OCI Misc. Change of Address/Occupation Minor US Passport Holder The documents in the Checklist are mandatory for your application to be processed. This list of documents is not exhaustive and is subject to change based on the requirements of the Indian Embassy/Consulate. The Indian Embassy/Consulate has the right to request for additional documentation. 1 copy of signed Checklist with the boxes ticked must be submitted with your documents. Sr. No 1 2 Documents OCI MISCELLANEOUS SERVICE FM Complete the OCI Miscellaneous Service Application Form on the Government Site (https://passport.gov.in/oci/) In order to complete the form, you will need at least ONE of the following:- o U-Visa Number o OCI Registration Number o OCI File Number All the Information below WILL HAVE to be entered, o Current Passport Number o Date of Birth o Place of Birth o Mother s Name The INFMATION below MUST MATCH what the applicant ENTERED WHEN HE HAD APPLIED F the IGINAL OCI CARD, failing which one cannot PROCEED. Select the Desired Miscellaneous Service needed. MIN SIGNATURE/THUMB For child of 5 years and above, signature/name of the minor child should be in the signature box under their photo on the first page of the application. Uploaded Signature/Thumb impression must be legible on plain white background. For child less than 5 years of age thumb impression of the minor child should be in the signature box under their photo using only a BLACK or BLUE inkpad. (Use Left Thumb for Boys & Right Thumb for girls). PARENT CANNOT SIGN ON BEHALF OF THE CHILD UNDER ANY CIRCUMSTANCES ON THE PART A. Applications should contain BOTH PARENTS signature on the PART B ONLY DULY NOTARIZED. MINS DO NOT SIGN PART B OF THE APPLICATION. Wherever parent s sign, the signature must match with the passport Required Upload Scanned

Wherever documents need to be self-attested, both parents must sign. Signature must be legible and on the plain white background. NOTE - If the signature is OUTSIDE of the boxes provided, the application will be REJECTED. If the thumbprint/signature isn t clear after they upload online, they need to upload again or minor has to sign in the application form. PHOTOS 2 most recent passport size color photos (no older than 6 months) of the applicant to be provided. Uploaded photo must be a most recent passport size color photo. Paste 1 photo on page 1 at the relevant place on the online form and send 1 photo with the Application. Photos to be 2 inches x 2 inches in size, in color, depicting front pose strictly against white background only on photographic paper. ADDITIONAL PARTICULARS FM Fill the Additional Particular Form and enclose the print out. All pages of Additional particulars form should have signatures of the applicant. All questions must be answered with details no question should be left blank. Where applicable, please answer as Yes or No. Do not answer as N/A. Incorrect/Incomplete applications and applications answered in N/A will not be processed. Must have Applicants name as per Current passport. PARENTAL AUTHIZATION FM DULY SIGNED BY BOTH THE PARENTS Must be DULY NOTARIZED. CURRENT US PASSPT - of the following pages of valid US Passport o Information page that contains the picture and basic details of the US passport. o Endorsement/amendment pages. o Passport should be valid for 8 months from the date of application. Only for Chicago and Houston Jurisdiction - Please send only colored Only for New York Jurisdiction - Please send colored notarized VISA STATUS F PARENTS ONLY IF PARENT(s) ARE FEIGN CITIZEN(s) (NON US) Provide copy of the following:- o Permanent Resident Card Both Sides. o Employment Authorization Card which states Serves as 1-512 or of Pages showing H1/H4, L1/L4, F1/F2, J1/J2 etc. NOTE: TRANSFER NOTICES RECEIPT NOTES ARE NOT ACCEPTED. VISA MUST BE VALID F AT LEAST 3 MONTHS. PARENTS US/FEIGN (NON US) PASSPT (Must be valid for at least 3 months) by parents. Submit of the following pages of valid original US/Foreign (Non US) Passport of parents:- o Color of the information page that contains the picture and basic details of the US/Foreign (Non US) Passport. o Color of the endorsement/amendment pages. o On US passport books that end with page 24, copy pages 22, 23 and 24. o In US passport books that end with page 28, copy pages 25, 26 and 27. o If parents have OCI cards, it is mandatory to provide copies of OCI cards as well. o If parents OCI Cards are copied, BOTH information pages on the OCI card should be copied. BIRTH CERTIFICATE For applicants born outside of India, a photocopy of the birth certificate any government issued document that shows the following must be provided:- o Parents Name o Date of Birth o Nationality If the applicant is applying through parents, one parent was born outside India then Birth Certificate and Passport copy of the parent born outside India. If the BIRTH CERTIFICATE/Documents are in any other language besides English, the document needs to be translated into English and it has to be notarized. PROOF OF ADDRESS For minors, a proof of address from BOTH parents legal guardian to match with the present address as per application. Only one of the following must be submitted:- Only State Issued Driving License Utility Bill - Water Utility Bill Electricity Utility Bill Gas Valid lease agreement signed by landlord and tenant (applicable ONLY F Atlanta, Houston and New York). For College/University Students living on campus in University housing /dormitories, a letter from your institution on the institution letter head explaining the exact address that you presently reside in will suffice (Not Applicable for Applications from Washington DC). Landing Papers Canadian Resident (For New York, Chicago, San Francisco jurisdiction only) o Canadian Citizens (through Naturalization) must provide a copy of their landing paper along with their documents.

13 14 15 NOTE: DO NOT CUT DRIVER S LICENCE COPY DOWN TO A SMALLER SIZE. PARENTS MARRIAGE CERTIFICATE Marriage certificate from marriage registering authority only will be recognized. Certificates issued by community institutions like Church/ Mosque/ Temple are not acceptable. PARENTS PROOF OF RENUNCIATION If one parent or both parents are naturalized citizens, one of the following MUST be produced as proof of renunciation for parent(s): - o Certificate of renunciation issued by the Embassy/Consulate of India. o Canceled Indian Passport with stamp stating the passport is canceled due to obtaining US/Foreign Citizenship (Non US). Not required for Atlanta Jurisdiction. Only for Washington DC Jurisdiction - Please send. If parents NEED to obtain RENUNCIATION CERTIFICATE, then they need to apply for renunciation certificate prior to submitting application for minor. PROOF OF INDIAN IGIN copy of first three and last two pages of the last Indian passport held. copy of any document that provides sufficient evidence that the applicant has held Indian Citizenship like:- o Any other proof substantiating the request. Usually applicants are able to submit a certificate of residence or place of birth of self/parents/grandparents from First Class Magistrate/District Magistrate (DM) of the concerned place. o copy of educational certificate issued by an institution of India and land ownership certificate by which eligibility may be reasonably ascertained. o Aadhar Card which should be and Not Color Print from Web to confirm Indian Origin of the applicant (Except Washington DC Jurisdiction).

16 17 o A duly notarized affidavit to that effect. o A copy of an Ancestral Nativity Certificate issued by an Indian official or magistrate with sufficient authority to confirm Indian Origin of the applicant. Only for Chicago and Houston Jurisdiction - Please send only colored Only for New York Jurisdiction - Do not send. Please send only colored notarized copy of Indian passport. The decision to whether accept this type of document as proof of Indian Citizenship will be made by the Embassy/Consular Officer and will be FINAL in such cases. PARENTS/GRPARENTS PROOF OF IGIN If the applicant does NOT HAVE an Indian Passport and is applying for OCI on the basis that his parents and/or grandparents were Indian Nationals, then the applicant must enclose the following documents:- o of Proof of both parents current nationality and previous nationality. o Applicant s birth certificate (in which your parents name is mentioned). o If the applicant is applying as a grandchild of an Indian National, then the applicant must submit his birth certificate as well as his parents birth certificate. All submitted birth certificates must show the parents names clearly so that it establishes the relationship with the applicants parents and grandparents, o Copies of first three and last two pages of BOTH parents /grandparents Indian passport, o of BOTH parents /grandparents domicile certificate issued by the competent authority, o of BOTH parents OCI cards/ grandparents OCI cards including information pages on OCI cards (Except Washington DC Jurisdiction). Only for Washington DC Jurisdiction - Please send. Note: If parents /grandparents have OCI/PIO cards, it is mandatory to give copies of BOTH Indian Passport & OCI/PIO Cards. NATURALIZATION CERTIFICATE If minor gained naturalization through his/her parents and possess no separate naturalization certificate, then copies of the parents

18 19 naturalization certificates and a statement signed by BOTH parents is required stating the circumstances in which their child received his/her naturalization status and why the child does not possess his/her own certificate. If you acquired US Nationality through Naturalization, a copy of your Naturalization Certificate is required. If you do not have an Indian passport and are applying for OCI on the basis that the parents and/or grandparents were Indian nationals then PARENTS / GRPARENTS NATURALIZATION CERTIFICATE IS NEEDED. If the applicant has more than one NATIONALITY after having INDIAN Citizenship, please provide all NATURALIZATION CERTIFICATES or PROOF OF NATIONALITY from all countries where the applicant has held citizenship. The US Naturalization Certificate should state FMER NATIONALITY AS INDIA. Certificates that state Country of Birth as India CANNOT be accepted. If copy is not available, then a Certified True copy from local USCIS office. Only for New York Jurisdiction - Do not send notarized Please send only colored AFFIDAVIT F CHILD S NATURALIZATION BY PARENTS ONLY IF MIN APPLICANT CANNOT PROVIDE NATURALISATION CERTIFICATE AS HAD ACQUIRED US CITIZENSHIP ON THE BASIS OF PARENTS: Applicant must provide DULY Signed and Joint Statement by Parents (Visit Form section to download the format). PROOF OF RENUNCIATION OF INDIAN CITIZENSHIP: If you ever held an Indian Passport, it is MATY to provide proof of Renunciation of Indian Citizenship along with your OCI application. One of the following MUST be produced as proof:- o Canceled Indian Passport with stamp stating the passport is canceled due to obtaining US citizenship. o Certificate of renunciation issued by the Embassy/Consulate of India.

o Deemed surrender certificate issued by the Embassy/Consulate of India. 20 21 22 23 24 Not Required for Atlanta Jurisdiction. For example if changing to a NEW CITIZENSHIP, then a certificate providing evidence of the new naturalization is required. NAME CHANGE DOCUMENTATION (If Applicable) If your name in any of the supporting documents provided is different in any way from the name in the US PASSPT, a copy of ONE of the following documents must be provided:- o Marriage Certificate showing the name changes if the name was changed at marriage. o Official Court Order for the name change. o Endorsement of name change during naturalization process. If in case the middle name was changed to an initial then a notarized statement letter is required. CKGS Disclaimer Form This must have Signature of the applicant.
