To begin the process, please contact Sarah Hale, Short-Term Missions Trip Coordinator, at (408) 230-6472 to discuss your possible future trip. 2. Next, complete this application at least two months in advance, and call Sarah to set up a time for an interview. If you are filling out an application form through another sending organization, that form may be sufficient for use with Hillside. Please contact Sarah in advance to discuss this. 3. To ensure the best success, plan early! The Missions Team will consider a candidate s timeliness, organization and communication as key indicators of responsibility. 4. Upon approval of the application, the Missions Team will provide further information about the next preparation steps. 5. DO NOT solicit any funds for support and/or make any commitments on behalf of Hillside Church without prior approval. 6. Candidates will need to provide a $50.00 deposit with the application if the airfare is to be purchased through Hillside. The $50.00 deposit is not refundable after the application has been approved and the ticket purchased. 7. Please keep Sarah updated regarding any changes in your plans or circumstances. Basic Requirements: Application for Short-Term Missions Trip The short-term mission worker needs to: 1. Have a saving faith whereby they are enabled to believe in the salvation of their souls via the work of the Holy Spirit and the Word of God. 2. Maintain an active and obvious commitment to Jesus Christ. 3. Be active at Hillside at least one year prior to the date of departure. 4. Be at least 16 years of age and/or meet other age requirements. 5. Possess the ability and desire to share Christ with others. 6. Demonstrate maturity and proven dependability, which is seen by peers and church leaders in his/her area of ministry. 7. Participate in required training by Hillside Evangelical Free Church and/or mission agency. 8. Receive parental approval (for candidates under 18 years of age). 9. If applicable, receive approval from sponsoring agency. 10. Attend all trip meetings. Absences must be cleared with the Trip Team Leader. 11. If you will be serving through a mission agency, it is your responsibility to communicate personally with them to find out the proper procedures required of you. Support Guidelines: 1. Typically, if a candidate is a part of a Hillside Church sponsored mission team, the Hillside Missions Team may cover up to 50% of the mission trip budget. If a candidate is being sent via a separate mission agency, the Hillside Missions Team may cover up to 33% of the mission trip. Candidates are encouraged to sacrificially pay what they can and obtain the balance of the budget by requesting support from friends and family. 2. All letters for support must be approved by the Missions Team. Please schedule time well in advance to review the letter. 3. In general, solicitation for support is to be sought from friends and family members of the candidate. Candidates may not make requests to the general Hillside Church membership, nor solicit Hillside Church functions such as Sunday schools classes, Bible studies, and so forth. Candidates should solicit funds solely from individuals with whom relationships have been established. Page 2 of 9 Revised 10/16/07

Any short-term missions program that is supported by Hillside Evangelical Free Church is committed to upholding the Free Church doctrine and furthering the purposes of the mission. Therefore, every applicant is required to be in agreement with the following statements. Please read the following carefully and sign at the bottom if in agreement. If there is any area that you have difficulty agreeing with, please indicate on the reverse of this form. STATEMENT OF FAITH The following twelve items constitute the statement of faith of this church. 1. We believe the Scriptures, both Old and New Testaments, to be the inspired Word of God, without error in the original writings, the complete revelation of His will for the salvation of men and the Divine and final authority for all Christian faith and life. 2. We believe in one God, Creator of all things, infinitely perfect and eternally existing in three persons: Father, Son, and Holy Spirit. 3. We believe that Jesus Christ is true God and true man, having been conceived of the Holy Spirit and born of the Virgin Mary. He died on the cross, a sacrifice for our sins according to the Scriptures. Further, He arose bodily from the dead, ascended into heaven, where, at the right hand of the Majesty on High, He now is our High Priest and Advocate. 4. We believe that the ministry of the Holy Spirit is to glorify the Lord Jesus Christ and, during this age, to convict men, regenerate the believing sinner, indwell, guide, instruct and empower the believer for godly living and service. 5. We believe that man was created in the image of God but fell into sin and is, therefore, lost, and only through regeneration by the Holy Spirit can salvation and spiritual life be obtained. 6. We believe that the shed blood of Jesus Christ and His resurrection provide the only ground for the justification and salvation for all who believe, and only such as receive Jesus Christ are born of the Holy Spirit and, thus, become children of God. 7. We believe that water baptism and the Lord's Supper are ordinances to be observed by the Church during the present age. They are, however, not to be regarded as a means of salvation. 8. We believe that the true Church is composed of all such persons who through saving faith in Jesus Christ have been regenerated by the Holy Spirit and are united together in the body of Christ, of which He is the Head. 9. We believe that only those who are, thus, members of the true Church shall be eligible for membership in the local church. 10. We believe that Jesus Christ is the Lord and Head of the Church and that every local church has the right, under Christ, to decide and govern its own affairs. 11. We believe in the personal and premillennial and imminent coming of our Lord Jesus Christ, and that this "Blessed Hope" has a vital bearing on the personal life and service of the believer. 12. We believe in the bodily resurrection of the dead: of the believer to everlasting blessedness and joy with the Lord; of the unbeliever to judgment and everlasting conscious punishment. STATEMENT OF MISSION PURPOSE The Evangelical Free Church Mission exists to glorify God through making disciples of Jesus Christ and incorporating them into congregations with the same purpose.
