Welcome Welcome to the Redmond Wellness Center @ Roper Corporation. The Center provides a variety of primary health care and urgent care services for employees enrolled in the Roper Corporation Employee Health Plan and their eligible dependents (those covered under the Roper Corporation Employee Health Plan Benefit). The healthcare professionals and the services they provide through the Wellness Center are the central focus for Roper Corporation s HealthAhead initiatives. HealthAhead is Roper Corporation s commitment to actively promote and support healthy lifestyles for employees and their families through wellness, nutrition, and physical activity. This patient Guidelines booklet provides a variety of useful information regarding the services provided through the Redmond Wellness Center @ Roper Corporation. The Center is staffed and operated by Redmond Regional Medical Center in Rome, Georgia, a healthcare services provider specializing in high quality hospital care and onsite healthcare services to companies like Roper Corporation. Patient Rights and Responsibilities Statement of Rights and Responsibilities for Patients Utilizing the Services of the Redmond Wellness Center @ Roper Corporation. PATIENT BILL OF RIGHTS A patient has the right to be treated with respect, consideration and personal dignity and with protection of his/her need for privacy. A patient has the right to impartial treatment regardless of race, national origin, religion, or handicap. A patient has the right to a prompt and reasonable response to questions and requests. A patient has the right to know who is providing medical services and who is responsible for his/her care. A patient has the right to be given information concerning diagnosis, planned course of treatment, alternatives, risks and prognosis. A patient has the right to refuse any treatment and be informed of the medical consequences of that refusal. A patient has the right to express grievances regarding any violation of his/her rights without fear of refusal of medical services. A patient has the right to confidentiality of his/her medical records. PATIENT RESPONSIBILITIES A patient is responsible for providing the clinic, accurate and complete information about present complaints, past illnesses, hospitalizations, medications and other matters related to his/her health. A patient is responsible for reporting to the provider any unexpected changes in his/her condition. A patient is responsible for listening to the provider's instructions and for asking questions if he/she does not understand the information given. A patient is responsible for following the treatment plan recommended by the provider. A patient is responsible for his/her actions if he/she refuses treatment or does not follow the provider's instructions. A patient is responsible for keeping appointments and when he/she is unable to do so, for notifying the clinic 30 minutes prior to the appointment time. A patient is responsible for treating the provider and health care staff with respect, consideration and personal dignity. Shouting, inappropriate language and behavior will not be tolerated. Patients that do not comply with these measures may be prevented from receiving personal care in the wellness center. 5 The Redmond Wellness Center @ Roper Corporation

Eligibility Employees and Employee Dependents covered under the company s employee health benefit plan are eligible for Wellness Center services. Employees who wish to use the services of the Wellness Center for anything other than work related injury or illness must be covered under the company s employee health benefit plan. Hours of Operation Physician and Provider Hours 6:00am 7:00pm Monday Wednesday 6:00am 7:00am Nurse only 6:00am 5:00pm Thursday - Friday 6:00am 7:00am Nurse only 8:00am 4:00pm Saturday Physical Therapy Hours 10:00am 7:00pm 12:00pm 4:00pm Monday Thursday Friday Dietician Hours 2:00pm 5:30pm Tuesday Some Saturdays ** Service interruptions and other special information regarding hours of operation will be communicated through employee communications at Roper Corporation and on the Wellness Center page at www.ropercorporationlafayette.com Appointments All visits to the Redmond Wellness Center are by appointment only unless the visit is work related or urgent care is needed (for example: brought in by first responders, asthma attack, etc.) To schedule an appointment, call direct to (706) 638-3559 or dial extension 206 on the Roper Corporation factory phone network. To avoid interrupting the schedules of other Wellness Center patients and the staff, it is important that you arrive 5 minutes before your scheduled appointment. Employees Employees are required to clock out during working hours for all visits to the Wellness Center unless the visit is work related (work-related accident, random drug screen, forklift physical, mandatory hearing test, etc.) Dependents Eligible Dependents may schedule appointments by calling (706) 638-3559. Dependents requesting appointments may be required to provide some personal information, including their Blue Cross Blue Shield of Georgia ID number, over the phone for purposes of verifying identification. New patients seeking primary care services will be required to undergo an initial physical exam to include a medical history and baseline measurements of the patient's height, weight, blood pressure, and any labs the provider feels appropriate. Patients will be required to show a photo ID and their Blue Cross Blue Shield card when signing in for their appointment. Dependents cannot be treated without first providing proof of identification and their Blue Cross Blue Shield of Georgia card. 6 The Redmond Wellness Center @ Roper Corporation

Appointment Times - The staff of the Wellness Center will make every effort to see patients with scheduled appointments as close to their scheduled times as possible. However, treatment of medical emergencies will always take precedent over treatment of non- emergencies. For this reason, patients may not always be seen at their exact appointment times. Tardiness - Patients more than 15 minutes late may have to be rescheduled at the next available appointment unless there is an opening available at that time. Cancellations Employee patients who will not be able to make their scheduled appointment are reminded to call the Wellness Center at (706) 638-3559 or extension 206 on the Roper Corporation factory phone network, as soon as possible to notify them of the need to cancel. If you cannot reach the Wellness Center staff, please leave a message on the Wellness Center's voice mail notifying them of your cancellation. Dependent patients who will not be able to make their scheduled appointment are reminded to call the Wellness Center at (706) 638-3559 as soon as possible, preferably more than 24 hours before their appointment if possible, to notify them of the need to cancel. If you cannot reach the Wellness Center staff, please leave a message on the Wellness Center's voice mail notifying them of your cancellation. The Wellness Center may restrict the services offered to patients who persistently cancel appointments without notice or are "no-shows" for appointments consistently. Security To help ensure the safety of Roper Corporation employees and visitors to the facility, all visitors including anyone visiting the Wellness Center-must first register with Security at the Main Entry Gate. Visitors will be required to present the Security Office with their driver s license or other picture ID and will be provided an identification badge and a parking permit. Children will be registered as visitors under the care of a supervising adult. Children should not be left unattended at any time during their visit. The Security Officer will direct visitors to the designated parking area for Wellness Center visitors. While onsite, Wellness Center visitors are restricted to the Wellness Center facility only. Unauthorized entry into other areas of the facility may result in expulsion from the plant and restricted further use of the Wellness Center services. 7 The Redmond Wellness Center @ Roper Corporation

Services Provided to Employees and Dependents The Redmond Wellness Center @ Roper Corporation provides services to employees and their eligible dependents enrolled in the company health benefit plan. Services are paid for by the company at no charge to the employee. These services include, but are not limited to: Occupational Medicine (employees only) Health education and supportive counseling with emphasis on prevention of chronic illness Primary and Urgent/Acute medical care (see definition below) Emergency stabilizations/transport Specialist referrals Diagnostic referrals Hearing and Vision referrals Sore Throat/Ears/Headaches Glucose monitoring Cough/Sinus/Respiratory Wellness Exams for both men and women Aches/Pains/Sprains Rashes Weight monitoring Cholesterol & Hypertension Screenings Minor dermatological conditions Physicals - Routine/Sports/Pre-employment Flu, mono, strep, and pregnancy testing Lab work - The Wellness Center will draw and send labs ordered by outside personal MD if: The labs meet the standard lab criteria. If the tests are covered by Roper health benefit plan The results of lab work ordered by an outside physician will be sent directly to the ordering physician. Copies of lab work results ordered by an outside physician will not be available from the Wellness Center. Physical therapy - A physical therapist from PerFormance Physical Therapy & Sports Medicine, provides physical therapy at specific times for patients referred by any physician or other licensed Practitioner, such as Physician's Assistant, Nurse Practitioner or Chiropractor. 8 Services Provided to Employees and Dependents

Urgent / Acute Care Work-in patients (those who don t have a scheduled appointment) may seek urgent or acute care and treatment at the Wellness Center during normal business hours. Depending upon the patient load at the time of the visit, however, there is no guarantee that non-emergency work-in cases will be seen. The Wellness Center may be able to see patients on a work in basis depending on the clinic schedule and the severity of the problem. We are not a walk in clinic (being seen without calling the Center in advance). Urgent/Acute care is defined as the short term treatment of sudden onset illness or injury and is provided to employees and their dependents that are enrolled in the employee health benefit plan. Proof of identity and a copy of your employee health plan insurance card will be required during registration for treatment. Examples of urgent/acute care needs include, but are not limited to: Sprains Sore throat Vomiting Diarrhea High Fevers Abdominal pain Ear Infections Skin infections Urinary Tract Infection Cough, Sinus infections Primary Care Primary care service is defined as long-term care by the healthcare professional serving as the patient s principal healthcare provider. Primary care services may include regular treatments for chronic conditions and services that are often referred to as wellness care. These available only to employees and their dependents enrolled in the employee health benefit plan. Proof of identity and a copy of your employee health plan insurance card will be required during registration for treatment. Additional Services Procedures involving work related accidents In an accident occurs at work that results in an injury, the employee must notify their Business Leader (BL) and report to the Wellness Center for assessment and treatment. Treatment for work related injuries is provided by the Wellness Center regardless of whether or not the injured employee participates in the company health benefit plan. The employee must bring a completed incident reports with them to the Wellness Center including the details of how and when the injury occurred, unless emergent treatment is needed (according to Roper policy). The Occupational Health Nurse will notify the BL for completion of an accident investigation form. The employee will be assessed, treated and returned to normal duty OR if modified duty is required, the Wellness Center will notify the BL to make accommodations by finding suitable work. Medical will fill out the restrictions on the Transitional Work Assignment and instruct the 9 Redmond Wellness Center @ Roper Corporation

employee to report BL. The Wellness Center, HR, and the BL will agree on modified work and the Transitional Work Assignment will be completed including review dates and an outline to transition to full duty. If the injury requires diagnostic tests or a referral out to a Worker's Compensation Certified medical provider, Collins and Company, Roper Corporation's third-party administrator for worker's compensation, will be notified and the Wellness Center/Collins will make the initial appointment and coordinate with the employee and panel provider. If the injury requires appointments for medical treatment or surgery including days away from work, a return to work release form will be filled out and presented to the Wellness Center when the employee returns to work. The Wellness Center, Roper Corporation's Worker's Compensation administrator, HR, and the BL will follow the employee's progress until a release to full duty occurs. Refer to the Roper Corporation Policy and Procedures Manual for more information on policies and procedures regarding work-related injuries. Physical therapy for work related injuries A physical therapist provides regularly scheduled physical therapy sessions Monday - Friday in the Wellness Center for patients with qualifying injuries that have been referred by a physician or licensed professional. Medical Emergencies In the event of a medical emergency, seek immediate medical treatment by calling 911 or visiting the nearest hospital emergency room or urgent care center. In the event of a medical emergency while on site at Roper Corporation, seek immediate medical treatment by visiting the Wellness Center or by calling 311from any company phone. The Redmond Wellness Center @ Roper Corporation is not equipped to serve as a medical emergency treatment facility. Employees who experience serious medical emergencies while at work will be stabiliz ed by the Wellness Center staff and transported to suitable medical facilities for treatment. 10 Redmond Wellness Center @ Roper Corporation

Examples of medical emergencies emergencies requiring treatment at a hospital or emergency room include but are not limited to: Chest pain Difficulty breathing or shortness of breath Broken bones Uncontrolled bleeding Seizure or loss of consciousness Severe allergic reactions Dispensary Services In some instances, the healthcare professionals in the Wellness Center may be able to provide patients with commonly prescribed medications at no cost to the patient through the center s prescription medication dispensary. If the medications are available from the dispensary, the patient will be given a one-month supply of their medications from the dispensary at no cost to the patient. A prescription for any refills will be written for filling at the patient s expense at a commercial pharmacy. Reciprocal Treatment for General Electric Employees Employees of General Electric who subscribe to the GE Employee Health Benefit plan are eligible for treatment at the Redmond Wellness Center @ Roper Corporation through a reciprocal agreement between GE and Roper Corporation. GE employees can contact Roper Corporation Human Resources office for full details. Other Resources and Services Wellness Services One of the most effective ways to avoid serious illness is to have regular check-ups with your health care provider. To help encourage Roper Corporation employees and their dependents to have regular wellness exams, our employee health benefits provide coverage for a number of preventative services for children and adults, including screenings and other services to detect medical conditions in advance. Covered Services include, but are not limited to: Treatment for preventative services including periodic health examinations for adults and dependent children. Flu vaccines (age restrictions apply) Mammograms Pap smears Prostate exams and testing 11 Redmond Wellness Center @ Roper Corporation

Pediatric Services age 2 and up. Sports physicals Acute care of viral and bacterial illnesses Evaluation and treatment of various skin conditions Laboratory testing NOTE ON LAB WORK : The results of lab work ordered by an outside physician will be sent directly to the ordering physician. Copies of lab work results ordered by an outside physician or medical provider will not be available from the Wellness Center. Blue Cross Blue Shield of Georgia 24/7 NurseLine Enrollees in the Roper Corporation employee health plan benefit qualify for use of the Blue Cross Blue Shield of Georgia 24/7 NurseLine. By calling 1-888-724-2583, you can speak directly to a nurse about your health questions or concern, 24 hours a day, 7 days a week, get information about BCBS of Georgia member services, or you can listen to recorded information on a variety of health topics. After hours EMPLOYEES WORK RELATED In the event of an injury when the Wellness Center is not open (weekends, holidays, plant closures), Security should be notified by dialing 311from any plant telephone. Security will then page-out for a 1st Responder. If a 1st Responder is not available, Security will call 911for an ambulance. If away from work - Call Security at 706.639.2207. The Security Officer will notify the appropriate staff member of your situation. PERSONAL In the event of a medical emergency you should seek immediate medical treatment by calling 911. If you need non-emergency medical advice during non-office hours you can call the Blue Cross Blue Shield of Georgia 24/7 Nurse line at 1-888-724-2583. There you can speak directly to a nurse about your health questions or concern, 24 hours a day, 7 days a week. DEPENDENTS In the event of a medical emergency you should seek immediate medical treatment by calling 911. If you need non-emergency medical advice during non-office hours you can call the Blue Cross Blue Shield of Georgia 24/7 NurseLine at 1-888-724-2583. Privacy Statement The privacy of your medical records is protected under specific Federal laws. The third party administrator of the Redmond Wellness Center is Redmond Regional Medical Center does collect and maintain medical information on the patients treated. A copy of the Wellness Center Privacy Policy is available upon request at the Receptionist desk. Roper Corporation Right to Know Information No one from Roper Corporation will have access to the personal medical records in the Wellness Center. However, there are exceptions that relate to administering pre-hire physicals, Workers Compensation, Family Medical Leave, short term disability, or long-term disability. Worker s Compensation All medical information on an individual who alleges a work related injury can be reviewed by medical staff. The law states that any previous medical condition even personal could be pertinent to a work related case. Since the Georgia law allows all medical providers in the Wellness Center may pull the file and review all medical information to determine if any personal medical information should be considered in the case. All relevant medical information will be determined by the on-site medical staff. Family Medical Leave Act Roper Corporation s Human Resources office administers the time and attendance policy. The Wellness Center qualifies requests for Family Medical Leave Act (FMLA). All requests for FMLA are brought to the Wellness Center. 13 Redmond Wellness Center @ Roper Corporation

Frequently Asked Questions Why can't people that don't have Roper Corporation Insurance go to the Wellness Center? Some of us can't afford it. All employees have an opportunity to choose medical coverage here at work once a year at open enrollment.we have not increased our insurance cost in several years and we have great coverage at a very low cost in comparison to other employers in the area. Employees can purchase insurance on themselves for $10/week if they pass the HRA and only $20 if they don't pass. However, employees are not required to take insurance coverage, they have a choice. Our Wellness Center is not open to the public. It is an enhanced benefit for those employees carrying insurance coverage on yourself (and family) here at work. Insurance premiums, FSA Accounts and 401K Accounts are all tax free income to employees under Federal Law. If we opened the Wellness Center to employees who don't carry our insurance, all employees and guests would have to file insurance through the employee's private insurance coverage (BCBS or other coverage). Employees would have to pay their co-pay and deductibles the same as any other Physicians office. The Wellness Center would be a stand-alone business, including paying taxes on income. It would no longer be a non-profit benefit for employees electing company insurance. Our Wellness Center was never intended to be open to the public. ******** Can you visit the Wellness Center during working hours? If so, will this count against your attendance? Employees are required to clock out during working hours for all visits to the Wellness Center unless the visit is work related (work-related accident, random drug screen, forklift physical, mandatory hearing test, etc.) The attendance policy allows employees to take a wellness day for visits to the Wellness Center. These wellness days for appointments in the Wellness Center will not affect the 90 days of attendance required to get a positive point, nor will it stop you from attaining Perfect Attendance. You will not need prior day approval for a Wellness Day used in the Wellness Center, but you will need to make your TL and AL aware of your appointment time in advance. ******** Will the Wellness Center give you prescriptions for weight loss medications? No. Studies have shown that such medications do not result in long-term weight loss. We want to assist you in making the necessary life-style changes that will result in long term weight loss and a healthier overall way of living. And what qualifies as a Wellness visit? Wellness visits are yearly well-person checkups for adults, or periodic well-baby check-ups for infants. Wellness check-ups performed by the staff of the Redmond Wellness Center @ Roper Corporation are provided at no cost to employees and their dependents covered under the Company s employee health benefit plan. Wellness checkups through providers other than the Wellness Center are paid 100% after the $50 copay for primary care and $65 for specialists due to changes in the new federal healthcare bill. However, if you go to the doctor for a checkup and they find a problem, like high blood pressure, kidney infection or any other ailment they treat you for and bill you with that medical code, the insurance administrator will probably not recognize it as a well visit. We have a no fault policy, if you have an acute illness, like sinusitis, or sore throat and you are advised to rest and take medicine causing you to miss work, it would cause an occurrence under our attendance policy. ******** What is a Physical Therapist? Physical Therapists apply research and proven techniques to help people recover function and get back in motion. All physical therapists are required to have a graduate degree and state license. They are trusted health care professionals with extensive clinical experience who examine, diagnose and then prevent or treat conditions that limit the body's ability to move and function. ******** Do I need a referral from the Wellness Center Physician to go to physical Therapy at Roper?
