PART I II III Document Summary Performance Information consists of performance information for plan investment options. This part shows you how the investment options have performed in the past. Investment Option Fees, Expenses and Restrictions shows you the fees and expenses you will pay if you allocate your plan account balance to a specific investment option. Annuity Information contains information about the annuity options under your retirement plan. Performance Information The table below focuses on the performance of investment options that do not have a fixed or stated rate of return. It shows how these options have performed over specified periods of time and allows you to compare them with an appropriate benchmark for the same time periods. Part II. Investment Fees, Expenses and Restrictions The table below shows fees and expense information for the investment options listed in Part I. It includes the Total Operating Expenses of the options in Part I. Total Operating Expenses are expenses that reduce the rate of return of the investment option. Rule #1 - Transfers to XYZ Fixed Account (Transfer) are prohibited. Fees and expenses are only one of many things to think about when deciding to invest. You may also want to think about whether an investment in a particular option, along with your other investments, will help you reach your financial goals. Part III. Annuity Information The table below focuses on the annuity options under the plan. Annuity contracts are insurance contracts that allow you to receive a guaranteed stream of payments usually beginning when you retire and lasting for your entire life. Annuities guarantees are subject to the issuing insurance company s financial strength and claims-paying ability. PLEASE NOTE THAT THESE GUARANTEED RETIREMENT INCOME OPTIONS ARE ONLY AVAILABLE IF PERMITTED UNDER YOUR PLAN S DISTRIBUTION PROVISIONS. Annuity Options As of (date) Name Lifetime Income Annuity Lifetime Income Annuity with a Guarantee Period Objective/Goals An income that is paid as long as the annuitant (typically the plan participant) is living. An income that continues as long as the annuitant is living but is guaranteed to be paid for a number of years. If the annuitant dies before all of the guaranteed payments have been made, payments are made to the named beneficiary) until the end of the guarantee period. No payments are made once the guarantee period has expired and the annuitant is no longer living. Pricing Factors/Restrictions/Fees Your annuity payments will depend in large part on the type of annuity option you choose. When you select an annuity option, it will apply to both fixed income payments and variable income payments, if available. Part IV. Plan-Related Information Plan-related Information is an explanation of general plan information. It includes a description of administrative service and recordkeeping fees and expenses that may be charged to your account. These fees are charged directly to the plan. This section also includes a list of the Participant-Elected Services Expenses. General Administrative Services, Recordkeeping and Other Services Expenses Table The table below focuses on the expenses associated with operating your plan. Fees and expenses for general plan administrative services, recordkeeping and other services (for example, plan trust and custodial services) may be charged to your plan.these fees and expenses may be charged to your individual plan account if they are not paid by the plan. How the expenses are charged to participant accounts will depend on the nature of the expense. For example, some fees may be charged as a fixed dollar amount or basis point charge per participant or as an amount spread across the plan account balances based on individual account balances. The dollar amount actually charged to your plan account during the previous quarter for such administrative or individual expenses will be reported to you on your quarterly statement. Investment Option Instructions: Your Plan lets you direct the allocation of your plan account among the investment options listed in Part I. You may make changes to your allocation via your Plan s website or by calling 800-543-2520. Limitations on Investment Options: Limits on making changes to your allocations may be imposed by the Plan or by the funding option provider. Any limits or restrictions made by a mutual fund manager are described in the prospectus for the mutual fund. They include restrictions intended to prevent market timing (i.e., rapid trading in and out of a mutual fund). In addition to the limits and restrictions described in the prospectus, the Employer may have other restrictions on making changes to your allocations. If your Employer has additional limits, they will be described in a separate document that will be provided to you by your Employer. Voting, Tender, and Similar Rights: Certain investment options that your plan account may hold may give you the opportunity to vote on proposals from the mutual funds or the underlying funds in a variable annuity. If and when such opportunities happen, you will receive a notice with the instructions on how to take advantage of what is being offered. Your private information is kept confidential and will not be revealed except as required by law. 4

Participant-Elected Services Expenses Table The table below focuses on expenses for optional services available through your Plan. Certain fees may be charged to your individual account for optional services you use. Participant Elected Services Expenses Service Fee Amount Frequency Description ABC Advisory Services 0.40% per year Monthly Covers ABC Advisory Services Loan Origination Fee $75.00 Per request This fee covers the processing of your loan. This fee is applied each time you request a loan from your retirement account. The fee is taken from the proceeds of the loan. Loan Maintenance $50.00 Annually This fee covers annual maintenance of your existing loan. If you have more than one loan, you will be assessed the fee for each outstanding loan.
