover their care. Medicare, Medigap and most Medicare Advantage Plans are designed to cover hospital stays, doctor bills and some short term skilled nursing. Unfortunately, they may not cover the cost of everyday assistance in your own home. While your loved ones will have the best intentions to be there for you when a crisis arises, many are not prepared and quickly become overwhelmed by the amount of work involved in care giving. Purchasing traditional home health care insurance is limited to only the extremely healthy, creating challenges for seniors in need of another option. True Freedom Plans are a viable alternative to traditional home health care insurance. Our plans support seniors with their activities of daily living and companionship providing them independence in the comfort of their own home. All True Freedom Home Care Plans Feature: Guaranteed Acceptance No Health Questions No Claim Forms No Deductible No Co-pays No Age Limits Our plans can also provide urgent care benefits and overnight benefits in the comfort of your home. Services are provided by a network of home care providers while also giving you the option to choose a friend or neighbor to administer care. 4 Network Services Members will receive non-medical Home Care Services provided by an approved Network Agency or Registry for up to 5 hours a day/5 days a week. Services are available between the hours of 9am & 5pm Monday thru Friday. In order to receive Network services, your membership must be past the one-time 180-day Interim Benefits Period following your initial enrollment. To receive assistance, just call the toll free customer service number: 1-888-245-9001 Services Include Assistance With: Meal Planning/Preparation Dressing Grocery Shopping Laundry & Ironing Grooming Medication Reminders Monitor Diet & Food Expirations Changing Linens Accompany To Doctors Appointments Light Housekeeping Activity Schedule Oversee Home Deliveries Accompany To Place Of Worship Phone Calls Mail Organization Answering The Door Birthday Reminders Taking Out Garbage & much more THERE S NO PLACE LIKE HOME True Freedom Members never have to be concerned about locating or contacting a network home care agency to receive their services. All non-medical home care is scheduled by one toll-free call to the True Freedom home office. For the convenience of our members and their families, the Customer Care Coordinators at American Senior Services completely organize, arrange & schedule all of the home care services of your True Freedom Plan. ONE CALL DOES IT ALL True Freedom II Plans are Service Contracts, NOT Insurance.

5 Freedom Services Members will receive non-medical Home Care Companion Services provided by an approved friend or neighbor (it cannot be a family member or someone living in the same household.) FREEDOM Hours can be utilized during all days of the week, including evenings & weekends for up to 8 hours a day/5 days a week. In order to receive full access to the Freedom Services, your membership must be past the one-time 180-day Interim Benefits Period following initial enrollment. All FREEDOM Services are organized, arranged & scheduled by our Customer Care Coordinators at American Senior Services. To receive assistance, just call the toll free customer service number: 1-888-245-9001 Services Include Assistance With: Meal Planning/Preparation Dressing Grocery Shopping Laundry & Ironing Grooming Medication Reminders Monitor Diet & Food Expirations Changing Linens Accompany To Doctors Appointments Light Housekeeping Activity Schedule Oversee Home Deliveries Accompany To Place Of Worship Phone Calls Mail Organization Answering The Door Birthday Reminders Pet Care & much more URGENT CARE BENEFIT If a member were to need home care service during the 180 day Interim Benefits Period, the FREEDOM Home Care Companion Service would be made available for up to 10% of the Freedom days/hours of their selected plan. (Memberships must be active for 10 days.) OVERNIGHT SERVICE CARE Members can request FREEDOM Home Care Companion Service for Overnight Service Care when needed. Overnight service must be approved by American Senior Services. True Freedom II Plans are Service Contracts, NOT Insurance.

6 True Freedom II Membership Plans ELITE Up to 200 Days or 1000 Hours 500 Hours: Network Home Care Services 500 Hours: Freedom Home Care Companion Services ULTRA Up to 150 Days or 750 Hours 375 Hours: Network Home Care Services 375 Hours: Freedom Home Care Companion Services PREFERRED Up to 100 Days or 500 Hours 250 Hours: Network Home Care Services 250 Hours: Freedom Home Care Companion Services STANDARD Up to 60 Days or 300 Hours 150 Hours: Network Home Care Services 150 Hours: Freedom Home Care Companion Services BASIC Up to 30 Days or 150 Hours 75 Hours: Network Home Care Services 75 Hours: Freedom Home Care Companion Services All memberships include NETWORK Home Care Services for up to 5 Hours a Day/5 Days a Week or FREEDOM Home Care Companion Services for up to 8 Hours a Day/5 Days a Week. Either NETWORK or FREEDOM Services can be alternated daily during a week but both cannot be utilized in one 24-hour period. Members have the option of electing to use all of the Days/Hours of their plan as FREEDOM Home Care Services. True Freedom II Plans are Service Contracts, NOT Insurance.

7 Built-In Features True Freedom II Plans FRIEND OR NEIGHBOR Members can select a friend or neighbor (not living in the same household) to provide the FREEDOM Home Care Companion Service. Selected individuals must be approved by American Senior Services Inc. FREEDOM Hours can be utilized during all days of the week, including evenings & weekends for up to 8 hours a day/5 days a week. ADDITIONAL SAVINGS A 10% Discount is available on each membership for Husband & Wife / Domestic Partners when enrolling at the same time. 5% Discounts are available for individuals selecting an annual membership payment. A total of 15% Savings is available on each membership for Husband & Wife / Domestic Partners when enrolling at the same time and selecting an annual membership payment. FLEXIBLE SERVICES Members can choose to utilize either their NETWORK or FREEDOM services on Monday thru Friday. Only FREEDOM hours/days are available on weekends. RESTORATION Once a member has exhausted all of their NETWORK and FREEDOM services, benefits are fully restored in 180 days. There is no limit to the number of times your benefits can restore. FREEDOM DISCOUNT REWARD PROGRAM Get rewarded for being a member! Earn a discount of 10% upon the anniversary and renewal of your membership. Discount only applies if no home care services have been utilized. The Freedom Discount Reward Program grants each member a 10% savings, year over year that can be utilized for up to 4 consecutive years from start date. True Freedom II Plans are Service Contracts, NOT Insurance.

The last few years have been particularly difficult. Not only because of their declining health, but because of insurance coverage and payment issues.
