Not able to obtain a driver license or permit. A reinstatement fee of $50 is due for an Admin Per Se Suspension action taken against the driver license or driving privileges. Permit has been restricted for 60 days as a result of an Admin Per Se or DUI Suspension. 34 License Canceled by MVD The annulment or termination of a driver license because of an error or defect or because the licensee is no longer entitled to the license. An application for a new license may be submitted for review by the department. 37 CDL Disqualification Driver s privilege to drive a commercial motor vehicle has been temporarily withdrawn for the time period indicated. Privileges can be reinstated after the disqualification period has ended and application for reinstatement is made; except in the case of a lifetime disqualification. 51 Identification Card Suspension 60 (CP) APS Suspension - Commercial Privileges Only 61 Ignition Interlock Device Identification card has been temporarily withdrawn during the period of the suspension and until application for reinstatement is made.
