Question: Unless I receive an updated payroll estimate by class code from you I will use the same payroll figures for computing premiums as your expiring policy. See Workers Compensation and Employers Liability Policy. 3. Question: Directors and officers insurance is not written on standardized policy forms like many other coverages. For that reason I need to review your current policy and quote at a minimum the coverage you currently have. Please see the Director s and Officer s Insurance Coverage. 4. Question: Please forward me a copy of your policy with National Union AND 3 year currently valued loss history from the company commonly called loss runs. The underwriter will also need to review a copy of your most recent audited financials. Please see the Non-Profit Director s and Officer s Insurance Coverage. 5. Question: Attached is the application for nonprofit D&O I will need completed and signed by the appropriate officer. All application forms will be completed once a vendor has been awarded the contract resulting from this RFQ. 6. Question: We are wanting to see if the request for services is for Property/Casualty/Auto/Workers Compensation training and development or benefits/wellness? DeKalb County, GA is only requesting coverage as listed in the RFQ documents for Workforce Development non-employee training program participants and the Workforce Development Board Members. Please review the scope of work on page 3 of the RFQ. 8. Question: Current risk management firm Dekalb is using? DeKalb County has obtained insurance coverage for its County employees from Travelers Insurance and American Home Assurance Co. Please be aware that this RFQ is only requesting coverage as listed in the RFQ documents for Workforce Development non-employee training program participants and the Workforce Development Board Members. See also Question and Answer #7. 9. Question: Current carriers Dekalb is using for the insurance policies? See Question and Answer #7 and #8. 10. Question: Any specific areas Dekalb has of interest for training and procurement? DeKalb Workforce Development has no interest in training and procurement. 11. Question: Are you asking for the bids to include premiums for each of the following policies: Workers Compensation, Employers Liability, Directors and Officers? If so, we will need much more information on the payrolls, losses, budget. Or are you only asking for the fees associated with the risk management company for their services throughout the year? DeKalb Workforce Development is seeking request for quotes that include the insurance coverage premiums as well as the Insurance Broker/Agent fees and/or commissions for providing the services as outlined in the RFQ.

Question: Are you requesting pricing on the premium for the workers compensation, employers liability, and directors and officers coverage or just the price for the agency services associated with the purchasing of those policies? See Question and Answer #11. 13. Question: We will need: the current payrolls and class codes on the workers compensation policy Current hard copy of the loss runs Number of board members and annual budget See the Workers Compensation and Employers Liability Policy and the Non- Profit Directors and Officers Insurance Coverage for additional information to assist in providing the requested information. The Workforce Development Board has 23 voluntary members. These members neither participate in payroll nor do they approve the processing of payments. The Workforce Development Board has an annual budget of $5.3 million. A hard copy of the loss runs is not available at this time. As indicated on page 3 of the RFQ, DeKalb Workforce Development has no losses over the last 5 years with the current insurance carriers. 14. Question: The RFP seeks and has fields on Attachment B to include the broker fee/ commission amount. However, there is also a blank field for the premiums. As there are no applications, coverage specifications (payroll amounts, revenue, etc.), it would be difficult to send to market or obtain formal premium quotations on your behalf. Please clarify if you are seeking broker fee/ commission quotes, policy premiums per policy or both See Question and Answer #11. 15. Question: It has always been our understanding that Workers Compensation Insurance is the same as Employers Liability Insurance. Are you seeking something in addition to Workers Comp & Director s and Officers Liability? See Question and Answer #7 and #9. 16. Question: What are the annual payrolls used to currently rate your Workers Comp policies? This can be found on the current Workers Comp policy. See the Workers Compensation and Employers Liability Policy. 17. Question: Are there different class codes used to rate the current employees on the Workers Comp policy? This will also be found on the current policy. We will need to know the class code, and subsequent payroll allocated to each.
See Question and Answer #16. The pay rate for non-employee program participants range from $8.00 - $15.00 per hour for 35 hours per week for a maximum of 12 weeks. 18. Question: For Director's and Officer's Liability, I will need to know the current number of board members. See Question and Answer #13. 19. Question: Also, for D&O, do you have a current Financial Statement that you can share? See County Audit Report and Question and Answer #13. 20. Question: Lastly, I see on the Request that you state "No Losses". Do you have a "Loss Run" report from both Hartford and National Union Fire Insurance that you can provide?
