Glaucoma Glaucoma is an eye disease characterized by increased intraocular pressure that causes pathological changes in the optic disk and defects in the field of vision. Most subcategories require a seventh character for the stage (unspecified, mild, moderate, severe, or indeterminate). Assign as many codes from category H40 as needed to identify the type of glaucoma, the affected eye, and the glaucoma stage. 3

Glaucoma Guidelines It is possible for a patient to have bilateral glaucoma, with each eye being of the same or different types, and the same or different glaucoma stages in each eye. Specific guidelines have been created to address the coding of these situations. The guidelines largely vary on the basis of whether the classification distinguishes laterality. When a patient is admitted with glaucoma and the stage of the glaucoma progresses during the admission, only the code for the highest stage documented is coded. 4

7 th character glaucoma stage Care should be taken not to confuse a glaucoma in which the stage is unspecified or not documented (seventh character "0") with a glaucoma stage documented as "indeterminate" (seventh character "4"). 0 Stage unspecified 1 Mild stage 2 Moderate stage 3 Severe stage 4 Indeterminate stage The assignment of the "indeterminate" stage should be based on clinical documentation and is reserved for glaucomas whose stage cannot clinically be determined. 5

4 th character Subcategories H40 H40.0 Glaucoma suspect H40.1 Open angle glaucoma H40.2 Primary angle-closure glaucoma 5 th character : 5 th character : 5 th character : 0 - preglaucoma, unspecified 1 - open angle with borderline findings, low risk 2 - open angle with borderline findings, high risk 0 - unspecified 0 - unspecified 1 - primary 1 - acute 2 - low tension 2 - chronic 3 - anatomical narrow angle 3 - pigmentary 3- intermittent 4 - steroid responder 4 - capsular glaucoma with pseudoexfoliation of lens 4 - residual 5 - ocular hypertension 5 - residual stage 6 - primary angle closure without glaucoma damage 6

For H40.6 also use additional code for adverse effect, if applicable, to identify the drug. These subcategories require a 7 th character. 7
