Retirement declaration form What is the form about? Please complete this form if you are retiring and claiming your LGPS pension but do not complete it more than three months before you retire. You need to send us your completed form so that: you can notify us of the decisions you make about your pension we can process and pay your pension under LGPS rules we can check your total pension benefits under HMRC rules Your pension will be delayed if you do not send us the form. If we have to pay your pension or lump sum without your form, we will assume that you have exceeded the lifetime allowance and your pension and lump sum will be taxed in full. What will happen next? We will pay your pension and any lump sum when we receive: your correctly completed form and photocopies of relevant documents Information from your employer. If you have a deferred (frozen) pension, we already have this authority from your employer for redundancy, efficiency or ill health retirement if you paid AVCs, payment of your AVC fund from the provider; they cannot do this before retirement We will write to tell you how much your pension and any lump sum will be. If there is a lump sum, we will pay it on your retirement date or as soon after as possible. We will make your first pension payment on the first possible pension pay date following your retirement. Section 3 Identification and partnership status 3.1 What is your partnership status? Your partner may qualify for a pension if you die. This depends on your circumstances and on the rules when you paid into the LGPS. Your cohabiting partner may qualify for a pension if you paid into the LGPS on or after 31 March 2008 and meet certain conditions. You can find out more on our website: www.hants.gov.uk/pensions. Do not send original documents. Please ensure that they support any change of name that you have had. We cannot pay your pension if we cannot verify your identity. For example, if you no longer use the surname on your birth certificate you will need to send another document too. We will only return documents if you ask. We will return original documents by Royal Mail but receipt cannot be guaranteed. Page 5 of 11

Section 4 Exchanging pension for lump sum 4.1 Exchanging pension for lump sum Do you wish to exchange part of your annual pension to receive a tax-free lump sum? Please tick one option: 1. Yes and wish to give up the maximum pension allowed 2. Yes I wish to give up this specific amount.. 3. No I do not want to give up any annual pension 4. I don t know Please send me a pension estimate before I make a decision. If you are leaving on Enhanced Voluntary Redundancy (EVR) or any other type of employer initiated retirement we can only accept a pension estimate request from your employer You may exchange some of your annual pension for a tax-free lump sum. Doing so will not affect the value of pensions paid to your dependants after your death. If you paid into the scheme before 1 April 2008, you will receive a tax-free lump sum, but you can still choose to exchange some annual pension to increase the lump sum. For each 1 of annual pension that you give up, you will get 12 as a tax-free lump sum. There is a limit to the total tax-free lump sum you can take, which is set by HMRC. HMRC rules mean that you must decide whether to exchange pension for lump sum before we calculate your pension. We cannot change the amounts afterwards, so please ensure you are happy with your decision before you send us your form. You can use your last pension statement to help you decide whether to exchange any pension for lump sum, and if so how much to exchange. Or you may decide not to exchange any pension for lump sum, or to exchange as much pension as possible for the maximum lump sum. If you request an estimate by ticking option 4, we will send your estimate as soon as we can. Your pension and any lump sum may be delayed while we wait for information from your employer. If there is a delay, please ask your employer if they have sent the information. We will only accept pension estimate requests from the employer for EVR or any other employer initiated retirement, otherwise your estimate will be based on early retirement and may be lower than expected. 4.2 In-house AVCs Do you have an in-house AVC fund? No - Please go to Section 5 Yes - please tick one of the options below: 1. I wish to use my AVC fund to maximise my overall tax free lump sum 2. The amount of my AVC fund I wish to use to increase my lump sum is 3. Please send me details of the options available for my AVC fund Important notice about AVCs If you have an AVC we will pay as soon as possible but cannot calculate or pay until We receive your written decision about your AVC. We receive the cheque from the AVC provider. They cannot pay the AVC to us without the last payment from your employer, or before your retirement date. This means that your pension may be delayed by at least four weeks after your retirement. If you pay AVCs in your final month, it may take longer for your AVC provider to send us the cheque. We will pay your pension as soon as we can once we receive it. What do the options above mean? You can take some or all of your AVC fund as a tax free lump sum (up to HMRC limits), but you can only take it all as a lump sum if you draw it at the same time as your main LGPS benefits and provided when added to your LGPS lump sum it does not exceed 25% of the overall value of your LGPS benefits (including your AVC fund) If you want details of the options available before you decide please tick (3). We will send you details once the AVC provider tells us the current value of your AVC fund. Page 7 of 11

Section 5 Taking your pension early I declare that I am voluntarily applying for early payment of my pension and any lump sum. I understand that early payment will permanently reduce my benefits. If you stopped paying into the LGPS before 1 April 2014, earliest you can choose to receive your pension is age 60. You can only claim your pension between 55 and 60 with your former employer s consent. If you stopped paying into the LGPS after 31 march 2014, you can choose to retire and receive your pension from age 55. If you choose to retire before your normal pension age, your pension and any automatic lump sum will reduced. If this applies to you, please tick the declaration above. The table below shows the maximum reductions that would apply to your pension and any automatic lump sum, depending on how many years before your normal pension age (NPA) you take your pension. If you have LGPS membership before April 2014, the reductions may be less than those in the table. If you have chosen to retire before your normal pension age, we will return this form to you if you have not ticked the declaration above. If your employer has approved ill-health retirement, voluntary redundancy, compulsory redundancy or efficiency retirement, your pension and lump sum will not be reduced so you do not need to tick this declaration. Lifetime allowance (LTA) HMRC sets a lifetime allowance (LTA). If the value of all your benefits exceeds the LTA, you must pay tax on the excess. Most people s benefits are within the LTA, but under HMRC rules we must assess them. The tax charge would be 55% of the excess lump sum and 25% of the excess pension, plus any PAYE deductions. Tax year LTA 2013/2014 1.5 million 2014/2015 1.25 million Do not include state pension or pension credit, spouse s, civil partner s or dependant s pension in this section. Do not declare any pensions that are first payable after your LGPS pension. LTA protection If you have an HMRC certificate, you should give your pension administrators a copy when any pension (including your LGPS pension) is due. They will take account of the protection when they assess your benefits against the LTA. Section 6.2 If you have any other benefits becoming payable on the same date as your LGPS benefits, please state the order in which you want them assessed against the LTA. The order you choose is important because it will decide which schemes will: be responsible for settling any tax liability if you exceed the LTA take the value of the others into account when assessing the LTA used The order you state will not affect the amount of any tax you pay. Your other pension providers can tell you the percentage of LTA taken up by your other pensions. Page 10 of 11

Section 7 Declaration Please read the following statements and ensure you fully understand them. Then sign the declaration below: I have read the notes and completed this form. The information I have given is correct and complete to the best of my knowledge. This includes the bank details in section 2, and section 5 if it applies to me. I will not use my retirement lump sum to contribute to one or more registered pension schemes, in breach of HM Revenue and Customs (HMRC) recycling rules (see notes below). I understand that I cannot change the decisions I have made on this form once Pensions Services has received and processed the form. If further tax becomes due because information I have given proves to be wrong, I understand that I will be liable to pay the tax charge and any resulting penalty imposed by HMRC. If you break recycling rules, you will have to pay a tax charge of 40% to 55% of the value of your retirement lump sum.
