Cancellation Policy When a cycle of treatment is cancelled for any reason the patient will be liable for any costs incurred before cancellation. For example, if a standard IVF cycle is cancelled after treatment planning plus one monitoring scan, the costs incurred before cancellation would be 285 for the treatment planning appointment plus 35 for a scan. A credit note will be issued for the difference between the original treatment cost, minus the costs incurred. Any credit balance can be used against future treatment costs or a cheque refund can be requested. No refund is given for unused medications. MFS is not legally able to take back medication from patients once the drugs have left the clinic. Left-over medications should be stored appropriately by the patient and may be used for future treatment cycles. It is the patient s responsibility to advise the clinical staff if they have medications left over from previous cycles that may be used for a future cycle. If, before embryo transfer, the clinical team recommends that all embryos are frozen for use in a future cycle, there is no charge for the first attempt to transfer frozen embryos, other than for any drugs required. Subsequent frozen embryo transfers, even if the first has resulted in a cancellation, are fully chargeable. Egg share recipients, surrogacy, egg freezing or embryo freezing. If a cycle of treatment is cancelled for egg share recipients, or patients undergoing surrogacy or egg/embryo freezing, the circumstances will be assessed on an individual basis and any credit given.

Costs: Important Information This information should be read with the Midland Fertility Services Terms and Conditions. MFS operates an open and transparent fee structure. Published costs include all of the usual stages of treatment, from the planning appointment through to pregnancy scan or treatment follow-up. These costs do not include any medication required throughout the treatment as each patient requires a different prescription. MFS reserves the right to alter the fees at any time. Usually fees are reviewed annually with effect from April. Where possible, MFS will give one month's notice of any changes. Treatment and Drugs Patients are responsible for paying fees in full at least 4 days before the start of any planned cycle to ensure enough time for the required medication to be ordered and delivered. In addition: drugs ordered by MFS from the pharmaceutical company totalling less than 50 per patient incur a processing charge of 5 storage of patient drugs at MFS incurs a charge of 50 the clinical team prescribe a standard dose of medication for treatment, to try to minimise waste. It may be necessary to 'top-up' this dose with additional medication and the patient will incur these additional costs the cost of any 'top-up' drugs issued from MFS stock are to be paid on the day they are prescribed no refund may be given to unused drugs which have been removed from MFS invoices for male procedures should be settled in full 4 days before the treatment date any unpaid costs will result in treatment being delayed. MFS reserves the right to cancel appointments or refuse treatment at short notice if fees are not paid before that appointment Additional Costs HFEA levies are beyond the control of MFS and may be subject to change. An additional HFEA levy is applicable to all IVF, ICSI and frozen embryo transfer cycles and on IUI cycles using donor sperm. The current HFEA levies are listed in the List of Charges any additional procedures and/or investigations required at any point during treatment will be discussed with the patient before being carried out and further charges may apply. Please contact a member of the finance team before proceeding to confirm or clarify any such charges electronic witnessing is a rigorous laboratory procedure to effectively identify and track patients gametes and embryos. Major treatment cycles (eg IVF and ICSI cycles) incur an electronic witnessing cost of 40 and minor treatment cycles (eg IUI and frozen embryo transfers) incur a cost of 20. The electronic witnessing charge is applied to all new treatment cycles and is non-refundable the first year's storage of frozen gametes and embryos is included in the original treatment cost (as indicated on the List of Charges ). Any further years of storage will be charged at an annual rate, as detailed in the List of Charges Payment Options MFS accepts payment by cash, cheque, credit and debit cards, (excluding American Express) credit card transactions incur a supplementary charge of 2% to cover charges levied by the processing bank.
