Check List Documents to be submitted with Marriage Form A Baptism Certificate of the Groom Baptism Certificate of the Bride NOTE: A Baptismal Certificate must be an original document issued within six (6) months of the date of this interview. If it is truly impossible (not just inconvenient) to obtain a Baptismal Certificate, Baptism (and Chrismation) may be proven by affidavits from two (2) persons who were present at the Baptism, such as parents, other family members and godparents using Marriage Form E).
