1 GI 7 HERNIA, ABDOMINAL Includes Inguinal, Femoral, Umbilical, and Ventral Hernias; Includes Abdominal Wall Hernia Repair. Background This case definition was developed by the Armed Forces Health Surveillance Branch (AFHSB) for the purpose of epidemiological surveillance of abdominal hernias and hernia repair. Clinical Description An abdominal hernia is an abnormal protrusion of an organ or tissue through a defect in the abdominal wall. Hernias occur most frequently in the inguinal, umbilical, and femoral regions of the body and are caused by a combination of increased abdominal pressure (e.g., from obesity, heavy lifting, constipation, coughing or sneezing) and an opening or weakness in the muscle or fascia. Symptoms include an obvious swelling beneath the skin of the abdomen or groin; the swelling may be tender, may disappear when lying flat, and may be accompanied by discomfort when lifting or bending forward. Nearly all have the potential risk of becoming strangulated (i.e., having their blood supply cut off) which is considered a medical emergency. 1 As such, the recommended treatment for most hernias is surgical repair. Estimates of the incidence rates of abdominal hernias in the U.S. population suggest a 5% lifetime risk with more than 600,000 surgical repairs occurring annually in the U.S. 2 Case Definition and Incidence Rules For surveillance purposes, a case of abdominal hernia is defined as: One hospitalization or outpatient medical encounter with a case defining diagnosis of abdominal hernia (see ICD9 and ICD10 code lists below) in any diagnostic position. For surveillance purposes, a case of abdominal wall hernia surgical repair is defined as: One incident case of abdominal hernia as defined above PLUS: - One hospitalization with a case defining procedure (PR) code for a surgical abdominal wall hernia repair (see ICD9 and ICD10 code lists below), coincident with, or subsequent to, the incident diagnosis, in any position; or - One outpatient medical encounter with a case defining Current Procedure Terminology (CPT) code for a surgical abdominal wall hernia repair (see CPT code list below), coincident with, or subsequent to, the incident diagnosis, in any position. 2 GI 7 Case Definition and Incidence Rules (continued) Incidence rules: For individuals who meet the case definition: For abdominal hernia and abdominal wall hernia surgical repair the incidence date is considered the date of the first hospitalization or outpatient medical encounter that includes the defining diagnosis. An individual may be considered an incident case only once per surveillance period. Exclusions: Individuals with a case defining medical encounter prior to the surveillance period. Codes The following ICD9 and ICD10 codes are included in the case definition: Condition ICD-10-CM Codes ICD-9-CM Codes Inguinal hernia K40 (inguinal hernia) 550 (inguinal hernia) K40.0 (bilateral inguinal hernia, with obstruction, without - K40.00 (bilateral inguinal hernia, with obstruction, without gangrene, not specified as - K40.01 (bilateral inguinal hernia, with obstruction, without gangrene, K40.1 (bilateral inguinal hernia, with - K40.10 (bilateral inguinal hernia, with gangrene, not specified as - 550.12 (inguinal hernia, with obstruction, without mention of gangrene, bilateral, not specified as - 550.13 (inguinal hernia, with obstruction, without mention of gangrene bilateral, - 550.02 (inguinal hernia, with gangrene, bilateral, not specified as - 550.03 (inguinal hernia, with gangrene, bilateral, - K40.11 (bilateral inguinal hernia, with gangrene, K40.2 (bilateral inguinal hernia, without obstruction or - K40.20 (bilateral inguinal hernia, without obstruction or gangrene, not specified as - K40.21 (bilateral inguinal hernia, without obstruction or gangrene, - 550.92 (inguinal hernia, without mention of obstruction, bilateral, not specified as - 550.93 (inguinal hernia, without mention of obstruction bilateral,

This case definition was developed in August of 2016 by staff from the Epidemiology and Analysis (E&A) division and the Medical Surveillance Monthly Report (MSMR) for use in a MSMR article on abdominal hernias. 1 The case definition was developed based on reviews of the ICD9 codes, the scientific literature, and previous AFHSC analyses. Case Definition and Incidence Rule Rationale None

12 GI 7 Code Set Determination and Rationale ICD9 code 96.27 (manual reduction of hernia) is not included in the code set as the procedure is considered a temporary measure to reduce symptoms and not a definitive surgical repair. The corresponding ICD10 codes are included as they are defined by the words repair, external approach suggesting definitive treatment. The following codes were not included in the 2016 MSMR article. 1 They are included in the code set above because, when these codes are recorded in the record of a patient who has also been diagnosed as an incident case of a hernia, the codes likely indicate an abdominal wall hernia repair. - 46.40 (revision of intestinal stoma, not otherwise specified) - 46.41 (revision of stoma of small intestine) - 46.42 (repair of pericolostomy hernia) - 46.43 (other revision of stoma large intestine) Reports AFHSB reports on hiatal hernia in the following reports: None Review Oct 2016 Aug 2016 Case definition reviewed and adopted by the AFHSB Surveillance Methods and Standards (SMS) working group. Case definition developed by AFHSB E&A and MSMR staff. Comments None
