The Eecutive Physical Program at NYU Langone provides patients with a comprehensive and convenient annual health eamination. Through our network of ambulatory care offices, we offer private and attentive medical eaminations tailored to each individual s medical needs. Overview The Eecutive Physical Program provides for world-class amenities and an ecellent patient eperience by NYU Langone s physicians and staff. Our program offers: A personalized health care eperience that assures convenience, privacy, comfort and attention with minimal disruption to your daily routine Epert clinical care provided by NYU Langone s world-class practitioners in serene and elegant surroundings Assistance with local transportation and accommodations (including transportation between sites when services are not available at a single location), wireless internet access, and coordination of all appointments and referrals. The Program NYU Langone Medical Center s Eecutive Physical program provides comprehensive diagnostic and multispecialty care generally in a single location. NYU Langone s Ambulatory Care network offers convenient eam locations throughout the metropolitan area. Our physician teams are comprised of specialists in internal medicine, cardiology, gastroenterology, sports medicine, orthopaedics, pulmonology, dermatology, radiology and women s health services. Additionally, we can facilitate connections with any specialty across NYU Langone s wide range of medical and surgical epertise. The Eecutive Physical program offers three levels of health assessment from which to choose ensuring that the program is tailored to the individual s age and health risks. These packages allow for fleibility in selecting the level of care that best meets your individual health evaluation needs. You will first be asked to choose a package level and location; then your physical eam and other services will be scheduled. Prior to your appointment, you will receive electronic notification of the eam date and location, as well as all necessary paperwork and instructions. In advance of your appointment, you will be provided with a comprehensive medical history questionnaire to complete. Our medical team will review the questionnaire, noting any outstanding risk factors, eisting health conditions and family history. This information assists our team in identifying any additional tests or screenings recommended for you to achieve a thorough eecutive physical. On the day of your eam, you will arrive at your selected location and will be greeted by our Eecutive Physical program staff. During your visit, you will meet with NYU Langone s board-certified physicians, a comprehensive medical history will be taken and a complete physical eam will be performed. You will undergo the various tests included in your program. For your convenience, a team member will escort you through our facility throughout your visit with us. Three Levels from which to choose: Gold, Platinum and VIP. Gold Package The Gold Package includes the following: Comprehensive medical history and physical eam Comprehensive lab assessments Full body skin eam Electrocardiography (EKG) Chest X-Ray (when clinically indicated) Pulmonary function assessment (spirometry) Hearing test and vision screening Written summary of results Phone consultation Physician Related health educational materials Comprehensive Medical History and Physical Eam A comprehensive medical history will be taken and physical eamination performed. Comprehensive Lab Assessments Complete blood analysis, including cholesterol profile and urinalysis will be conducted. The following laboratory tests are included in an Eecutive Physical: Urinalysis to detect kidney disease, bladder disease, and infection CBC with differential to detect anemia, blood diseases, and infection Complete chemistry profile to detect kidney disease, diabetes, and liver disease Lipid panel to view a complete profile of blood cholesterol levels Thyroid stimulating hormone to detect an under or overactive thyroid C-reactive protein to detect an inflammatory marker that may signify risk for coronary artery disease Prostate-specific antigen (PSA) to detect prostate cancer or prostate enlargement. This test is sometimes indicated for males over the age of 40 or those with a significant family history. Hemoglobin A1C to evaluate lifetime risk of acquiring diabetes, a multi-factorial disease that is influenced by both genetics and lifestyle choices, including diet and activity level Vitamin D which is important for healthy bones and may also have implications for the immune system and cardiovascular system functions Full body skin eam A complete full body skin eamination by an NYU Langone dermatologist. The dermatologist will check for skin cancer and eamine rashes, moles and other lesions. Electrocardiography (EKG) Electrocardiography (EKG) to screen for a number of potential cardiac issues including irregular heart rhythms, damage to the heart muscle, enlargement of the heart s chambers, mineral imbalances in the blood and detection of damage to the heart muscle. Chest X-Ray Chest X-ray to screen for lung cancer, infection, and other lung diseases (for those individuals where it is clinically indicated). Pulmonary Function Assessment Spirometry to measure lung function. This pulmonary function test specifically measures the amount (volume) and/or speed (flow) of air that can be inhaled and ehaled. Hearing Test and Vision Screening Hearing (audiometry) test and vision screening using the Snellen Chart are performed to asses current vision and hearing levels to determine if a more in depth evaluation of these functions is necessary. Written Summary of Results The Eecutive Physical program staff will collect all eam, laboratory and test results and produce a health/physical report designed to assess your health and well being. This report will be delivered to you within five business days of your visit. The written report will include: Medical history and eamination findings Available labs and study results Relevant fitness and preventative health educational materials The Eecutive Physical Program will send a copy of this report to your primary care physician and/ or other providers involved in the maintenance of your health, upon your request. Phone Consultation with Physician After you have received the comprehensive written summary of your physical, you will have the opportunity for a follow-up phone consultation with your Eecutive Physical physician to discuss the results at your convenience. NYULangoneHealth Online Health Connection You will be given access to NYULangoneHealth which offers you personalized and secure online access to portions of your medical record. It enables you to securely use the Internet to help manage and receive information about your health.
Platinum Package The Platinum Package includes all of the services offered in the Gold Package plus the following additional tests: Echocardiography Echocardiography which is a test that uses sound waves to produce images of your heart. This commonly used test allows your doctor to see how your heart is beating and pumping blood. Your doctor can use the images from an echocardiogram to identify various abnormalities in the heart muscle and valves. Stress Echocardiography Stress echocardiography which is a test that helps diagnose heart disease with the help of ultrasound images. Following eercise or other stress to the heart, the images reveal parts of the heart that may not be receiving enough blood oygen because of blocked arteries. VIP Package The VIP Package includes all of the services included in the Platinum package plus the following additional test and services: Screening Colonoscopy Screening Colonoscopy which is recommended for persons over the age of 50 or earlier, if there is a family history of colon cancer Note: NYU Langone Medical Center anesthesia and pathology services associated with the screening colonoscopy are included in this package. Physician Consultation with Specialists Consultation with an NYU Langone cardiologist to review findings from the Eecutive Health Physical and related diagnostic testing. The cardiologist will make specific recommendations for you for maintaining a healthy lifestyle and improving your health. Consultation with an NYU Langone gastroenterologist to review findings from the Eecutive Physical including screening colonoscopy. The gastroenterologist will make recommendations based on their findings. Additional services available with any package through NYU Langone Medical Center (additional fees apply) Aesthetic Surgery Consultation A complete consultation with an NYU Langone aesthetic surgeon. We offer a full range of aesthetic procedures (surgical and non-surgical) through NYU Langone specialists. Specific services can be arranged upon request.
