Rapides Parish School Board Student s Contract For Alcohol Abuse Policy Violation FIRST OFFENSE - POLICY A I understand that I have violated the Rapides Parish School Board s Alcohol Abuse Policy and agree to comply with the provisions of Policy A. I agree to do the following: 1. I will not return to my home school or attend school events or activities until all of the requirements of Policy A have been completed and I have consent of my principal to return. 2. I will attend a state approved Alcohol substance abuse treatment center for 30 school days and upon discharge, provide verification of admission and discharge to my principal along with the aftercare recommendations of my substance abuse counselor. 3. I will participate in any aftercare treatment program as recommended by my substance abuse counselor during the remainder of the school year and my failure to comply will result in my expulsion and failure for the school year. 4. I will participate and comply with instructions of the school level employee assigned by my principal, the Drug Free School Coordinator, and any instructions of my principal during the remainder of the school year. My failure to comply will result in my expulsion from school. 5. I understand my rights to appeal this decision under the Rapides Parish School Board Policy which have been explained to me by the school principal. I will attend the alternative school for 30 school days and will attend and cooperate with any substance abuse counselor as required by Rapides Parish School System. 3. Transportation shall be provided by my parent or me to the alternative school and all counseling sessions. 4. After the 30 day period I will provide verification to my principal of my attendance at the alternative school and the aftercare recommendations of the substance abuse counselor. 5. I will participate and comply with instructions of the school level employee assigned by the principal, the Drug Free School Coordinator, and any instructions of my principal during the remainder of the school year. My failure to comply will result in my expulsion from school. 6. I understand my rights to appeal this decision under the Rapides Parish School Board Policy which have been explained to me by the school principal. DEFINITIONS: APPENDIX GLOSSARY (1) ALCOHOL - Any beverage of high or low alcoholic content as defined by Louisiana laws relating to the sale of alcoholic beverages; any medication preparation or solution containing alcohol when used or possessed for use as a mood or mind altering substance. (2) APPEAL - To apply for review of a case or particular issue to a higher tribunal. (3) DETENTION - Activities, assignments or work held before the normal school day, after the normal school day, or on weekends. (4) DRUG - Any Controlled Dangerous Substance listed in Schedule I, II, III, IV and V as provided by La. R.S. 40:964 as amended; also, any chemical substance used to produce mind or behavior altering effects when inhaled, ingested, or injected, and any such substance, although not actually so used, when possessed with the intent to use or distribute the substance to produce the said effects; including, but not limited to Rush, aerosol sprays, liquid paper and body or mind altering substances. (5) EMPLOYEE - Any and all school board personnel, including but not limited to teachers, teacher aides, principals/assistant principals, lunchroom workers, office and janitorial staff, maintenance workers and bus drivers. (6) EXPULSION - Unless otherwise defined as a permanent expulsion, and except as provided by Paragraph B(2) of R.S. 17:416, an expulsion shall be defined as removal from all regular school settings for a period of not less than one school semester, during which time the school board shall place the pupil in an alternative school setting; permanent expulsion is the removal of the student from all school settings in the parish, including alternative schools, for a definite or indefinite period of time as provided by the affirmative vote of the Rapides Parish School Board; a student permanently expelled from the Rapides Parish School System shall not be readmitted except upon affirmative vote of the Rapides Parish School Board. (7) IN-SCHOOL SUSPENSION - Is the removal of a pupil from his/her normal classroom setting but maintaining the student under supervision within the school. Pupils participating in an in-school suspension may receive credit for work performed during the in-school suspension. However, any pupil who fails to comply fully with the rules for in-school suspension shall be subject to immediate suspension. (8) POSSESSION - (1) Actual physical possession; (2) constructive possession being ownership or the presence of circumstances indicating that the drug, alcohol, weapon or other object is subject to the student s dominion or control; (3) use or consumption of alcohol or any drug while on school property or any school - sponsored event; (4) Appearance by a student at school, a schoolsponsored event or upon school property or a school bus or while in route to or from school after having consumed or ingested alcohol or a drug as demonstrated by objective manifestations such as breath odor, speech alterations, bizarre or unusual behavior, unsteadiness of gait or posture, admission of the student, chemical testing or like evidence, while not constituting actual physical possession of a drug or alcohol, shall nevertheless subject the student to a disciplinary recommendation as set out in the portions of this policy relating to substance abuse and alcohol. (9) PRINCIPAL - Chief Administrative Officer of a school or designee (10) SCHOOL - Any and all public schools owned and operated by the Rapides School Board, including but not limited to any public elementary, secondary or any school for the mental or physically handicapped, L.E.A.D., or Adult Education. (11) SCHOOL PROPERTY - All property used by the School Board for school purposes, or events 137

including but not limited to school playgrounds and school buses. (12) SCHOOL SPONSORED EVENTS - Any activity under the sponsorship of a Rapides Parish School. (13) SEMESTER - that period of time established by the board resolution adopting the beginning and ending of a semester as designated by the school board calendar approved for any academic year. (14) STUDENT- Any pupil enrolled in the Rapides Parish School System or actually attending a Rapides Parish School although not enrolled. (15) SUSPENSION - Is any denial of school attendance from the regular school setting. 138

RECEIPT AND STATEMENTS OF COMPLIANCE AS PROVIDED BY R.S. 17:235.2 Student s Name (Type or Print) We have received notification of the following Rapides Parish School Board Policies: Student Code of Conduct including: Discipline Student s Rights and Responsibilities Substance Abuse Internet Use Policy Harassment/Intimidation Policies Transfer Policies TOPS Core and TOPS Tech Curricula Graduate Exam GEE 21 Cyberbullying Policy I, the undersigned student, do hereby commit to do at least all of the following: A. Attend school daily except for excused absences; B. Arrive at school on time each day; C. Demonstrate significant effort to completing all required homework assignments, and D. Follow school, school board and classroom rules. We, the undersigned student and parent/guardian, certify that we understand that students shall be in attendance ninety-four percent (94%) of the required time. Based on the instructional calendar of 171 days, 380 minutes, students may not exceed ten (10) instructional days per year or five (5) instructional days per semester of absences. When a student has exceeded the allowable days of absence as defined by the 94% mandate, a committee appointed by the school principal will review the student s absences. The maximum number of student days is 171 days equivalent to 64,980 instructional minutes and equal to 380 minutes per day. This maximum attendance requirement must be met in order to receive credit. I further understand for all doctor s excuses and other absences, documentation must be submitted to the school within three (3) days of returning to school or it is not accepted. I, the undersigned parent or legal guardian, certify that I have read the rules and policies in the Student Code of Conduct and Policies Handbook from which this receipt was taken and I commit to do all of the following: A. Ensure that my child attends school daily except for appropriate excused absences; B. Ensure that my child arrives at school on time each day and remains at school all day unless there is an emergency situation; C. Ensure that my child completes all required homework assignments; and D. Attend all required parent/teacher or parent/principal conferences. I understand that under state law, the school system may be required to report to Families in Need of Services (FINS) serious or deliberate violations of these commitments. I further understand that if I and my child refuse to sign and return this receipt, the student shall be suspended until the receipt is signed and returned. I further understand that in the event the student is administered a drug or alcohol test at the school s direction in accordance with testing policies, our signature below is consent to persons administering the test for the release of all test documents and results to the Superintendent, principal or their designee.
