LIST 1. Copies of all income tax returns filed by the corporation for the past three calendar years. 2. The original of all certificates of stock owned or beneficially held by the corporation, either alone or jointly with any other person or persons, in any corporation or corporations. 3. Copies of all financial statements issued by the corporation showing its financial condition during the past three calendar years and the current year. 4. All original notes, deeds, title certificates, mortgages, or security instruments of any kind showing or tending to show the existence of debts owed to the corporation or property owned by the corporation or property in which it has a security interest. 5. All writings of financial records in the possession or custody, or subject to the control of the corporation, showing or tending to show all money due it, with any records it may have showing or tending to show all persons who owe it money. 6. All insurance policies insuring loss to any property, real or personal, that the corporation owns, alone or jointly with any other person. 7. All bills of sale, deeds, contracts, or other documents showing transfer to any and all property executed by the corporation during the past three calendar years and the current year. 8. Any and all bank statements, canceled checks or other memoranda showing all transactions with any bank or banks with which the corporation did business during the past three calendar years and the current year. 9. Income tax records of the corporation for the current tax year. CORPORATE INTERROGATORIES IN AID OF EXECUTION 1. Provide the following information about yourself (the person answering on behalf of the corporation): What is your full name and address? What Office or Position do you presently hold with the Debtor Corporation: Set forth the offices or positions that you held in the past with the corporate debtor.

2. What are the names, titles and addresses of the other corporate officers? 3. Provide the following information about the Corporation's business: When was the business of the Corporation commenced? What is or was the nature of the business? Is the Corporation still in business? If so, where is its principal place of business? If not, when was the Corporate entity officially dissolved. How was it dissolved, by whom was it dissolved and who maintains the Corporate records? 4. What books of account does or did the Corporation maintain and by whom or under whose supervision have the records or books of account been kept in the last five years? 5. List all safe deposit boxes, bank or savings accounts kept or maintained in at any time over past two years by the corporation giving the name and address of the banks and the account numbers. If any of the accounts have been closed, give the date of their closure and the amount on deposit at the time of closure.

6. When was the last inventory of the Corporation's property taken, by whom, and what was the total dollar value? 7. Has the Corporation issued any financial statements within the past two years, and if so, to whom were they issued? (Provide names and addresses of the entity who received the statement and the date of the statement) 8. Answer the following questions about the locations where the Corporation conducts its business. At what addresses does the Corporation conduct its business, indicate for each location whether the corporation owns or leases the premises. If the premises is leased, what rents are paid and to whom? (Provide name, address and amount of rent) 9. What assets does the Corporation have at the present time? 10. Are there any mortgages or other liens on the Corporation's property? If so, describe the property, its location, its value, the amount of the mortgage or lien, and the name and address of the holder.

11. What repayments of loans has the Corporation made within the past five months and to whom? (Provide name and address) 12. What property has the Corporation transferred or otherwise disposed of during the past year other than in the usual course of business? 13. Has the Corporation assigned any of its accounts receivable and if so, to whom? (Provide name and address) 14. Has any execution or attachment been levied against the Corporation's property within the past five months and if so, give the particulars including a description of the property seized and the name and address of the person causing such levy. 15. List all personal withdrawals or payments from the Corporation which have been made to officers, directors or Managing Executives during the past year. 16. Does the Corporation pay the premiums of any life insurance or annuity policies for any stockholder, officer, director, or key employee or a member of the family of any such person?

If so, what are the names of the companies and the numbers and amounts of each policy? Who are the beneficiaries in each policy? What is the present cash value of each such policy? 17. List all autos, trucks, trailers, tractors, airplanes, boats, motorcycles, and motorized equipment owned by the Corporation providing a complete description of each, where it is located, the serial number, the license number, and nay outstanding balance due on its purchase price and the name and address of the creditor to which said amount is owed. 18. Does the Corporation own any real estate? If so, where is it located and what is its value? List any mortgage amount due on said property and the name and address of the mortgage. 19. Does the Corporation have shares or a proprietary lease in a cooperative or condominium? If so, provide a complete description of the interest together with the location of the cooperative or condominium. 20. Does the Corporation own any chattel or real estate mortgages or have any interest in any mechanic s lien or other lien on property? If so, state where the documents may be found and in whose possession they are.

21. Does the Corporation lease any property to others? If so, provide a complete description of the property and its location. 22. Does the Corporation: receive royalties from any patent, copyright or invention, or own a seat in any stock, cotton, produce or other exchange, stocks, bonds, defense bonds, or other securities? If so, list and describe the item including its location and the person who has possession of it. 23. Did the Corporation ever borrow money and pledge or deposit as collateral security any property, real or personal? If so, state names and addresses of persons with whom such security was deposited and describe the collateral. 24. Does any person or legal entity owe the Corporation money? If so, list the name and address of each person or legal entity and the amount owed. If a judgment has been entered in the Corporation's favor, list the amounts, the name and address of the debtor, and the Court and Case Number in which the Judgment was rendered. 25. Are there any Judgments against the Corporation? If so, provide the names and addresses of those holding the Judgments and the amounts of each. 26. Is the Corporation a party or have any right or interest in any action now pending in any Court? If so, explain. 27. Does anyone hold any property or money in trust for the Corporation? If so, provide the amount

or a description of the property and the name and address of the person or entity holding same. 28.
