What does health care designed around you really mean? In a time when health care is rapidly changing, we promise to deliver affordable, quality care. Southern New Hampshire Health System, made up of Southern New Hampshire Medical Center and Foundation Medical Partners, believes that providing high quality health care in a cost-effective manner is crucial to the health and economic future of the greater Nashua community. Lower cost, higher value. We are working to make health care more affordable and to reduce the burden on employers and families. Driving increasing costs are Medicaid and Medicare reimbursement rates, which do not cover the cost of care provided to patients enrolled in those programs. Cost shifting (negotiating higher reimbursement rates from commercial insurance companies to cover the shortfall of Medicare and Medicaid) closes the gap but accelerates increases in the cost of health care insurance for employers. Lowest Costs in NH Southern New Hampshire Health System is making smart, targeted investments in technology and in coordination of care to help lower costs without jeopardizing patient safety or quality. t The Health System addresses the need for walk-in care in an alternative, lower cost model. Staffed by physicians, physician assistants and nurse practitioners, Immediate Care of Southern New Hampshire is open seven days a week to the general public. Immediate Care patients pay only a doctor s office co-payment; health plans pay the same amount they would pay if the patient went to any other physician practice. Unlike many urgent care centers in New Hampshire, Immediate Care costs patients the same as a visit to their doctor and avoids the expense of a trip to the emergency room. 4
Here when you need us. Southern New Hampshire Health System offers convenient access to services where our patients live and work. Our network of more than 200 primary care and specialty providers, along with Immediate Care facilities and Emergency Department services, ensure that the right care is available when our patients need it. Foundation Medical Partners Primary and Specialty Care The Health System offers nearly 60 primary and specialty care practices throughout Greater Nashua. A primary care physician is a patient s go to person for routine care. If specialized care is needed, primary doctors can easily connect patients with qualified specialists within The Health System. Immediate Care of Southern New Hampshire For non-life threatening conditions that develop on weekends, holidays, or other times a patient s primary doctor is unavailable, Immediate Care is available in Nashua, Hudson and Merrimack. Walk-in patients receive high quality, affordable care for minor illnesses and injuries, with laboratory and x-ray services on-site. Southern New Hampshire Medical Center Emergency Services For serious conditions and life-threatening emergencies, our Emergency Department in Nashua is open 24 hours a day, seven days a week. The Medical Center is verified by the state of NH as a Trauma Center, with an experienced staff qualified to respond to all emergencies. Innovative solutions have increased the level of care available in Nashua. For example, our physicians treating stroke patients have access to a TeleStroke program offering real-time consultations with Massachusetts General Hospital neurologists based in Boston. Emergency patients requiring more advanced care are transported to another hospital as necessary. 5

Advanced Care Options Patients can count on world-class care from Southern New Hampshire Health System. The Medical Center is home to a state-designated Trauma Center, a Level III Newborn Intensive Care Unit, and state-of-the-art operating suites that allow our surgeons to perform image-guided surgery with greater precision. We have a nationally accredited breast health program and our diagnostic tools and imaging services are nationally accredited for CT and PET scanning, MRI, ultrasound and more. But the higher level of care that The Health System provides is about more than technology. It s about our people and our strong relationships with renowned leaders in health care, including Massachusetts General Hospital for the areas of pediatrics, cancer, neurology, and trauma. EMR enhances patient care. Electronic Medical Records (EMR) can save time at the doctor s office and can be lifesaving in an emergency. Southern New Hampshire Health System invested in a sophisticated EMR system because we believe that when all of our providers have easy access to important health information, our patients receive higher quality, safer care. With one touch of a screen, each Foundation Medical Partners provider can quickly retrieve a patient s most up-to-date medical record, including images, lab and test results, and physician notes. Southern New Hampshire Health System patients benefit from EMR in critical ways: n n n Reduced duplication of diagnostic and lab tests Improved coordination of care among doctors and nurses Increased patient safety through easier tracking of medications Measuring Success The benefits of EMR go beyond tracking what has occurred during a doctor s office visit or hospital stay. The Health System uses EMR to securely share best practices among all of its providers and to gauge the success of preventive care measures like blood pressure checks and flu vaccines against national benchmarks. Health Reminders Foundation Medical Partners soon will implement Care Manager, a new EMR-integrated program designed to enhance preventive care measures for patients with chronic conditions like diabetes and coronary artery disease. With Care Manager, our patients will receive important health reminders, even if they haven t visited their doctors recently. n Enhanced communication between The Medical Center s Surgical and Emergency Departments, other hospital units, and physicians in the community 9

Quality Care Our Health System doctors and nurses follow nearly 400 protocols and checklists that promote best practices for treating many conditions. The Medical Center is consistently recognized for quality in the treatment of heart attacks, heart failure, and pneumonia, as well as preventing infections. These efforts are monitored and trended. We seize opportunities for improvement and strive to set the bar higher year after year. Publicly available performance results for a dozen care measures show The Medical Center scoring above 90% in all areas. 10
