COUNSELOR INFORMATION Disclosure Statement The following is a disclosure statement for the counseling department at Denver Christian School, which includes the following counselors: Jennifer L. Jackson, M.A., LPC: I am a Licensed Professional Counselor (#3899) in the state of Colorado receiving my license in November 2004. My degrees include: o B.A., Psychology/Youth Ministry, Colorado Christian University, 1998. o M.A., Counseling, Colorado Christian University, 1999. I have been a School Counselor with Denver Christian School for 15 years, which has included individual and groups counseling. I have been a supervisor for graduate level counseling interns for 6 years. I have been supervising unlicensed psychotherapists in pursuit of counseling licensure for the last 7 years. Benjamin J. De Nooy, M.A., NCC: I am a Registered Psychotherapist (#103248) in the state of Colorado. I am a graduate of Denver Seminary with a Master of Arts in Clinical Mental Health Counseling and am currently working toward licensure in the state of Colorado. My degrees include: o B.A., Psychology, Dordt College, 2009. o M.A., Clinical Mental Health Counseling, Denver Seminary, 2012. I served as a counseling intern during the 2011-2012 academic year with Denver Christian Schools K- 8 campuses where I conducted individual and group counseling. I have been a School Counselor at Denver Christian for 3 years. Any graduate level interns for the school year August 2014 to May 2015. 2. REGULATION OF PSYCHOTHERAPISTS The Colorado Department of Regulatory Agencies has the general responsibility of regulating the practice of licensed psychologists, licensed clinical social workers, 1

licensed professional counselors, licensed marriage and family therapists, certified school psychologist, and unlicensed individuals who practice psychotherapy. The practice of licensed or registered persons in the field of psychotherapy is regulated by the Mental Health Licensing Section of the Division of Registrations. The Board of Registered Psychotherapists can be reached at 1560 Broadway, Suite 1350, Denver, Colorado 80202, (303) 894-7800. As to the regulatory requirements applicable to mental health professionals: Registered psychotherapist is a psychotherapist listed in the State s database and is authorized by law to practice psychotherapy in Colorado but is not licensed by the state and is not required to satisfy any standardized educational or testing requirements to obtain a registration from the state. Certified Addiction Counselor I (CAC I) must be a high school graduate, complete required training hours and 1,000 hours of supervised experience. Certified Addiction Counselor II (CAC II) must complete additional required training hours and 2,000 hours of supervised experience. Certified Addiction Counselor III (CAC III) must have a bachelor s degree in behavioral health, complete additional required training hours and 2,000 hours of supervised experience. Licensed Addiction Counselor must have a clinical master s degree and meet the CAC III requirements. Licensed Social Worker must hold a master s degree in social work. Psychologist Candidate, a Marriage and Family Therapist Candidate, and a Licensed Professional Counselor Candidate must hold the necessary licensing degree and be in the process of completing the required supervision for licensure. Licensed Clinical Social Worker, a Licensed Marriage and Family Therapist, and a Licensed Professional Counselor must hold a master s degree in their profession and have two years of post- masters supervision. A Licensed Psychologist must hold a doctorate degree in psychology and have one year of post- doctoral supervision. PRIVACY RIGHTS & CONFIDENTIALITY Generally speaking, the information provided by and to the client during therapy sessions is legally confidential and cannot be released without the client s consent. There are exceptions to this confidentiality, some of which are listed in section 12-2

43-218 of the Colorado Revised Statutes and the HIPAA Notice of Privacy Rights you were provided, as well as other exceptions in Colorado and Federal law. These are situations where you, the client, are in serious risk to harm either yourself or others such as potential suicide, child abuse and neglect, or grave disability. You should be aware that, except in case of information given to a licensed psychologist, legal confidentiality does not apply in a criminal or delinquency proceeding. If a legal exception arises during therapy, if feasible, you will be informed accordingly. The Mental Health Practice Act (CRS 12-43- 101, et seq.) is available at: http://www.dora.state.co.us/mental- health/statute.pdf. There may be times when consultation with other mental health professionals is necessary, specifically when students are meeting with counselors outside of Denver Christian. Client confidentiality is still protected as an authorization for a release of information form must be obtained from the guardians for consultation to occur. 4. CLIENT RIGHTS & IMPORTANT INFORMATION In a professional relationship, sexual intimacy is never appropriate and should be reported to the board that licenses, registers, or certifies the licensee, registrant or certificate holder (State Grievance Board, as listed above). You are entitled to receive information from your therapist about the methods of therapy, the techniques used, the duration of your therapy (if known), and the fee structure. You can seek a second opinion from another therapist or terminate therapy at any time. 5. REGARDING DIVORCE & CUSTODY LITIGATION The therapeutic relationship is unique and has a foundation in trust. As such, when a counselor is asked to testify in court for such an issue as divorce and child custody, damage to the counseling relationship occurs, creating difficulty in the therapeutic alliance between therapist and client. If you are involved in divorce or custody litigation, our role as therapists is not to make recommendations to the court concerning custody or parenting issues. By signing this Disclosure Statement, you agree not to subpoena any counselors, interns or supervisors to court for testimony or for disclosure of treatment information in such litigation; and you agree not to request that we write any reports to the court or to your attorney, making recommendations concerning custody. The court can appoint professionals, who have no prior relationship with family members, to conduct an investigation or evaluation and to make recommendations to the court concerning parental responsibilities or parenting time in the best interests of the family s children. 6. SUPERVISION Our commitment to ensure quality of care is enhanced through appropriate supervision. The work of Benjamin J. De Nooy, M.A., NCC and any graduate level intern is supervised by Jennifer L. Jackson- Gustafson, M.A., L.P.C. Cases may be 3

discussed with her for supervision and guidance, but the same above guidelines of confidentiality will be firmly administered and maintained. Jennifer L. SESSION LENGTH, FEE INFORMATION & DURATION OF COUNSELING Scheduled counseling sessions are typically 30-45 minutes in length, but vary based on the student and counselor schedules. There are no fees required for counseling services at Denver Christian Schools. The counselor and supervisor reserve the right to recommend termination of counseling or referral of a client for a higher level of care when appropriate. 8. EMERGENCY PROCEDURES Our counseling team is only available on campus on a part- time basis during school hours. The hours that we can be reached during the 2012-2013 school year are as follows: Mondays and Thursdays from 8:00am- 1:00pm.
