360 Degree Performance Review 360 degree Performance review assessment tool Complete 360 o assessment Estimated completion time for respondents: 15 to 20 minutes. } Who completes the 360 o assessment? The program director, members of the board of directors, direct (s), coworkers, and possibly clientele. The gym owner and / or the program director select and agree upon the list of those who will complete this portion of the performance review. These are individuals with whom the employee relates on a regular basis in the course of his or her work with the organization. The selection ideally provides representation across departments and levels where possible. } The 360 o assessment form is given or e-mailed to the recipients. Respondents may fill out the form on screen and e- mail it back or fill out the printed form. Comments are encouraged. } The composite 360 o assessment will be made available to the employee at the time of his or her review.

Instructions for the 360 o Performance Review: } Respond to each of the statements in each of the categories on the pages that follow. Use the assessment system outlined below.
