Week 10: The Insight The Overactive/Excessive Insight Type S: 31 year old Asian female seeks advice for recurring migraines. She states that she craves chocolate and cheese, and drinks a glass of wine in the evening to unwind. A patient intake questionnaire also reveals nearsightedness, less than 6 hours of sleep per night and vivid dreams. O: BMI = 23.5; Fasting glucose = 95; Cortisol high A: (1) Migraines; (2) Nearsightedness; (3) Insomnia; (4) Hyperglycemia; (5) High cortisol P: Examine whether she has food intolerances by doing an elimination diet, especially removing the chocolate, cheese, and alcohol. Work on stress management practices utilizing a simple 5-minute meditation daily. Track dreams and see if there are symbolic messages that need further examination. Week 10: The Insight The Underactive/Deficient Insight Type S: 48 year old Caucasian male complains of chronic fatigue as well as difficulty with concentration and memory. He drinks 5-6 caffeinated beverages per day to keep him going and eats very little fruits and vegetables. Explains that he has difficulty finding the energy for exercise. States he is worried about Alzheimer s disease, which runs in his family. O: BMI = 23; Cortisol low; Cognitive test indicates low functioning. A: (1) Fatigue; (2) Cognitive impairment P: Focus on dietary fruits and vegetables, especially blue-purple foods. Assess whether omega-3 supplementation is warranted. Begin with a 4-minute walk on a daily basis with the goal of working up to 30 min/day over a month. Do further genetic tests to see his personal risk of AD. Get fasting blood sugar for next visit. Their Story A few years ago I started sleeping in a lot, even until midday. Since then I have struggled to get back to early rising and sustain energy all day. Another side effect of over sleeping was depression, around this same time I also started using cannabis on an almost daily basis and I ve had a yoyo relationship with it ever since where I either smoke every day or not at all. I also find I need to drink coffee daily as a stimulant. I would like to have the self-control to moderate it and have a little bit now and then as I enjoy it but be strong enough to not have it everyday.
