Instruction page for completion of Identity verification, third party determination and politically exposed foreign persons (PEFP) form (Life insurance) Complete this form for all universal life and permanent life insurance products. For non-registered investment and wealth products: accumulation annuities (including segregated funds), guaranteed investment certificates (GICs), payout annuities, mutual funds and linked deposit notes, complete form E4105. To be completed with the understanding that Sun Life Financial will rely on the information to conduct customer due diligence required under the Proceeds of Crime (Money Laundering) and Terrorist Financing Act and to satisfy applicable regulatory requirements. In order to be exempt from completing the E4104, an entity must meet the following criteria as stated in Sections 62(2)(m) and (n) of the Proceeds of Crime (Money Laundering) and Terrorist Financing Regulations: (m) The entity must be a public body, or a corporation that has minimum net assets of $75 million on its last audited balance sheet and whose shares are traded on a Canadian stock exchange or a stock exchange designated under subsection 262(1) of the Income Tax Act (Canada), and operates in a country that is a member of the Financial Action Task Force; or (n) The entity is a subsidiary of a public body or a corporation referred to in paragraph (m) and the financial statements of the entity are consolidated with the financial statements of that public body or corporation; From Section 1(2) of the Proceeds of Crime (Money Laundering) and Terrorist Financing Regulations, public body means: (a) any department or agent of Her Majesty in right of Canada or of a province; (b) an incorporated city, town, village, metropolitan authority, township, district, county, rural municipality or other incorporated municipal body or an agent of any of them; and (c) an organization that operates a public hospital and that is designated by the Minister of National Revenue as a hospital authority under the Excise Tax Act, or any agent of such an organization. If the entity is exempt, check the YES checkbox and complete sections 5, 6 and 7 of the E4104, as applicable. If the entity is not exempt, check the NO checkbox and complete all sections of the form that apply to the type of applicant/owner on the application. Refer to the diagram on page 5 for an example of how to complete this form for the multiple layers of ownership of the entity applicant. If completing a paper form and additional space is needed for any section of this form, complete the applicable sections on a new E4104 form, along with sections 5, 6 and 7 of that same E4104. 1 Identity verification 1.1 Individual, sole proprietor or signing officers of a corporation/partnership/not-for-profit entity/other non-corporate entity (including trust or estate) Complete if the applicant/owner is an individual, which includes a sole proprietor, including the FATCA self-certification question 'Are you a U.S. resident for tax purposes (which includes a U.S. citizen)?'. Complete for signing officers of a corporation, partnership, not for profit entity or other non-corporate entity (including trusts or estates). Completion of the International tax classification for an entity (4545-E) form is mandatory. Acceptable documents to view can be a driver s licence, birth certificate, passport or permanent resident card. A valid document is an original (not a copy), has a unique identifying number, has not expired (if document has an expiry date) and is issued by a Canadian federal, provincial or territorial government for that individual. Submit the Certificate of incumbency (E4207) form, the International tax classification for an entity (4545-E) form and paper copies of all relevant corporate documents (i.e. articles of incorporation, annual returns and shareholder agreements, etc.). 1.2 a) Corporation Complete name, detailed principal business (for example, Office Equipment Leasing or Residential Landscaping), registration number, date of incorporation and where the corporation was registered. Indicate the not for profit status of the corporation. Select either NO or YES. If YES, indicate whether the not for profit solicits public contributions, is registered with CRA and record the CRA registration number. 1.2 b) Directors of the board Complete if the applicant/owner is a corporation including not for profit corporations. For 'Detailed occupation/principal business', a detailed occupation or job title is required. Examples: Manager of Customer Service or Salesperson of Office Equipment. If 'Retired' is provided as the occupation, include details of the pre-retirement occupation. 1.2 c) Individual shareholders that own all or a portion of the entity applicant Record the name of each individual who has an ownership interest in the entity applicant. Each individual owner must be asked if they own 25% or more of the entity applicant. In each case where the individual does own 25% or more of the entity applicant, the detailed occupation (for example, Manager of Customer Service or Salesperson of Office Equipment) and address sections must be completed in addition to the name section. If 'Retired' is provided as the occupation, include details of the pre-retirement occupation. Examples: Retired Building Contractor or Retired Teacher. 1.2 d) Is the corporation identified above in section 1.2 a) owned in whole or in part by another entity? Select either NO or YES. If YES, indicate whether the not for profit solicits public contributions, is registered with CRA and record the CRA registration number. List Directors of the board (not for profit entity). 1.4 b) Trustees/trust beneficiaries/trust settlor (payor)/estate executor/estate beneficiaries Complete name, address and detailed occupation (for example, Manager of Customer Service or Salesperson of Office Equipment), for each trustee or estate executor. If 'Retired' is provided as the occupation, include details of the pre-retirement occupation. Complete name, address and detailed occupation (for example, Manager of Customer Service or Salesperson of Office Equipment) for each trust beneficiary or estate beneficiary. If 'Retired' is provided as the occupation, include details of the pre-retirement occupation. Complete name, address and detailed occupation (for example, Manager of Customer Service or Salesperson of Office Equipment) or type of business for each trust settlor. If 'Retired' is provided as the occupation, include details of the pre-retirement occupation. 2 Third party determination A third party is any person or entity, that is not the owner, who may give instructions, make use of or make payments to the contract/policy. Some examples of third parties include: a power of attorney acting on behalf of the applicant/owner. a person or entity paying for the policy who is not the applicant/owner. A NO or YES answer must be given to the third party question. It cannot be left blank. If YES, provide additional information about the third party. The address should be the residential/business address of the third party (include the street, city, province, country and postal code). PO Box or General Delivery is not acceptable. Refer to the form (E4104) for details. A NO or YES answer must be given to the PEFP question. It cannot be left blank.
